На настоящий момент накоплено достаточное количество сведений о секреторной функции адипоцитов, что ведет к признанию роли жировой ткани в регуляции функционирования сердечно-сосудистой системы. Изучения молекулярно-генетических основ функционирования жировой ткани может помочь более глубокому пониманию этиопатогенетических механизмов сердечно-сосудистых заболеваний и разработать эффективную стратегию их профилактики и контроля. В данном обзоре проанализирована информация об особенностях профиля адипокинов и цитокинов, секретируемых различными типами жировой ткани (подкожная, эпикардиальная, периваскулярная), а также об изменении экспрессии ряда ключевых генов в адипоцитах, полученных из различных жировых депо, у пациентов с сердечно-сосудистыми заболеваниями. This review includes the data from relevant articles describing genomic predictors of the non-infectious systemic inflammatory response syndrome (SIRS) resulting from cardiac surgeries, published before December, 2016, and archived in the PubMed and Google Scholar databases. Dysfunctional changes and remodeling of adipose tissue (АT) are associated with the formation of microcalcifications in the vascular wall. Biologically active substances synthesized by АT (adipocytokines) can act as promoters and inhibitors of vascular calcification development. The few available experimental and clinical studies do not fully explain the possible mechanisms of these effects.Aim. To study the relationships between the adipocytokine profiles of adipocytes in epicardial and perivascular AT with the severity of coronary artery calcification in patients with coronary artery disease (CAD).Material and Methods. A total of 125 patients with CAD aged 59 (53; 66) years were examined. The isolated adipocytes of subcutaneous adipose tissue (SAT), epicardial adipose tissue (EAT), and perivascular adipose tissue (PVAT), obtained during coronary artery bypass grafting, were used to determine gene expression and secretion of adipocytokines (adiponectin, leptin, and IL-6). Expression of adipocytokine genes was assessed using quantitative PCR with detection of products in real time (real-time qPCR); the concentration of adipocytokines in the culture medium was determined by enzyme-linked immunosorbent assay using R&D Systems kits (Canada). Coronary artery (CA) calcification degree was assessed by multislice spiral computed tomography (MSCT) method. The calcium index of CA was determined by the Agatston method using the Syngo Calcium Scoring software package (Siemens AG Medical Solution, Germany).Results. Massive coronary calcification (CC) had the highest prevalence (58.8%) in patients with CAD. The highest level of expression of the ADIPOQ gene in all types of fat stores was observed in patients with moderate/medium CС compared to those with massive CС; the maximum expression of ADIPOQ was observed in the culture of PVAT adipocytes. Expression of the LEP and IL6 genes in massive CC was higher, with the maximum values in the culture of EAT adipocytes relative to SAT and PVAT adipocytes. Decreases in the levels of ADIPOQ mRNA and its secretion, increases in the levels of mRNA of LEP and IL6 and their secretion in adipocytes of the EAT and PVAT were associated with the development of СС in patients with CAD.Conclusion. Proinflammatory adipokines produced by adipocytes of patients with CAD during hypoxia induced vascular calcification by stimulating oxidative stress, osteoblast differentiation, apoptosis, and proliferation of smooth muscle cells. Endothelial cells, when stimulated with proinflammatory adipocytokines, tended to transform into osteoblasts, which further aggravated the degree of vascular inflammation and calcification.

Необходимость увеличения ýффективности профилактических мероприятий обусловливает поиск маркеров, позволяющих оценивать индивидуальные риски развития заболевания. Среди таких маркеров наиболее перспективными являются полиморфные варианты ключевых генов, участвующих в патогенезе ФП. Атеросклероз занимает лидирующую позицию в структуре заболеваемости и смертности среди всей сердечно-сосудистой патологии как в России, так и во всем мире. Имеются свидетельства того, что в развитии эндотелиальной дисфункции, являющейся первым этапом атерогенеза, помимо классических факторов риска также играют роль соматические мутации, а в атеросклеротических бляшках и пораженных сосудах отмечается повышенный уровень различных ДНК аддуктов. Однако остается открытым вопрос о фундаментальных основах роли соматических мутаций в патогенезе атеросклероза, что наиболее важно в условиях увеличения генотоксической нагрузки на организм человека, особенно в регионах с развитой промышленностью. Целью данного исследования явилось изучение особенностей секреции проатеросклеротических цитокинов IL-6 и IL-8 и экспрессии их генов в культурах первичных эндотелиальных клеток коронарной и внутренней грудной артерий, различающихся по степени подверженности атеросклерозу и экспонированных мутагеном алкилирующего механизма действия митомицином С. Концентрация изученных цитокинов в культуральной среде и уровень мРНК соответствующих генов измерялись методами количественной полимеразной цепной реакции и иммуноферментного анализа в двух временных точках - непосредственно после 6 часов экспозиции клеток митомицином С (первая временная точка) и после 6 часов экспозиции клеток мутагеном с последующими сутками культивирования в культуральной среде без митомицина С (вторая временная точка). Установлено, что в первой временной точке в экспонированных митомицином С культурах наблюдается статистически значимое снижение секреции и экспрессии гена IL8, а также снижение уровня мРНК гена IL6. Во второй временной точке, наоборот, концентрация и уровень мРНК гена IL8, а также генная экспрессия IL6 резко возрастали в культурах, экспонированных митомицином С по сравнению с неэкспонированным контролем (p<0,01), причем эндотелиальные клетки коронарной артерии, наиболее часто поражаемой атеросклерозом, были более чувствительны к мутагенному воздействию, чем эндотелиальные клетки внутренней грудной артерии, в наименьшей степени подверженной атерогенезу. Таким образом, впервые в эксперименте in vitro показаны мутаген-индуцированные изменения характера секреции и экспрессии генов проатеросклеротических цитокинов IL-6 и IL-8 в культурах первичных эндотелиальных клеток различных артерий человека. Atherosclerosis is a leader in morbidity and mortality among all cardiovascular pathologies both in Russia and around the world. Nowadays there are evidences that somatic mutations in addition to classical risk factors can play a role in the development of endothelial dysfunction - the initial stage of atherogenesis, and atherosclerotic plaques are characterized by increased level of various DNA adducts. At the same time, a fundamental base of the contribution of mutagenesis to atherogenesis is still unstudied thought it is important for industrial regions with high genotoxic risk The aim of this research was to study the secretion and gene expression of proatherosclerotic cytokines IL-6 and IL-8 by primary human coronary- and internal thoracic artery endothelial cells characterized by different sensitivity to atherogenesis and exposed to alkylating mutagen mitomycin C. Concentration of the studied cytokines in culture medium and mRNA level of the corresponding genes were measured using quantitative polymerase chain reaction and enzyme-linked immunosorbent assay at two time points - immediately after 6 hours of cell incubation with mitomycin C and after 6 hours of cell incubation with mutagen followed by 24 hours incubation in the complete media without mitomycin C. It was found that at the first time point, a significant decrease in the secretion and gene expression of IL-8, as well as a decrease in the mRNA level of the IL-6 gene in the exposed culture was discovered. At the second time point, on the contrary, the concentration and mRNA level of IL-8, as well as expression of the IL6 gene sharply increased in cultures exposed to mitomycin C in comparison with control (p <0.01), and the coronary artery cells were more sensitive to mutagenic effects than the cells of the internal thoracic artery. Thus, for the first time in in vitro experiment, mutagen-induced changes in the secretion and gene expression of proatherosclerotic cytokines IL-6 and IL-8 in primary human endothelial cells derived from various arteries have been shown.
Aim. To determine the expression of adiponectin, leptin and I interleukin-6 (IL-6) in subcutaneous, epicardial and perivascular adipose tissue, depending on the presence of cardiovascular risk factors.Material and methods. The study included 90 patients with stable coronary artery disease (CAD) who underwent coronary artery bypass grafting. Samples of adipose tissue were obtained during surgery. The levels of matrix ribonucleic acid (mRNA) of the studied adipocytokines were determined in the presence/absence of the main cardiovascular risk factors.Results. Differences in the expression of genes of the studied adipocytokines in different sex and age groups of patients were revealed, depending on the tissue belonging of adipocytes. Expression of adiponectin in the epicardial and perivascular adipose tissue (EАT and PVAT, respectively), as well as of leptin in the PVAT was less pronounced in men. However, the level of IL-6 mRNA in the subcutaneous adipose tissue (SAT) of men was three times higher than in women, and in the PVAT it was lower. The maximum expression of leptin and IL-6 in the EAT and PVAT was found in persons aged 50-59 years. The presence of dyslipidemia is associated with a decrease in the expression of adiponectin in the EAT, PVAT, and IL-6 in the PVAT. In patients with hypertension (HTN), there was a low level of adiponectin mRNA in the EAT against the background of high leptin levels in the EAT and IL-6 in SAT and EAT. In hypertension with a duration of more than 20 years, there was a decrease in adiponectin expression and an increase in leptin in all types of AT. In smokers, an increase in the expression of adiponectin in the SAT, EAT, PVAT and leptin in the SAT, EAT was found.Conclusion. Associations of traditional cardiovascular risk factors with imbalance of adipocytokines of local fat depots in patients with CAD were revealed. The detected imbalance is manifested by a decrease in the expression of cardioprotective adiponectin in the EAT, PVAT, an increase in leptin and IL-6, which is an unfavorable sign. The presence of such risk factors as male sex, age of 50-59 years, dyslipidemia and hypertension in patients can enhance atherogenesis and contribute to the further progression of CAD.
Актуальность. Кальций-фосфатные бионы (КФБ) формируются при перенасыщении крови ионами кальция и фосфора и, циркулируя в кровотоке, повреждают эндотелиальные клетки (ЭК), при растворении в лизосомах вызывая их лизосомально-опосредованную клеточную гибель, которая может сопровождаться выделением провоспалительных цитокинов в микроокружение. Цель исследования. Оценить профиль секреции провоспалительных цитокинов первичными артериальными ЭК человека под воздействием сферических КФБ (СКФБ) и игольчатых КФБ (ИКФБ). Материалы и методы. СКФБ и ИКФБ в равных концентрациях (100 мкл, оптическая плотность суспензии на длине волны 650 нм 0,08-0,10) были добавлены к конфлюэнтным культурам первичных ЭК коронарной и внутренней грудной артерии человека в 6-луночных планшетах. Культуральная жидкость забиралась после 24 часов культивирования с последующим измерением в ней уровня провоспалительных цитокинов (интерлейкин(ИЛ)-1β, ИЛ-6, ИЛ-8, ИЛ-10, ИЛ-12, ИЛ-23, фактор некроза опухоли-α, γ-интерферон) посредством иммуноферментного анализа. Кроме того, из клеточной массы выделяли РНК посредством соответствующих колонок с последующей обратной транскрипцией для синтеза кДНК и оценкой экспрессии генов, кодирующих вышеуказанные цитокины (IL1B, IL6, CXCL8, IL10, IL12A, IL12B, IL23, TNF, IFNG) и некоторые рецепторы к ним (IL1R1, IL1R2, TNFRSF1A, TNFRSF1B), методом количественной полимеразной цепной реакции. Результаты. Воздействие СКФБ и ИКФБ приводило к статистически значимому повышению экспрессии генов IL1B, IL6, CXCL8, IL12A и IL23, при этом ЭК коронарной артерии характеризовались более выраженным ответом на воздействие КФБ на уровне транскриптов в сравнении с ЭК внутренней грудной артерии. Хотя воздействие ИКФБ в большинстве случаев приводило к более выраженному увеличению экспрессии генов цитокинов в сравнении с СКФБ, на уровне белков эта зависимость от формы бионов не наблюдалась. Экспозиция СКФБ и ИКФБ приводила к повышению выделения ИЛ-6 обоими типами артериальных ЭК, в то время как повышенная секреция ИЛ-8 наблюдалась лишь при экспозиции ИКФБ ЭК внутренней грудной артерии. Заключение. Добавление КФБ к культурам первичных артериальных ЭК способствует выделению этими клетками провоспалительных цитокинов ИЛ-6 и ИЛ-8 в результате повышения экспрессии генов (IL6 и CXCL8 соответственно) независимо от формы КФБ (сферической или игольчатой). Background. Calcium phosphate bions are formed under blood supersaturation with calcium and phosphate. Circulating bions provoke endothelial injury via lysosome-dependent cell death accompanied by release of proinflammatory cytokines into the microenvironment. Aim. To evaluate the cytokine secretion profile of human primary arterial endothelial cells (ECs) exposed to either spherical (CPB-S) or needle-shaped (CPB-N) calcium phosphate bions. Materials and Methods. CPB-S and CPB-N were added at equal concentrations (100 µL, optical density 0.08-0.10, at 650 nm wavelength) to confluent cultures of primary coronary artery (HCAEC) and internal thoracic artery (HITAEC) endothelial cells in 6-well plates. At 24 h of culture, the cell culture supernatant was collected, and respective cytokines (IL-1β, IL-6, IL-8, IL-10, IL-12, IL-23, tumor necrosis factor-α, and γ-interferon) were measured by an enzyme-linked immunosorbent assay. Expression of cytokine (IL1B, IL6, CXCL8, IL10, IL12A, IL12B, IL23, TNF, IFNG) and cytokine receptor genes (IL1R1, IL1R2, TNFRSF1A, TNFRSF1B) was quantified with the reverse transcription-polymerase chain reaction. Results. Exposure to both CPB-S and CPB-N led to statistically significant increases in the expression of IL1B, IL6, CXCL8, IL12A, and IL23 genes. HCAEC were characterized by a greater response to CPB at the transcript level compared to HITAEC. Although CPB-N induced a more pronounced cytokine response than CPB-S, such shape-dependent effects were negligible at the protein level. The exposure to CPB-S and CPB-N resulted in elevated release of interleukin-6 by both types of arterial ECs while the interleukin-8 release was augmented exclusively in response to the CPB-N exposure of HITAEC. Conclusion. Exposure of primary arterial ECs to CPB induced the release of IL-6 and IL-8 due to the expression of respective genes regardless of the CPB shape (spherical or needle-shaped).
