Comparative analysis showed reliable advantages of vascular ligation during pancreatectomy.
The aim of the study was to specify peculiarities of the blood supply to the body and tail of the human pancreas determining topographic variants and vascular bed of the splenic artery.Material and methods. The artery basin of the pancreas body and tail was studied in 46 native and 42 organ complexes consisting of the pancreas, duodenum, spleen, retropancreatic tissue, celiac trunk with the main branches, a section of the superior mesenteric artery up to the entrance into the root of the small intestine mesentery with the preliminary artery injection by gelatin mass. The organ complexes were fixed in 10% formalin solution.Results. The study revealed two variants of the splenic artery topography in relation to the pancreas and determined the degree of tortuosity of the artery. In 31.7% of cases the vessel passed retro-pancreatically and in 68.3% suprapancreatically. Moderate tortuosity of the splenic artery was detected most frequently (44.82%); in 34.48% of cases severe tortuosity was detected; in 17.24% of cases there was no tortuosity in the artery. The correlation between area/convolution and the number of pancreatic arteries was found to be 0.864 (p=0.05), which is considered to be a high parameter according to the Cheddock scale. In severe tortuosity of the splenic artery 3.16±0.37 pancreatic arteries on average closed to the corpus caudal segment of the pancreas, in moderate tortuosity – 5.09±1.24, in straight passage – 9±1.22.Conclusion. The regularities revealed in the spleen artery topography, blood supply of the pancreas body and tail allow determining operational risks and tactics of surgical interventions on the corpus caudal segment of the pancreas.

Оптимизация хирургического лечения хронического панкреатита с учетом вариантов кровоснабжения головки поджелудочной железы.Материал и методы. Исследование выполнено в два этапа. На первом изучались экстраорганные артерии поджелудочной железы и двенадцатиперстной кишки методом препарирования 42 комплексов органов верхнего этажа брюшной полости. На втором, клиническом, этапе пациенты в зависимости от способа оперативного вмешательства разделялись на две группы. Пациентам группы А (32 человека) выполнялась резекция головки поджелудочной железы с продольным панкреатоеюноанастомозом по Фрею, в модификации 2003 г. с изменениями согласно полученным нами анатомическим данным. Пациенты группы В (44 человека) оперированы по классической методике Фрея в модификации 2003 г. В ходе ретро и проспективного исследования проведена сравнительная оценка результатов лечения.Результаты. Установлено, что передняя панкреатодуоденальная артериальная дуга имеет несколько вариантов расположения по отношению к панкреатодуоденальному комплексу. Однако источники артерий, формирующие дугу, и места их отхождения характеризуются относительным постоянством, что позволяет перевязывать элементы передней дуги вне измененных тканей поджелудочной железы до этапа иссечения паренхимы. Интраоперационная кровопотеря в группе А составила менее 400 мл у 24 пациентов (р<0,01), в группе В -у 22 пациентов (р<0,01). Осложнения в раннем послеоперационном периоде наблюдались у 18,75% пациентов группы А и 22,7% группы В. В отдаленном послеоперационном периоде (через 6 месяцев после операции) по визуально-аналоговой шкале (шкала ВАШ) абдоминальный болевой синдром в группе А 29 пациентов оценивали на уровне 1-2 (р<0,01), в группе В 31 пациент отмечал те же значения (р<0,01).Заключение. Сочетание предварительной перевязки элементов передней панкреатодуоденальной артериальной дуги с более радикальным иссечением рубцово-измененной паренхимы поджелудочной железы,позволяет оптимизировать хирургическое лечение хронического панкреатита.Ключевые слова: хронический панкреатит, осложнения, артерии поджелудочной железы, операция Фрея, абдоминальный болевой синдром, шкала ВАШ, качество жизни Objectives. Проведенное исследование показало, что головка поджелудочной железы и двенадцатиперстная кишка получают артериальную кровь как от передней, так и от задней панкреатодуоденальных артериальных дуг. При этом от задней дуги к кишке отходит большее количество ветвей, чем от передней, позволяя сохранять адекватное кровоснабжение двенадцатиперстной кишки при перевязке элементов передней дуги. Также выявлено, что по зоне кровоснабжения, образованию ветвей и анастомозов, нижняя и задняя артерии поджелудочной железы являются взаимозаменяемыми. Обе артерии в одном исследованном комплексе не встречались.Ключевые слова: хронический панкреатит, поджелудочная железа, кровоснабжение.Активная хирургическая тактика в лечении хронического панкреатита (ХП) в последние годы стала применяться в связи с неэффективностью консервативной тера-пии у 45,8% больных [2,4,9,11]. При вы-полнении оперативных вмешательств час-то не учитываются варианты топографи-ческой анатомии панкреатодуоденальных сосудов, что является причиной крово-течений, а сами операции оказываются технически сложными. При этом увеличи-вается количество послеоперационных осложнений, частота которых составляет 24,6%, а при рецидивах заболевания -до 57,4% больных [2, 3,9], по данным не-которых авторов до 88,2-90,4% [2, 3]. Хирургическая анатомия сосудов подже-лудочной железы и двенадцатиперстной кишки описана во многих работах [1,7,8]. Однако приведенные данные об источ-никах кровоснабжения этих органов, раз-норечивы и недостаточны. Многие авторы, уделяя внимание только анатомическим вопросам, упускают важные моменты топографии, имеющие принципиальное значение в практической хирургии [6,8].Целью данного исследования было изучение экстраорганных артериальных сосудов панкреатодуоденальной зоны и их значение при оперативном лечении хронического панкреатита.Материалы и методы Исследование включало в себя ана-томическую и клиническую части. Изуче-ние топографо-анатомических особенно-стей артериального кровоснабжения пан-креатодуоденальной зоны выполнено на 42 анатомических комплексах, состоящих из: желудка с брюшным отделом пищево-да с участком малого сальника (до ворот печени), желудочно-ободочной связкой, желудочно-селезеночной связкой, желу-дочно-поджелудочной связкой, двенадца-типерстной кишки, части тощей кишки с брыжейкой, поджелудочной железы с за-брюшинной клетчаткой, селезенки. Экс-траорганные артерии панкреатодуоде-нальной области и поджелудочной железы исследовали с использованием метода препарирования с предварительной инъ-екцией сосудов модифицированной мас-сой Тейхмана-Тихонова [8]. Для более полного изучения связей между системами артерий чревного ствола и верхней брыже-ечной артерии проводилась полихромная Российский медико-биологический вестник имени академика И.П.
