Background: Glucagon-like peptide 1 agonists differ in chemical structure, duration of action and in their effects on clinical outcomes. The cardiovascular effects of once-weekly albiglutide in type 2 diabetes are unknown. Methods: We randomly assigned patients with type 2 diabetes and cardiovascular disease to the addition of once-weekly subcutaneous injection of albiglutide (30 mg to 50 mg) or matching placebo to standard care. We hypothesized that albiglutide would be noninferior to placebo for the primary outcome of first occurrence of cardiovascular death, myocardial infarction, or stroke. If noninferiority was confirmed by an upper limit of the 95% confidence interval for the hazard ratio of less than 1.30, closed-testing for superiority was prespecified. Findings: Overall, 9463 participants were followed for a median of 1.6 years. The primary composite outcome occurred in 338 of 4731 patients (7.1%; 4.6 events per 100 person-years) in the albiglutide group and in 428 of 4732 patients (9.0%; 5.9 events per 100 person-years) in the placebo group (hazard ratio, 0.78; 95% confidence interval [CI ], 0.68 to 0.90), indicating that albiglutide, was superior to placebo (P<0.0001 for noninferiority, P=0.0006 for superiority). The incidence of acute pancreatitis (albiglutide 10 patients and placebo 7 patients), pancreatic cancer (6 and 5), medullary thyroid carcinoma (0 and 0), and other serious adverse events did not differ significantly between the two groups. Interpretation: In patients with type 2 diabetes and cardiovascular disease, albiglutide was superior to placebo with respect to major adverse cardiovascular events. There seems to be variation in the results of existing trials with GLP-1 receptor agonists, which if correct, might reflect drug structure or duration of action, patients studied, duration of follow-up or other factors.

This study evaluates the impact of hypoglycemia on the lives of Ukrainian patients with type 2 diabetes mellitus. The secondary objective was to explore patient–physician relationships and the attitudes of patients towards various informational resources on diabetes management. Three focus groups with 26 patients were conducted. Qualitative information was evaluated using content analysis. The results show that patients with type 2 diabetes mellitus in Ukraine are adapting to potential attacks of hypoglycemia; however, they still experience periodic manifestations of hypoglycemia that significantly affect their psychological well-being. This result is similar to observations made in other countries. Ukrainian patients >40 years old mainly receive information on disease management from endocrinologists, and rarely use internet resources on diabetes management. Information provision was especially important at the early stage of the disease, when patients lack information on hypoglycemia manifestations and could therefore fail to identify and manage it properly.

The aim: To evaluate the effect of vitamin D deficiency or insufficiency on the compensation of the disease in adolescents with diabetes mellitus type 1. Materials and methods: 124 patients were examined, among them: 33 from the control group, 91 adolescents with type 1 diabetes mellitus. All patients were between 10-18 years old and were residents of the Podillya region of Ukraine. Results: According to the results of the study Vitamin D levels in adolescents with type 1 diabetes mellitus are significantly lower, comparing with healthy adolescents of the same age group. Vitamin D levels among boys with type 1 diabetes mellitus were significantly lower comparing to girls of the same group. The boys of the control group had significantly higher levels of vitamin D in the blood comparing to boys with type 1 diabetes mellitus, p <0.001. Conclusions: The majority of adolescents aged 13-18 years with type 1 diabetes mellitus have vitamin D deficiency or insufficiency regardless of the state of glycemic control of the disease.

Резюме. Актуальність. Гіпотиреоз посідає одне з провідних місць у структурі ендокринних захворювань. Актуальніст ь проблеми втрати працездатності внаслідок гіпотиреозу підкреслюється зростанням не тільки первинної, але й хронічної інвалідності. Мета дослідження: вивчити особливості динаміки первинної й загальної інвалідності внаслідок гіпотиреозу. Матеріали та методи. Досліджувалась медико-експертна документація 596 хворих та інвалідів, у тому числі 375 хворих та інвалідів внаслідок післяопераційного гіпотиреозу (у тому числі 113 осіб, які були направлені на медико-соціальну експертну комісію (МСЕК) вперше, та 262 осіб, які проходили черговий переогляд), та 221 хворого та інваліда внаслідок ідіопатичного гіпотиреозу (у тому числі 31 особи, направленої на МСЕК вперше, та 190 осіб, які проходили черговий переогляд). Проведено аналіз офіційних статистичних даних інвалідності внаслідок гіпотиреозу дорослого працездатного населення. Результати. Динаміка інтенсивного показника внаслідок ідіопатичного та післяопераційного гіпотиреозу має позитивну тенденцію. Інтенсивний показник визнаних інвалідами внаслідок післяопераційного гіпотиреозу значно покращився. Однак інтенсивний показник первинної інвалідності ідіопатичного гіпотиреозу має негативну динаміку. Висновок. Враховуючи той факт, що показники повної та часткової реабілітації при ідіопатичному гіпотиреозі залишаються низькими, основним принципом дій щодо інвалідів стає їх реабілітація. Рекомендується впровадження в роботу лікувально-профілактичних закладів комплексу реабілітаційних заходів, відновне лікування в реабілітаційних центрах та санаторно-курортних умовах.
