Шумакова» Минздрава России, Москва, Российская Федерация В сложившейся ситуации дефицита донорских органов выполнение трансплантации сердца от так назы ваемых возрастных доноров является одним из реальных путей повышения доступности и увеличения количества пересадок сердца, прежде всего у потенциальных реципиентов, нуждающихся в неотложном ее выполнении. При подготовке и выполнении трансплантации сердца от возрастных доноров необходи мо учитывать повышенный риск развития ранней дисфункции сердечного трансплантата, наличия транс миссивного атеросклероза коронарных артерий, развития и прогрессирования васкулопатии сердечного трансплантата, что может негативно сказаться на ранней и отдаленной выживаемости реципиентов. При правильном подборе пары «донор-реципиент», эффективном разрешении ранней дисфункции сердеч ного трансплантата, своевременном применении лечебных мер, направленных на профилактику васку лопатии сердечного трансплантата, а также на коррекцию трансмиссивного атеросклероза коронарных артерий, ранние и отдаленные результаты трансплантации сердца от возрастных доноров сопоставимы с результатами трансплантации от доноров со стандартными критериями. Ключевые слова: сердечная недостаточность, трансплантация сердца.hearT TranSPlanTaTiOn frOM OlDer DOnOrS In the current situation of the shortage of suitable donor organs, heart transplantation from older donors is one of the ways to increase the performance of more heart transplants, particularly, in patients with urgent need of transplantation. While planning a heart transplantation from older donor one should consider increased risk of early cardiac allograft dysfunction, preexisting coronary artery disease, accelerated transplant vasculopathy which may adversely affect early and longterm survival of recipients. Subject to careful selection of donor-reci pient pairs, effective prevention and treatment of early cardiac allograft dysfunction, preexisting atherosclerosis and transplant vasculopathy the early and longterm survival of heart transplant recipients from older donors is comparable to heart transplantation from young donors.

Целью настоящего исследования была сравнительная оценка эффективности ингаляционного оксида азота и комби нации ингаляционного оксида азота с сурфактантом BL в комплексной терапии ОРДС после операций с ИК. В иссле дование включили 53 больных в возрасте от 21 до 76 лет. Исследование выявило, что при ОРДС, осложняющем опе рации с искусственным кровообращением, назначение сурфактанта BL на фоне терапии ингаляционным оксидом азота повышает эффективность влияния ингаляционного оксида азота на артериальную оксигенацию, обеспечивает ускорение регресса нарушений оксигенирующей функции легких, сокращает продолжительность ИВЛ и укорачива ет сроки госпитализации в отделении интенсивной терапии.The present study was undertaken to comparatively evaluate the efficacy of inhalational nitric oxide alone and in combina tion with surfactant BL in the complex therapy for acute respiratory distress syndrome (ARDS) after surgery under extra corporeal circulation (EC). The study included 53 patients aged 21 to 76 years. It has revealed that in ARDS that compli cates operations under EC, the use of surfactant BL during therapy with inhalational nitric oxide enhances the latter's effects on arterial oxygenation, accelerates the regression of pulmonary oxygenizing dysfunction, and reduces the duration of artificial ventilation and the length of stay in an intensive care unit.Острый респираторный дистресс синдром (ОРДС) остается нередким осложнением опе раций с искусственным кровообращением (ИК). Терапия ОРДС всегда имеет комплекс ный характер и должна включать меры как по поддержанию артериальной оксигенации, так и направленные на регресс повреждения лёгоч ной ткани. Изучают различные методы респи раторной терапии ОРДС: ИВЛ с оптимальным уровнем положительного давления в конце вы доха (ПДКВ), методику «открытых лёгких», ИВЛ в положении на животе; ИВЛ с инверти рованным отношением вдоха и выдоха, высоко частотную осцилляторную ИВЛ и др.[1]. Ис следуют лекарственную терапию ОРДС с помощью глюкокортикоидов, внутривенных и ингаляционных лёгочных вазодилататоров, препаратов экзогенного сурфактанта [2]. Ак тивно обсуждаемой лечебной мерой при ОРДС является ингаляционный оксид азота (иNO). Хотя в мультицентровых исследованиях не ус тановлено снижение летальности от ОРДС при иNO терапии, ее настоятельно рекоменду ют использовать при жизнеугрожающей арте риальной гипоксемии [3][4][5].В последние годы за рубежом опубликова ны результаты отдельных экспериментальных и клинических исследований, показавшие пато генетическую обоснованность при ОРДС соче танного использования иNO, как селективного лёгочного вазодилататора, и препаратов экзо генного сурфактанта [6,7]. Шумакова» Минздрава России, Москва, Российская Федерация Сочетанная трансплантация сердца и почки является одним из возможных направлений эффективно-го лечения больных c сочетанием терминальной застойной сердечной недостаточности (ЗСН) и почеч-ной недостаточности. Выделяют одномоментную, комбинированную, или симультанную (simultaneous), трансплантацию сердце-почка (КТСП) от одного и того же донора, а также последовательные (staged) трансплантации сердца и почки от двух генетически различных доноров. ISHLT также отмечает рост числа ежегодно выполняемых КТСП -от 22 (1994 г.) до 97 (2012 г.). В статье приводится мировой опыт выполнения КТСП, обсуждаются показания к ее выполнению, в том числе у пациентов с диализ-незави-симой почечной недостаточностью. Отмечается, что особенностью КТСП по сравнению с изолирован-ной ТС и аллотрансплантацией трупной почки является меньшая частота и выраженность отторжения как сердечного, так и почечного трансплантата, а также частота возникновения болезни коронарных ар-терий пересаженного сердца. Приводится собственный опыт двух последовательных трансплантаций сердца и почки. Обсуждаются показания для одномоментной (симультанной) и последовательной транс-плантации сердца и почки. There are two types of combined transplantation of heart and kidney: 1) simultaneous heart-kidney transplantation (SHKT) from the same donor; 2) staged transplantation of heart and kidneys from two genetically different donors. The ISHLT registry in 2014 reported an increase in the number of SHKT over the years: from 22 in 1994 to 97 in 2012. World experience demonstrated excellent results of SHKT. Recipients of SHKT had superior survival, lower rates of acute cardiac and renal rejection compared to heart recipients. This article discusses the indications for simultaneous or staged heart-kidney transplantation in patients with dialysis-independent or dialysis-dependent renal failure, results and posttransplant survival of SHKT recipients. The author describes his own experience of 2 staged combined heart-kidney transplantations.
