The authors consider anatomical/physiological characteristics and a role of different autonomic CNS regions, including insula cortex, amygdala complex, anterior cingulate cortex, ventral medial prefrontal cortex, hypothalamus and epiphysis, involved in the regulation of cardiovascular activity. The damage of these structures, e.g., due to the acute disturbance of cerebral blood circulation, led to arrhythmia, including fatal arrhythmia, in previously intact myocardium; systolic and diastolic dysfunction, ischemic changes considered in the frames of cerebro-cardial syndrome. On the cellular level, the disturbance of autonomic regulation resulted in catechol amine excitotoxicity, oxidative stress and free radical myocardium injury.

При ВИЧ-инфекции частота поражения нервной системы стоит на втором месте после иммунной системы, и на долю острых нарушений мозгового кровообращения (ОНМК) приходится 1-5% всех поражений нервной системы при ВИЧ-инфекции. Встречаемость инсультов у лиц моложе 45 лет со СПИДом в 10 раз выше, чем в общей популяции у лиц того же возраста. Предполагается роль ВИЧ-ассоциированной васкулопатии и оппортунистических инфекций в увеличении рисков возникновения инсульта. Цель исследования: изучение особенностей течения ОНМК у больных с ВИЧ-инфекцией, поступающих в специализированные сосудистые центры, в зависимости от характера инсульта и выявление у них факторов риска ЦВЗ. Материалы и методы. Исследования проведены у 73 пациентов с ОНМК, госпитализированных по экстренным показаниям в сосудистые центры Санкт-Петербурга. В группу 1 вошли 33 (45,2%) человека (средний возраст 49±11 лет), у которых была выявлена ВИЧ-инфекция; 40 (54,8%) человек (средний возраст 49±7 лет) без ВИЧ-инфекции составили группу 2. В зависимости от характера инсульта -геморрагический (ГИ) или ишемический инсульт (ИИ) -группы 1 и 2 были разделены на подгруппы. Все пациенты обследовались в соответствии с протоколом по ведению больных с ОНМК, предусматривающим клинические, лабораторные, нейрорадиологические, ультразвуковые методы исследования. ВИЧ-инфекция подтверждалась лабораторными методами исследования -ИФА и иммуноблотинг с определением антигенов и антител к ВИЧ. Стадии ВИЧ-инфекции определялись в соответствии с классификацией В. И. Покровского в модификации Федерального научно-методического центра по профилактике и борьбе со СПИДом. Результаты исследования. Выявлено преобладание в группе 1 доли пациентов с 3 стадией ВИЧ-инфекции. При геморрагическом инсульте в группе 1 чаще выявлялись менингеальный симптомокомплекс и тромбоцитопения, а при ИИцефалгический синдром, лимфопения, тромбоцитопения, повышение СОЭ, ферментов АЛТ и АСТ, а также меньшая доля кардиоэмболических инсультов и незначительный регресс неврологических симптомов по сравнению с пациентами без ВИЧ-инфекции (р<0,05). Выводы. Основными факторами риска цереброваскулярных заболеваний (ЦВЗ) у больных с ВИЧ-инфекцией являются заболевания сердечно-сосудистой системы при меньшей доли больных с ИБС при ИИ, атеросклероз церебральных сосудов. ВИЧ, оказывая патогенное действие на систему гемостаза и эндотелий сосудов, играет значимую роль в повышении риска развития сосудистых катастроф у больных с ВИЧ-инфекцией. Меньший регресс неврологической симптоматики и более тяжелое течение заболевания при ВИЧ-инфекции обусловлены вторичными оппортунистическими заболеваниями, коинфекциями, поздними сроками госпитализации в стационар, гемостазиологическими нарушениями и в большинстве случаев -отсутствием АРВТ. Несмотря на определенные успехи в лечении эпилепсии, прежде всего связанные с активным развитием фармакологии, в 30 % случаев отмечается ее резистентное течение, особенно фокальных форм височной эпилепсии. Одной из основных причин является мезиальный височный склероз (МВС), считающийся абсолютным эпилептогенным субстратом, не поддающимся консервативному лечению в 90-92 % случаев. Оперативное лечение представляется единственным методом, способствующим улучшению качества жизни таких пациентов. Однако, процент хирургической помощи при эпилепсии остается низким. Структурированная информация по клиническим проявлениям, способам диагностики, подходам к хирургическому лечению и его исходам должна способствовать пониманию проблемы среди специалистов. Данные об эффективности операций у пациентов с фармакорезистентной эпилепсией могут помочь своевременному оказанию высокотехнологичной помощи больным с мезиальной височной эпилепсией.
Epilepsy is the most taking place disease of the human nervous system. Despite the fact that we are continuously introducing new medicines into therapy of this illness we still are failing to take control over seizures in 1/3 cases. Persisting seizures gain considerable influence on patients’ living standard and within children audience cause delays in person maturity. Given article shows deep analysis of the last years researches which gave background to discover main mechanisms of pharmacoresistance. The issues of pathophysiological features of medicine resistance and reasons of false diagnostics are as well discussed. Authors highlighted few pathologic conditions where applied differential diagnostics allowed to avoid diagnostical mistakes. True definition of the certain seizures features according to the up to date classification makes it possible for clinical staff to prescribe correct anticonvulsant therapy, so far diminishing the risk of pseudo resistance outcome of disease. The article introduces criteria of natural resistance which if being recognized allows to perform dew time surgical treatment and therefore cuts seizures in most of the cases.
