Ведущие этиологические факторы неонатальных судорог (НС) изучались у новорожденных, которые были распределенына группы в зависимости от гестационного возраста (ГВ): I – 84 глубоко недоношенные с ГВ 28 нед. и менее, II– 52 с ГВ 2932 нед., III – 12 с ГВ 3336 нед., IV – 17 доношенные с ГВ 3741 нед. Клиникоинструментальноеобследование всех 165 пациентов определило следующие причины НС: 72,1% – церебральная гипоксияишемия;6,1% – периинтравентрикулярные кровоизлияния (ПИВК) III/IV степени; 9,7% – внутриутробные инфекции с поражениемЦНС; 9,7% – нейроинфекции; 1,2% – церебральные дисгенезии; 1,2% – генетические заболевания. При этом ПИВК IIIIVст. как причина НС отмечались только у недоношенных с ГВ до 32 нед. Среди доношенных увеличивалась роль нейроинфекций(23,5%). При обследовании детей с НС следует учитывать вероятность генетических заболеваний, преждевсего, при отсутствии очевидных причин раннего поражения ЦНС. Это важно, поскольку что в настоящее время возрастаютвозможности таргетной терапии при эпилептических синдромах генетической природы. Рассматриваются перспективыприменения леветирацетама у детей с НС и ранними формами эпилепсии.
The introduction of modern technologies and the widespread use of intensive care in neonatology have led to an increase in the survival rate of children, who were born deeply premature with a low, very low, and extremely low body weight. The purpose of the study: evaluation of the peculiarities of neuropsychic development and the frequency of occurrence of neuropsychiatric disorders by the age of 5-8 years in children, who were born deeply premature with extremely low, very low, and low body weight in comparison with healthy peers. We examined 122 patients aged from 5 years 0 months to 8 years 0 months, 36 were born with very low body weight, 36 with extremely low body weight, 50 with low body weight, and 30 children were included into the control group. To assess the main complaints in patients, a structured questionnaire for parents was used. To study emotional and behavioral disorders in children, T.Achenbach’s structured questionnaire for the parents was used. Results and discussion. Data from the T.Achenbach’s parents’ questionnaire indicated significantly more expressed and pronounced manifestations of isolation, psychosomatic disorders, anxiety, problems of socialization and thinking, attention disorders, problems of behavior and aggressiveness in the examined children. In the groups of the examined patients, compared with peers, were significantly higher scores on the scales of “Internalizing manifestations” and “Externalizing manifestations”. It was shown that profound prematurity and birth with extremely low, very low and low body weight are important predisposing factors for the neurodevelopmental disorders in children: autism spectrum disorders, attention deficit hyperactivity disorder, developmental dyspraxia, specific learning difficulties. Early diagnosis of these conditions determines the effectiveness of therapeutic measures with the need of individual approach and comprehensive medical-psychological-pedagogical support.
ASL (Arterial Spin Labeling) – a novel modality of MR angiography – is based on radio-frequency labeling of aqueous protons in the arterial blood; the method is used to monitor blood supply to organs, including the brain. So far there has been little information on the use of ASL in children with focal epilepsy, especially in the pre-surgery period.Aim:to evaluate the perfusion patterns in seizure-free children with drug resistant focal epilepsy (FE) using the ASL mode of MRI.Materials and methods.We studied the ASL data of 54 (23-boys/31 girls) patients with FE treated in the Dpt. of Neurology at the Russian State Children Hospital from 2015 to 2018. The patients’ age varied from 4 months to 17 years. All images were produced with a 3T GE Discovery 750W system.Results. We found several brain perfusion patterns in children with FE; among other factors, those patterns depended on the clinical status of the patient, i. e. the interictal period or the early post- seizure period. The main pattern of the interictal period was characterized by a focal decrease in perfusion located around a structural focus identified on MRI scans. In the early post-seizure period, there was an increase in the arterial perfusion in the area of a structural epileptogenic lesion.Conclusion.ASL-MRI is an effective diagnostic method providing more information on children with FE during their pre-surgery phase. The ASL modality needs further research to rationalize its wider use as a preferred diagnostic tool or as a combination with the more complex PET and SPECT.

У детей с эпилепсией значительно чаще, чем у ровесников, встречаются тяжелые формы нарушений нервно-психического развития: общая интеллектуальная недостаточность и расстройства аутистического спектра. Приводятся данные о частоте встречаемости общей интеллектуальной недостаточности и расстройства аутистического спектра (РАС) при эпилепсии. Рассматриваются четыре возможных варианта ассоциации эпилепсии и РАС: РАС и эпилепсия могут развиться одновременно, но независимо, и вызываются они разными причинами; РАС и эпилепсия взаимосвязаны, являются проявлением одного и того же генетического дефекта или раннего повреждения ЦНС; РАС -следствие эпилептического процесса, вовлекающего развивающиеся нейронные сети, ответственные за коммуникацию и социальное поведение; РАСрезультат реакции на изоляцию у детей с эпилепсией. Обсуждаются данные исследований, подтверждающих положительное влияние леветирацетама на поведение и когнитивные функции (память, внимание) детей с эпилепсией. Эпилепсия, дети, нарушения нервно-психического развития, эпилептические энцефалопатии, общая интеллектуальная недостаточность, расстройства аутистического спектра, леветирацетам.
