Интерпретация каждой электрокардиограммы должна включать измерение и оценку интервала QT, как увеличение, так и уменьшение продолжительности которого ассоциировано с риском возникновения жизнеугрожающих желудочковых аритмий и внезапной сердечной смерти. В настоящей статье отражены методологические подходы к определению, измерению и оценке длительности интервала QT, рассмотрен характер его зависимости от пола, возраста и частоты сердечных сокращений у пациента, а также описан ряд показателей, вычисляемых на его основе. На основании проведенного анализа литературы сформулированы рекомендации по измерению и оценке интервала QT стандартной электрокардиограммы. Желудочковые нарушения ритма (в частности, эпизоды фибрилляции желудочков и пароксизмальной желудочковой тахикардии) сопряжены с существенным увеличением риска внезапной сердечной смерти (ВСС) у пациентов с хронической сердечной недостаточностью (ХСН). Наиболее широко применяемая стратегия стратификации риска ВСС у пациентов с ХСН использует в качестве основного критерия низкую фракцию выброса левого желудочка, однако современные эпидемиологические исследования подтверждают потребность в поиске новых маркеров ВСС, в частности, электрокардиографических и визуализирующих. Наиболее обоснованным и перспективным представляется комплексный подход, сочетающий совокупность последовательного применения неинвазивных и инвазивных методик, что позволяет улучшить идентификацию лиц из группы высокого риска, которым для предотвращения ВСС показана инвазивная стратегия, однако которые не всегда соответствуют «классическим» критериям имплантации ИКД.Ключевые слова: хроническая сердечная недостаточность; внезапная сердечная смерть; желудочковые нарушения ритма; фракция выброса левого желудочка; электрокардиографические маркеры
Роль генетического компонента в механизме возникновения медикаментозно обусловленного синдрома интервала QT (СУИ QT) не подлежит сомнению, а многообразие обнаруженных генетических вариаций не ограничивается мутациями в генах, напрямую регулирующих структуру ионных каналов. В течение последних лет был идентифицирован ряд латентных мутаций в генах врожденного СУИ QT и обнаружены функциональные полиморфизмы в тех же или иных генах, связанные с повышенным риском внезапной сердечной смерти. К одним из наиболее перспективных предикторов развития медикаментозно обусловленного удлинения интервала QT относятся полиморфные варианты гена адаптерного белка нейрональной синтазы оксида азота (NOS1AP), кодирующего синтез данного адаптерного белка, в свою очередь, влияющего на физиологические эффекты нейрональной NO-синтазы и оксида азота, одним из которых является участие в регуляции желудочковой реполяризации. В данной статье отражены современные представления о механизмах медикаментозно обусловленного удлинения интервала QT и аритмогенеза у пациентов с дефицитом NO-синтазы и ее адаптерного белка, вызванных различными полиморфными вариантами гена NOS1AP. Genetic alterations have very important meaning in the mechanism of the onset of drug-induced QT interval syndrome (LQTS). Diversity of the detected genetic variations is not limited to mutations in the genes that directly regulate the structure of ion channels. Recently a number of "latent" mutations in the genes of congenital LQTS have been identified and single nucleotide polymorphisms in the genes associated with an increased risk of sudden cardiac death have been found. One of the most promising predictors of drug-induced QT interval prolongation is genetic variability of the neuronal NO synthase adaptor protein gene (NOS1AP), which encodes the synthesis of this adaptor protein, which in turn affects the physiological effects of neuronal NO synthase and nitric oxide, which regulate ventricular repolarization. This article reflects the up-to-date information on the mechanisms of drug-induced QT interval prolongation and arrhythmogenesis in patients with a NO synthase and its adaptor protein deficiency, caused by single nucleotide polymorphism variations of the NOS1AP gene.
Синдром удлиненного интервала QT (СУИ QT) может быть как врожденным, так и приобретенным, причем последний наиболее часто вызывается воздействием лекарственных препаратов. Клинические наблюдения показывают, что прием одних и тех же доз препаратов по-разному отражается на продолжительности интервала QT и риске возникновения желудочковой тахикардии типа «пируэт» у разных пациентов. Согласно концепции «резерва реполяризации» у пациентов с лекарственно-индуцированным СУИ QT существует генетически запрограммированный низкий резерв реполяризации, т. е. набор мутаций в генах ионных транспортеров, который не проявляется клинически до контакта с триггерным лекарственным препаратом. В течение последнего десятилетия идентифицирован ряд подобных «латентных» мутаций в генах врожденного СУИ QT и обнаружены функциональные полиморфизмы в тех же или иных генах, связанные с повышенной восприимчивостью к заболеванию. Методики полноэкзомного секвенирования и полногеномного поиска ассоциаций позволили оценить широкий спектр однонуклеотидных полиморфизмов в пределах всего экзома, а также определить взаимосвязь между геномными вариантами и клиническими фенотипами. В настоящем обзоре отражены основные достижения в области молекулярно-генетического направления диагностики лекарственно-индуцированного СУИ QT и дальнейшие перспективы исследований в данной области. Long QT syndrome (LQTS) can be both congenital and acquired. The latter one is most often caused by the effects of drugs. Clinical observations show that taking the same doses of drugs has different effects on the duration of the QT interval and the risk of torsades de pointes in different patients. According to the “repolarization reserve” concept, patients with drug-induced LQTS have a genetically programmed low repolarization reserve, i.e. a set of mutations in the genes of ionic transporters that do not manifest clinically prior to the contact with the trigger drug. During the last decade, a number of “latent” mutations in the genes of the congenital LQTS have been identified and functional polymorphisms in the same genes associated with increased susceptibility to the disease have been found. The methods of the whole exome sequencing and genome-wide association studies let to evaluate a wide range of single nucleotide polymorphisms within the entire exome and to investigate the relationship between genomic variants and certain phenotypes. The present review reflects the main advances in the molecular genetic direction of diagnostics of drug-induced LQTS and further prospects for research in this area.
