Adipose tissue as an endocrine organ synthesizes a large number of biologically active substances, adipocytokines, which have both local and systemic effects influencing the vascular wall, tissue sensitivity to insulin, glucose metabolism, and systemic inflammation. The data obtained from clinical and experimental studies demonstrate the close relationship between the imbalance of adipocytokines and pregnancy complications such as insulin resistance, gestational diabetes, and preeclampsia. In this connection, close attention of obstetrician-gynecologists and endocrinologists is focused on etiopathogenic aspects of the formation of gestational complications with metabolic disorders caused by an imbalance of adipocytokines with maternal obesity and to the search for markers of these disorders. The review presents the current literature data on adipose tissue hormones and their influence on the course of a gestational process.

Резюме Цель работы: изучить особенности увеличения веса тела в динамике гестационного процесса у женщин с различными исходными антропометрическими показателями. Материалы и методы: проведено проспективное исследование с участием 100 беременных. Обследование включало измерение роста, массы тела, определение индекса массы тела (ИМТ) с последующим вычислением прибавки массы в I, II и III триместрах беременности. Результаты. У женщин с избыточной массой тела и ожирением чрезмерная прибавка веса во время беременности встречается чаще, чем при исходном дефиците массы и нормальном весе. Наименьшая величина общей гестационной прибавки веса установлена у беременных с ожирением. В то же время для этой категории женщин характерны наиболее частые отклонения от рекомендованного ВОЗ гестационного набора массы тела. Заключение. Величина гестационной прибавки массы зависит от исходного ИМТ. С ростом ИМТ увеличивается частота чрезмерной гестационной прибавки массы. Materials and methods. This prospective study included 100 pregnant women; the measurements of their height, body weight, and BMI were made at the start and then on the 1st, 2nd and 3rd trimesters of pregnancy. Results. In the overweight/ obese women, an excessive weight gain during pregnancy is more common than that in the women with initial weight deficiency or those with normal weight. The least gestational weight gain was found in pregnant women with obesity. At the same time, the obese women showed more deviations from the WHO-recommended gestational body weight gain as compared with the women with normal or low initial weights. Сonclusion. The magnitude of gestational weight gain depends on the initial BMI. Aim. Conduct a correlation analysis of the dependence of risk factors in the etiopathogenesis of chronic venous diseases. Materials and methods. On the basis of the GBUZ TO “Maternity Hospital № 2” of the city of Tyumen, an analysis of the patients’ data was carried out in the form of a clinical examination and questionnaires. The average age of the examined patients is 26 years. The survey involved 468 women in the period 2017-2019. Results. Among the surveyed patients, 52,4% had CVD. Pregnant women with CVD have subjective symptoms and objective symptoms. Dilated veins exist for less than 5 years in 86,5% of pregnant women, and in 13,5% – for more than 5 years. Varicose veins, with a predominant localization on the left lower limb, were found in 7,5%, on the right – in 11,1%, and on both legs – 81,5%. Conclusion. The use of a statistical indicator made it possible to calculate the dependence of the appearance of a disease in the presence of a certain risk factor – in this case, the correlation is direct, the relationship is close (0,94); as well as to evaluate the prognosis for pregnant women prospectively and create complexes of measures to prevent venous diseases, taking into account the individual characteristics of patients.
Аннотация. Здоровое и безопасное питание является одним из важнейших факторов, способствующих благо-приятному течению гестационного процесса и обеспечивающих здоровье будущего ребенка. В статье представ-лены результаты анализа фактического питания и показателей состава тела 82 беременных женщин с различными типами распределения жировой ткани. Выявленные нарушения нутритивного статуса характеризуются несба-лансированностью пищевых рационов, дефицитом белка и углеводов и избытком жиров. Показано, что приме-няемый традиционно для диагностики ожирения антропометрический метод с вычислением индекса массы тела не во всех случаях позволяет выявить пациентов с висцеральным ожирением. Вместе с тем, именно висцеральная жировая ткань, выделяя целый ряд биологически активных веществ -адипоцитокинов, вызывает метаболические изменения в организме беременной, неблагоприятно влияющие на течение гестационного процесса. Комплекс-ное обследование, включающее изучение структуры рационов питания и нутритивного статуса позволяет вы-явить беременных со скрытым ожирением, относящихся в группу риска по развитию метаболических и гестаци-онных осложнений. Objective. To develop a model of an integral index for complex assessment of varicose vein risk factors and estimate the influence of pregnancy on the occurrence of chronic vein diseases. Varicose veins (VV) and chronic venous insufficiency are one of the most widespread human diseases. This pathology in pregnancy has a clear picture, which is quite different from varicose veins in non-pregnant women. Materials and methods. Analysis of 1974 individual medical cards of pregnant women and puerperas of Maternity Hospital № 2 for the period of 20162019 was carried out. Out of them, 456 cards had a diagnosis of varicose veins of the lower limbs. The method was based on statistical analysis and integral indices. Results. The following risk diapasons were determined: low risk (favorable prognosis) 0.440.85; medium risk (favorable prognosis is possible) 0.861.28; high risk (unfavorable prognosis) 1.292.13. The quantity of pregnancies is not an unfavorable prognosis for women. Long statistical loads give unfavorable prognosis (1.341.43); family predisposition is an unfavorable prognosis (1.66); lifting loads depending on the quantity in grams (1000 favorable sign; 500010 000 unfavorable (1.211.870)); wearing tight clothes is not an unfavorable prognosis (0.88); hypodynamia medium risk (1.14); diet low and medium risk (0.750.95), obesity high risk (1.242.15). Conclusions. The obtained results permitted to detect the most significant risk factors for varicose veins during pregnancy. Medium and high-risk indices are the following: long statistical loads, family predisposition, lifting loads, hypodynamia, diet and obesity. They influence the development of varicose veins. There is a possibility to present a prospective prognosis for pregnant women and develop a complex of measures to decrease risks for chronic vein diseases taking into account individual features of patients.
