Based on available publications, the article systematizes information about some forms of lesions of the central nervous system (CNS), their pathogenesis and clinical manifestations in the case of COVID-19. The risk factors, mechanisms of development, diagnostic approach, and the age characteristics of patients with neurological complications of COVID-19 are discussed. The specific mechanisms of the neuroinvasiveness and neurovirulence of the SARS-CoV-2 virus, regardless of the age of patients and the presence of risk factors, lead to systemic damage to the endothelium of small-caliber vessels, generalized thrombovasculitis, and an increased risk of ischemic and hemorrhagic strokes. At the same time, the most vulnerable category is elderly and senile patients with cardiovascular and metabolic disorders (arterial hypertension, obesity, diabetes mellitus), which sharply worsen treatment outcomes. The clinical experience accumulated during the COVID-19 pandemic indicates the development of the following most frequent post-covid neurological complications and consequences in mainly elderly and senile patients: cranial mononeuropathies, chemosensory dysfunction, encephalopathy, insomnia, stroke, acute meningoencephalitis, acute disseminated encephalomyelitis, acute polyneuropathy Guillain–Barré syndrome, and transverse myelitis. The benefits of vaccination against COVID-19 far outweigh the risks of possible post-vaccination neurological complications and consequences, especially in elderly and senile patients.

При ВИЧ-инфекции частота поражения нервной системы стоит на втором месте после иммунной системы, и на долю острых нарушений мозгового кровообращения (ОНМК) приходится 1-5% всех поражений нервной системы при ВИЧ-инфекции. Встречаемость инсультов у лиц моложе 45 лет со СПИДом в 10 раз выше, чем в общей популяции у лиц того же возраста. Предполагается роль ВИЧ-ассоциированной васкулопатии и оппортунистических инфекций в увеличении рисков возникновения инсульта. Цель исследования: изучение особенностей течения ОНМК у больных с ВИЧ-инфекцией, поступающих в специализированные сосудистые центры, в зависимости от характера инсульта и выявление у них факторов риска ЦВЗ. Материалы и методы. Исследования проведены у 73 пациентов с ОНМК, госпитализированных по экстренным показаниям в сосудистые центры Санкт-Петербурга. В группу 1 вошли 33 (45,2%) человека (средний возраст 49±11 лет), у которых была выявлена ВИЧ-инфекция; 40 (54,8%) человек (средний возраст 49±7 лет) без ВИЧ-инфекции составили группу 2. В зависимости от характера инсульта -геморрагический (ГИ) или ишемический инсульт (ИИ) -группы 1 и 2 были разделены на подгруппы. Все пациенты обследовались в соответствии с протоколом по ведению больных с ОНМК, предусматривающим клинические, лабораторные, нейрорадиологические, ультразвуковые методы исследования. ВИЧ-инфекция подтверждалась лабораторными методами исследования -ИФА и иммуноблотинг с определением антигенов и антител к ВИЧ. Стадии ВИЧ-инфекции определялись в соответствии с классификацией В. И. Покровского в модификации Федерального научно-методического центра по профилактике и борьбе со СПИДом. Результаты исследования. Выявлено преобладание в группе 1 доли пациентов с 3 стадией ВИЧ-инфекции. При геморрагическом инсульте в группе 1 чаще выявлялись менингеальный симптомокомплекс и тромбоцитопения, а при ИИцефалгический синдром, лимфопения, тромбоцитопения, повышение СОЭ, ферментов АЛТ и АСТ, а также меньшая доля кардиоэмболических инсультов и незначительный регресс неврологических симптомов по сравнению с пациентами без ВИЧ-инфекции (р<0,05). Выводы. Основными факторами риска цереброваскулярных заболеваний (ЦВЗ) у больных с ВИЧ-инфекцией являются заболевания сердечно-сосудистой системы при меньшей доли больных с ИБС при ИИ, атеросклероз церебральных сосудов. ВИЧ, оказывая патогенное действие на систему гемостаза и эндотелий сосудов, играет значимую роль в повышении риска развития сосудистых катастроф у больных с ВИЧ-инфекцией. Меньший регресс неврологической симптоматики и более тяжелое течение заболевания при ВИЧ-инфекции обусловлены вторичными оппортунистическими заболеваниями, коинфекциями, поздними сроками госпитализации в стационар, гемостазиологическими нарушениями и в большинстве случаев -отсутствием АРВТ. Post-stroke developing epileptic seizures represent a severe complication aggravating post-stroke condition. Epilepsy can exacerbate cognitive, psychopathological, somatic disorders resulting from cerebrovascular and comorbid diseases. Despite that post-stroke epilepsy (PSE) is a rather common type of acquired structural epilepsy, the issues related to diagnosis and management often raise difficulties for clinicians. Patients with severe strokes affecting brain, cortex, acute symptomatic seizures and intracerebral hemorrhage are at greater risk of developing PSE. Timely neurophysiological, neuroradiological research methods, assessed blood biomarkers as well as prognostic models provide information that complements PSE clinical risk factors. The management of post-stroke acute and long-term (late) symptomatic seizures differs markedly. At the same time, the choice of an optimal anticonvulsant drug should be based not only on its effectiveness, but also on related side effects, pharmacodynamics as well as an impact on concomitant diseases. Drug interactions, especially between anticonvulsants and anticoagulants or antiplatelet agents also affect a choice of treatment, which should be taken into consideration for management of PSE patients.

При этом риск развития инсульта существенно повышается с возрастом: если в возрасте 45-54 лет частота инсультов составляет один на 1000 человек в год, в возрасте 65-74 года -один на 100 человек, а старше 85 лет -один на 30 человек [2, 6, 10]. Выявлено, что у 75-89 % больных инсульт развивается после 65 лет, 50 % из них -старше 70 лет и около 25 % -старше 85 лет. При этом возраст отрицательно сказывается на исходах инсульта. The OBJECTIVE was to improve the results of surgical treatment of patients with discogenic radiculopathy by preoperative neuropsychological examination of patients with an increased risk of unsatisfactory outcome of the operation.METHODS AND MATERIALS. Neuropsychological status of 77 patients with discogenic radiculopathy in the preoperative period was studied. For this purpose, we assessed the intensity of pain, levels of personal and situational anxiety and the presence and severity of depression. The obtained data were compared with postoperative results of quality of life assessment for the main parameters, which include the level of pain and adaptation to daily activities.RESULTS. Data from neuropsychological tests were revealed, the presence of which in patients with discogenic pain syndrome suggests the risk of an unsatisfactory assessment of the operation performed by the patient. According to the long-term results of neurosurgical treatment, patients were divided into three groups: 1 – with a good result; 2 – patients with a number of preserved complaints, who rated the result of the operation as satisfactory; 3 – unsatisfactory outcome. In patients with unsatisfactory results of surgical treatment of discogenic radiculopathies with pain syndrome, patterns of psychological indices were found that were significantly associated with the outcome of treatment.CONCLUSION. Conducting a neuropsychological examination prior to surgery with the identification of predictors of chronic discogenic pain and patient dissatisfaction with the results of treatment can influence the surgical tactics and optimize the process of further treatment.
