РезюмеЦель: изучить прогноз у пациентов с впервые диагностированной формой фибрилляции предсердий у пациентов, перенесших кардиоэм-болический инсульт. Материал и методы. В открытое, проспективное исследование «АПОЛЛОН» (Антикоагулянтная терапия у Пациен-тОв с фибриЛЛяцией предсердий, перенесших кардиОэмболический иНсульт) за период 01.10.2013 -31.03.2015 после этапа скрининга (n=1291) был включен 661 пациент с фибрилляцией предсердий, перенесший кардиоэмболический инсульт, средний возраст которых со-ставил 68,0 [62,0; 74,0] лет. Период наблюдения -12 месяцев. Результаты. Отмечен высокий уровень коморбидности (артериальная ги-пертензия верифицирована у 100% пациентов, ишемическая болезнь сердца -у 61,4% больных, сахарный диабет у -52,3%, и хроническая болезнь почек у 32,1% пациентов). У 154 (23,3%) пациентов ФП впервые была диагностирована в момент развития кардиоэмболического инсульта. Пациенты с впервые диагностированной формой фибрилляции предсердий были сопоставимы со всей группой по возрасту, по-казателю шкалы HAS-BLED, частоте встречаемости артериальной гипертензии, ИБС, но среди них достоверно чаще встречались пациенты с хронической болезнью почек, сахарным диабетом и инфарктом миокарда. Наибольшая смертность в течение года отмечена в группе па-циентов с впервые диагностированной формой фибрилляции предсердий, где умерло -46 (29,9%) человек, что статистически значимо больше, чем в целом как по всей группе больных с фибрилляции предсердий, перенесших кардиоэмболический инсульт, так и у пациентов с другим формами фибрилляции предсердий (р<0,001). Умершие больные с впервые диагностированной формой фибрилляции предсердий были достоверно старше, а также имели более выраженную степень неврологического дефицита по шкале NIHSS. Заключение. Полученные данные демонстрируют высокую распространенность и неблагоприятный прогноз впервые диагностированной формой фибрилляции пред-сердий у пациентов с кардиоэмболическим инсультом. Aim. To assess probability of atrial fibrillation (AF) in patients after ST-elevated myocardial infarction (STEMI) with undergone primary percutaneous coronary intervention (PCI).Material and methods. Prospective study in the period from December 2015 to November 2017 was carried out with consecutive inclusion of 107 patients at outpatient follow-up stage after STEMI who have undergone primary PCI. The mean age of patients was 69,5+7,8 years (40 (37,4%) women). Follow-up period was 18 months and included three visits (V): V1 — when included in the study, V2 — after 12 months, and V3 — after 18 months. The endpoints of the study were cases of first diagnosed AF, cardioembolic stroke and death. At all visits predictive markers (Willebrand factor, Cystatin C, N-terminal prohormone of brain natriuretic peptide (NT-proBNP), galectin-3) in the blood plasma were determined.Results. After 18 months of follow-up period in 19 (17,8%) patients were diagnosed AF, 5 of them (4,7%) had cardioembolic stroke, and 3 patients died. Median time of AF development from the onset of STEMI was 308 days. Cox multivariate analysis showed that the risk factors for first diagnosed AF were NT-proBNP — relative risk (RR): 1,05; 95% confidence interval (CI): 0,99-1,10 (p=0,038), cystatin C — RR: 1,44; 95% CI: 0,98-2,12 (p=0,043), galectin-3 — RR: 1,20; 95% CI: 1,03-1,40 (p=0,022).Conclusion. There was a highly significant relationship between NT-proBNP of ≥400,0 pg/mL, cystatin С of ≥1,45 ng/mL and galectin-3 of ≥25 mg/mL in patients after STEMI who underwent primary PCI. These markers might serve as predictors of first diagnosed AF. Identification of these biomarkers may have potential therapeutic benefit in improving the prognosis of patients after STEMI who have undergone primary PCI and reduce mortality from cardioembolic stroke.

Efficacy and safety of a new domestically produced rFVIII-BDD (moroctocog alfa, Octofactor, CJSC "GENERIUM", Russia) was investigated in a controlled, open, prospective, multicenter clinical trial. After screening and a 4-days washout period 12 previously treated patients adolescents (age 12-18 years old) with severe hemophilia A (the activity of FVIII was less than 1%) were included in the clinical trial. The patients were given the moroctocog alfa as prophylactic treatment in a dose of 35±5 ME/kg 3 times per week during 21±1 weeks. Before participation in this clinical trial 2 pts had received treatment with another recombinant FVIII, 2 pts had therapy with pdFVIII and 8 pts had been treated with rFVIII and pdFVIII. The main criterion of drug efficacy was the incidence of spontaneous bleeding occurred within 48 hours after of the moroctocog alfa injection. The additional criteria were: · The severity of spontaneous bleeding occurred in 21 ± 1 week. · Number of injections needed for the one episode of bleeding according of its severity. · The total amount of drug administered over a period of prophylactic treatment and treatment "on demand". · The number of patients with severe hemophilia A with a residual activity of FVIII ≥1% in 48 hours after the injection on prophylactic therapy. During the follow up period (21±1 week) 17 bleeding episodes were registered, 2 of them (11,8%) were severe, 10 (58,8%) were moderate and 5 (29,4%) were mild (Tab.1). Number of bleeding episodes (spontaneous and traumatic) was 17 (1.55 in average). Number of spontaneous bleeding was 3 (17,6%), 1 of them was mild and 2 were moderate. The average number of injections that stop one bleeding episode was 1.7±0.8. The total amount of moroctocog alfa administered over a period was 1.502.000 ÌÅ for prophylactic treatment and 64.750 ÌÅ for "on demand" treatment. The number of patients with severe hemophilia A with a residual activity of FVIII ≥1% in 48 hours after injection on prophylactic therapy was 63,6% on visit 2, 90,09% on visit 3 and 81,1% on visit 4. The safety assessment was performed in 12 patients. There were 8 adverse events and 7 of them were not associated with drugs administration. There was one serious adverse effect, allergic reaction accompanied by arthralgia and cephalalgia. The patient was excluded from the trial without consequences for life and health. There were no infection transmissions and de novo inhibitor incident. The study showed that moroctocog alfa is effective and safe in prophylactic treatment and stopping of bleeding in adolescents with severe hemophilia A. Table 1. Efficacy evaluation Table 1.
