Международное общество по артериальной гипертензии и Мировая антигипертензивная лига организовали скрининговое исследование по измерению артериального давления (АД) под названием MMM19 -MAY MEASUREMENT MONTH 2019 для улучшения выявления повышенного АД и оценки контроля АД у пациентов с артериальной гипертензией (АГ). Цель. Определить приверженность к лечению российского населения по результатам скрининга АГ в рамках международной акции МММ19. Материал и методы. В течение мая 2019г в скрининге приняли участие 5394 человека из 21 региона России. Участие в акции было добровольным без ограничения по полу, возраст старше 18 лет. Во время скрининга трехкратно измерялось АД с помощью автоматических и механических тонометров, заполнялся опросник о наличии сахарного диабета, сердечно-сосудистых осложнений в анамнезе, курении и употреблении алкоголя. Также получена информация о дате последнего контроля АД и приеме статинов, ацетилсалициловой кислоты и антигипертензивных препаратов. Информация о росте и массе тела была получена со слов респондентов. Результаты. В анализ были включены данные 5274 пациентов в возрасте от 18 до 96 лет, из них 1834 мужчин (34,8%). Медиана возраста -25 лет, квартили -20 и 49 лет. Пропорция пациентов с АГ по результатам МММ19 в российской выборке составила 31,9%, а при исключении участников младше 25 лет -51,5%. Пациенты с АГ старше 25 лет принимают антигипертензивную терапию (АГТ) в 73,6%, при этом 38,6% пропускают прием препаратов, а достигают целевого уровня АД <130/80 мм рт.ст. только 11,8% мужчин и 17,2% женщин. Даже при использовании 4-5 антигипертензивных препаратов, только 20-30% участников акции достигали целевого уровня АД. Заключение. По результатам скрининга МММ19 в России лишь треть пациентов высокого риска измеряли АД в течение последних 12 мес., наблюдается низкая приверженность к приему АГТ и недостаточный контроль АГ. Более склонны к регулярному приему гипотензивных средств пациенты с АГ, имеющие в анамнезе сердечно-сосудистые осложнения.Ключевые слова: скрининг, артериальная гипертензия, артериальное давление, приверженность, осведомленность. Raised life expectancy of patients with cardiovascular diseases (CVD), due to continuous progress in drug treatment options and widespread use of innovate technologies, increase the burden of CVD on healthcare system. The development of human resources by highly qualified specialists is of fundamental importance. For the rational use of human resources to achieve the targets of federal project on the prevention of cardiovascular diseases, it is necessary not only to analyze the actual situation with medical staffing, but also the potential effects of staff shortages and imbalances on mortality. The review presents evidence of associations between staffing and quality of care and CVD outcomes.
Despite the advances in lipidology over the past decade, the control of dyslipidemia at the population level in Russia, as in a number of European countries, remains unsatisfactory. The need for novel organizational approaches to solving the problem at the regional and federal levels is obvious. This publication provides an overview of the implemented projects and the successful practical experience of lipid centers in Russia, as well as the prospects for the development of novel models that will optimize the care provision for patients with lipid metabolism disorders at the population level.
Current guidelines for the management of patients with dyslipidemia are well known and easily accessible. Despite this, according to research data based on actual clinical practice, selection of optimal tactics for managing patients with dyslipidemia often causes difficulties and leads to a failure to achieve the target levels. Tools such as clinical decision support system (CDSS) can help clinicians follow current clinical guidelines, taking into account the diversity of phenotypic profiles and side effects. This review highlights the effectiveness of CDSS implementation in medical practice as a means for making decisions in managing patients with dyslipidemia, as well as presents the algorithm for CDSS for lipid metabolism disorders created by specialists of the Almazov National Medical Research Center and the University of Milan.
Coronary artery disease (CAD) is the most common cardiovascular disease and the leading cause of morbidity and mortality. Acute coronary syndrome (ACS) as an abrupt destabilization of CAD, multiplies the risk of cardiovascular events. To reduce the incidence of recurrent cardiovascular events, timely tackling potentially reversible risk factors such as hypertension and/or hyperglycemia is imperative. However, a solid basis for a secondary prevention lies in the treatment of dyslipidemia and begins in the first hours of hospital admission. Despite considerable evidence regarding the efficacy and safety of lipid-lowering therapy, averagely only one third of patients maintain control of lipids. The main challenges are low adherence, poor continuity of medical care, and the lack of an ambulatory routine follow-up. Telehealth solutions are believed to address these barriers and may be considered as an add-on to in-person patient care. Telemonitoring of vital and laboratory parameters, remote patient counseling can be introduced into routine care delivery. Telemedicine shows promise for fostering better clinical effect, and provides health-related quality of life improvement.It is planned to conduct a pilot observational study aimed to create and to test an integrated solution, i.e. telemonitoring and remote counseling in patients of very high cardiovascular risk with ACS followed by myocardial revascularization. The goal is to determine the clinical effectiveness, i.e achievement of target values of blood pressure, lipid profile and glycemia, and patient-centeredness of this approach.
