Background: Mechanical circulatory support (MCS) is the only way to save a life for heart transplant candidates and to decrease of waiting list mortality. The choice between short-or long-term pretransplant MCS depends on of type and severity of CHF. One of the most frequently used methods of temporary MSC before orthotopic heart transplantation (OHTx) is veno-arterial extracorporeal membrane oxygenation (VA ECMO). The aim of this study was to analyze own experience of peripheral VA ECMO (pVA ECMO) in heart transplant candidates needed in urgent HT. Methods: This study included 182 pts [160 (87.9%) men and 22 (12.1%) female, age 43±1.2 yrs] supported with pVA ECMO in the period from 01. 01. 2013 to 31. 12. 2017 or 23.2% from all waiting list (n=786). Results: During VA ECMO, 16 (8.8%) of the 182 pts died. In most pts [n=13 (81.3%)] multiorgan failure/sepsis were the cause of death. One hundred and sixty-six (91.2%) pts were successfully bridged to OHTx or 27.9% from all heart transplant recipients (n=594) (2013-2017 yrs). The duration of pVA ECMO before OHTx (n=166) was 5.8±3.2 days. One hundred and forty-three (86.1%) from 166 pts were discharged to home. Post-transplant survival among heart transplant recipient with pre-transplant MCS by pVA ECMO was in comparison with recipients without pretransplant MCS [84.2% vs. 90.1% (6 months), 83.3% vs. 91.8% (1 years), 75.1% vs. 86.1% (2 years), 74.2% vs. 85.8% (3 years), 72.3% vs. 84.7% (4 years), 72.3% vs. 83.5% (5 years) respectively (P<0.0001)]. Conclusions: pVA ECMO is a useful tool of treatment of patients with INTERMACS profile 1/2. Results of OHTx at recipients bridged with VA ECMO are less successful that recipients without pre-transplant MCS. VA ECMO should be considered as a direct bridge to OHTx in conditions of limited financial resources of health care and high availability of donor's hearts.
Diabetes mellitus is one of the most prevalent chronic metabolic disorders. Its role in patients with heart transplantation is not unifi ed. According to some authors, post-transplantation diabetes mellitus increases the risk of acute rejections and infections, increases the incidence of coronary artery disease of the graft and reduces long-term survival of patients with heart transplantation. On the other hand other studies did not confi rm these fi ndings. However, when diabetic patients were stratifi ed by disease severity, recipients with less severe disease achieved better survival. Accordingly, posttransplant survival was not signifi cantly different between recipients with uncomplicated diabetes and nondiabetic recipients. Diabetes alone should not be a contraindication to heart transplantation. Well-selected diabetic patients achieve the same survival as nondiabetic patients. Conversely, patients with complicated diabetes have signifi cantly worse survival. Therefore, given the critical shortage of transplantable organs, maximal benefi t may be achieved by exploring alternative treatment options in individuals with severe diabetes. These include use of high-risk transplant lists and destination therapy.
Introduction. In case of donor heart shortage and expanding pool of patients waiting for heart transplantation (OHTx) liberalization of donor selection, especially use of donors with left ventricular hypertrophy (LVH), may be one of most realistic methods to extending number of OHTx.Aim: to evaluate early and late outcomes after OHTx from donors with LVH ≥1.5 cm.Methods. We reviewed 160 heart recipients who underwent OHTx from donors with LVH 1.5 cm or more from 2011 to 2017.Results. The duration of anesthesia was 6.5 ± 0.7 h, surgery – 4.7 ± 0.3 h, cardiopulmonary bypass – 63–290 (145 ± 47) min and ischemia time was – 86–426 (168 ± 44) min. ICU stay was 7.4 ± 8.5 days. Hospital mortality in the study group was 8,1% (n = 13) and 30-day survival was 91.9%. Patients with or without donor LVH had similar early and long-term survival (p = 0.659).Conclusions. Own experience demonstrates the satisfactory results of HT from donors with LVH. In more cases, LV systolic function of cardiac allograft quickly normalized in the early period after HT.
Трансплантация сердца от доноров, перенесших сердечно-легочную реанимацию (СЛР) в процессе кондиционирования, является одним из вариантов увеличения доступности трансплантации сердца (ТС). В исследование включили 28 реципиентов: 25 (89,3%) мужчин и 3 (10,7%) женщины, возраст от 20 до 68 (46,2 ± 13,7) лет, которым в ФГУ «ФНЦТиИО им. академика В.И. Шумакова» в период с 01.01.2011 г. по 31.12.2017 г. была выполнена ТС от доноров, перенесших СЛР, что составило 4,0% от общего числа ТС за анализируемый период (n = 698). По частоте развития ранней дисфункции сердечного трансплантата, потребовавшей применения посттрансплантационной МПК, реципиенты групп «донор с СЛР» и «донор без СЛР» достоверно не различались. При сравнительном анализе достоверного различия в одно-, трехи пятилетней выживаемости реципиентов групп «донор с СЛР» и «донор без СЛР» выявлено не было.

Трансплантации сердца (ТС) у пациентов с предсуществующим сахарным диабетом (СД) 2-го типа сопряжена с повышенными риском инфекционных и неинфекционных (почечная дисфункция, мультифокальный атеросклероз, болезнь коронарных артерий пересаженного сердца и т. п.) осложнений, которые могут негативно повлиять на выживаемость реципиентов в ранние и отдаленные сроки после ТС. Цель исследования. На основании одноцентрового ретроспективного исследования оценить влияние предтрансплантационого СД 2-го типа на ранние и отдаленные результаты ТС. Материалы и методы. В исследование включили реципиентов (n = 891), которым была выполнена ТС в период с 2011-го по 2018 г. Реципиенты были разделены на две группы: основная группа (группа «СД2») -реципиенты с дотрансплантационным СД 2-го типа (n = 80; 9,0%), контрольная группа (группа «без СД2») -реципиенты без СД 2-го типа (n = 811; 91,0%). Реципиенты обеих групп не различались по неотложности ТС (UNOS статус) и потребности в предтрансплантационной механической поддержке кровообращения (МПК). Результаты. На момент выполнения ТС реципиенты группа «СД2» по сравнению с реципиентами группы «без СД2» были старше по возрасту (54 [46; 59] года против 48 [35; 56] лет, p < 0,001), имели больший вес (p < 0,001) и индекс массы тела (p < 0,001), основным заболеванием чаще была ишемическая болезнь сердца (65,0% против 36,5%, p < 0,001), имели более высокие значения транспульмонального градиента (10,0 [7,0; 12,0] против 9,0 [6,0; 12,0] мм рт. ст., р = 0,024) и легочного сосудистого сопротивления (2,9 [2,2; 4,0] против 2,5 [1,8; 3,4] ед. Вуда, p = 0,038). Перед ТС реципиенты группы «СД2» имели выраженные проявления почечной дисфункции и повышенную коморбидность. Реципиенты обеих групп не различались по характеристикам донора сердца, продолжительности ишемии трансплантата и искусственного кровообращения, частоте возникновения выраженной ранней дисфункции сердечного трансплантата, потребовавшей МПК (12,5% против 10,7%, р = 0,74). У реципиентов группы «СД2» в раннем периоде после ТС большие потребность (100% против 28,0%, p < 0,001) и дозировки инсулинотерапии, более выраженные проявления почечной дисфункции и большая потребность в заместительной почечной терапии (45,0% против 27,9%, р = 0,003) не повлияли на продолжительность искусственной вентиляции легких, длительность лечения в ОРИТ (6 [5; 10] дней против 6 [5; 10] дней, p = 0,098) и госпитальную летальность (8,8% против 8,5%, p = 0,895). Наличие дотрансплантационного СД 2-го типа не оказало отрицательного влияния на частоту возникновения острого отторжения сердечного трансплантата, прогрессирование трансмиссивного атеросклероза коронарных артерий, частоту возникновения и выраженность васкулопатии сердечного трансплантата, на структуру и выраженность отдаленных инфекционных и неинфекционных осложнений и посттрансплантационную выживаемость.
