Вступ. Синдром грудного виходу — це збірний термін, що поєднує групу нейросудинних синдромів. За даними світової статистики, поширеність синдрому грудного виходу серед населення становить 0,3–2 %. Вибір обсягу оперативного втручання, оптимального часу для проведення хірургічного лікування та профілактика рецидивів залишаються актуальними проблемами на сьогодні. Метою дослідження було поліпшити результати хірургічного лікування хворих із синдромом грудного виходу шляхом диференційованого використання різних методів оперативного втручання. Матеріали та методи. Проаналізовано результати оперативного лікування 71 пацієнта із синдромом грудного виходу шляхом надключичного класичного переднього доступу. У 35 хворих використані стандартні методи хірургічного лікування у вигляді невролізу структур плечового сплетіння, ангіолізу підключичних судин, скаленотомії та резекції додаткового шийного ребра (за потреби), у 17 хворих під час стандартного оперативного втручання була одночасно здійснена резекція I ребра, і у 19 хворих під час стандартного оперативного втручання додатково встановлювалася електростимулююча система «НейСі-3М» (НВП «ВЕЛ», Україна) для тривалої електростимуляції плечового сплетіння в післяопераційному періоді. Результати. Позитивний ефект при використанні тривалої електростимуляції плечового сплетіння у вигляді поліпшення неврологічного стану і трофічних функцій відзначений у 94,7 % хворих. Висновки. Метод тривалої електростимуляції структур плечового сплетіння дозволяє значно скоротити період відновлення неврологічного стану та забезпечує стабільний регрес больового синдрому в післяопераційному періоді.
Relevance. Dysfunction of the muscles of the posterior surface of the forearm leads to loss of extension in the wrist joint, metacarpophalangeal joints, and loss of abduction and extension of the first finger. The cause of dysfunction is damage to the radial nerve, supraclavicular or subclavian damage to the brachial plexus. The long regeneration process makes it impossible to effectively use the injured limb for a long period of time. Palliative use of movements (transposition) of muscles can significantly reduce the time for the patient to return to active use of the injured limb. Each of the muscle transpositions has certain disadvantages associated with the development of pathological locomotor phenomena (PLF) in the wrist joint. Ways to overcome them are based on a purely mechanistic approach, which is most often simplified to change the point of attachment of the primary non-functioning effector muscles. Objective: to define most adequate complex surgical approach in restoring effective extension function in the wrist joint and metacarpophalangeal joints. Materials and Methods. A retrospective analysis of the surgical treatment of 30 consecutive cases of dysfunction of the muscles of the posterior surface of the forearm caused by traumatic damage to the structures of the peripheral nervous system (PNS) of various localization was carried out. 23 patients with damage to the radial nerve. 7 patients with pathology of the brachial plexus. The mean age of patients was 41 years (from 18 to 64 years). Mean terms to primary surgical treatment were 4.6 months. 7 patients underwent only revision of the radial nerve within the segment (defect >10 cm); 6 patients underwent neurotization of the posterior interosseous nerve using the Mackinnon technique; 5 patients underwent autologous plasty of the radial nerve (defect <10 cm); 5 patients underwent its neurolysis. Neurolysis was performed in 6 patients with pathology of the brachial plexus, neurotization of the posterior interosseous nerve was performed in 1 case using the Mackinnon method. All patients underwent transposition of the forearm pronator teres (PT) according to the standard technique. Twelve patients underwent transposition of the flexor carpi radialis muscle (FCR, 4 cases) or flexor carpi ulnaris (FCU, 8 cases) according to the standard technique. The results of transposition were analyzed after 1 month or later than 6 months, using a clinical neurological method. Regeneration of neural structures of PNS were analyzed within 9-12 months and additionally in terms later than 15 months both neurologically and electrophysiologically. Results. In 6 patients, there was no restoration of extension in the metacarpophalangeal joints (EMPJ), in 12 patients there was a complete recovery of EMPJ after interventions on the structures of the PNS (4 cases – autologous plasty, 7 cases – distal neurotization, 1 case – neurolysis of the radial nerve). In 8 patients, the formation of PLF was not observed during extension in the wrist joint after muscle transposition. In 15 patients, PLF “type B” was formed, and in 7 patients, PLF “type C” was formed within 1 month after muscle transposition. In none of the patients, PLF “type C” was observed to be preserved for >6 months. In 8 patients, a permanent PLF “type B” was formed, which in 4 cases transformed into PLF “type D”. The formation of a steady-state PLF “type D” was recorded in all cases of neurolysis of the PNS structures without restoring extension in the metacarpophalangeal joints by the method of transposition. The formation of a steady-state PLF “type B” was recorded in all cases of FCU transposition to restore extension in the metacarpophalangeal joints. In 11 cases of reduction in the primary function of the FCR as a result of its denervation (neurotization according to the Mackinnon method) or transposition of the FCR muscles (change in the primary attachment point), PLF “type B” did not develop. Conclusions. Based on the results of the study, it was found that the most adequate complex surgical approach to avoid the formation of a stable PLF caused by muscle transposition to restore extension in the wrist joint is Mackinnon neurotization or FCR transposition to restore EMPJ.
Summary. Brachial plexus injuries is a severe and debilitating trauma characterized by gross impairment of upper extremity function. Mostly this is the trauma of young active people of working age, which often leads to significant loss of upper extremity function and disability of the patient. On October 10, 2019, at the XVIII Congress of Orthopedists Traumatologists of Ukraine held in Ivano-Frankivsk, a round table was held on the basic issues of diagnosis and treatment of patients with brachial plexus injuries, and a multidisciplinary consensus was adopted on the basic principles of diagnosis and treatment of brachial plexus injuries. During the round table, the issues of determining risk groups for brachial plexus injury (patients with polytrauma, road trafic accident, patients with injuries of the shoulder girdle area, dislocations of the shoulder, open lesions of the shoulder girdle and subclavian area, patients with atypical pain in the shoulder, patients that lost upper extremity function after surgery in the supraclavicular and subclavian area, neck, mastectomy, radiation therapy) and the protocol of clinical examination of patients (which must include the possibility of active movements in the joints of the upper limb and stabilization of the scapula; it is obligatory to determine the strength of the muscles of the affected limb in comparison with the healthy one, to determine sensitivity in the corresponding areas of autonomous innervation of the roots; also it is important to determine Horner syndrome) were discussed. Also, the place and time of additional diagnostics methods (emergency ultrasound and radiography, after 3 weeks – needle and stimulation electroneuromyography (ENMG), MRI of the brachial plexus or cervical spine, CT myelography) and an algorithm for the supervision of patients with a brachial plexus injury were adopted; the centers of brachial plexus surgery were specified. All this later formed the basis of multidisciplinary consensus.
Введение. Одной из главных причин синдрома грудного выхода являются изменения в передней лестничной мышце в результате компрессии плечевого сплетения и подключичных сосудов. Стандартное оперативное вмешательство с целью устранения компрессии позволяет существенно улучшить результаты хирургического лечения, однако, учитывая длительность периода восстановления функций пораженных верхних конечностей и возобновление симптоматики, особенно боли, в послеоперационном периоде, проблема лечения СГВ остается актуальной на сегодняшний день. Целью исследования было улучшение результатов хирургического лечения больных с синдромом грудного выхода, обусловленным изменениями в передней лестничной мышце, путем усовершенствования методов длительной электростимуляции структур плечевого сплетения. Материалы и методы. В клинике восстановительной нейрохирургии были прооперированы 37 пациентов с синдромом грудного выхода, обусловленным изменениями в передней лестничной мышце. Всем пациентам проведены невролиз, ангиолиз и декомпрессия нейрососудистого пучка в надключичной области, в том числе 14 больным с использованием методики длительной электростимуляции структур плечевого сплетения «НейСи-3М» («ВЭЛ», Киев, Украина). Результаты. Сочетание декомпрессии структур плечевого сплетения и подключичных сосудов с дополнительной длительной электростимуляцией в послеоперационном периоде позволило достичь у 92,3 % пациентов положительного эффекта в виде восстановления нервно-мышечного аппарата, чувствительных и трофических функций верхней конечности, а также стабильного регресса болевого синдрома. Выводы. Длительная электростимуляция плечевого сплетения — эффективный метод для восстановления нервно-мышечного аппарата и чувствительных функций верхней конечности, обеспечивающий стабильное купирование болевого синдрома у больных с синдромом грудного выхода.
