Регистр пациентов с семейной гиперхолестеринемией и пациентов очень высокого сердечно-сосудистого риска с недостаточной эффективностью проводимой гиполипидемической терапии (РЕНЕССАНС)Цель. В 2017г Российский многоцентровый регистр семейной гиперхолестеринемии (РоСГХС) был преобразован в Регистр пациентов с семейной гиперхолестеринемией и пациЕНтов очень высокого сЕрдечно-Сосудистого риска с недоСтАточной эффективНоСтью проводимой гиполипидемической терапии (РЕНЕССАНС), целью которого стал максимальный охват не только больных семейной гиперхолестеринемией (СГХС), но и больных с имеющимися атеросклеротическими сердечно-сосудистыми заболеваниями, у которых современная гиполипидемическая терапия не позволяла добиться целевого уровня холестерина липопротеидов низкой плотности (ХС ЛНП). Материал и методы. Регистр РЕНЕССАНС является открытым, национальным, наблюдательным исследованием и включает больных с определенной и вероятной (согласно голландским или британским критериям) гетерозиготной и гомозиготной СГХС, а также пациентов очень высокого сердечно-сосудистого риска. Были разработаны 2 регистрационных формы: для больных СГХС и больных очень высокого риска. Врачи заполняли формы в бумажном и электронном виде. Учитывали наличие факторов риска атеросклероза и анамнеза сердечно-сосудистых заболеваний, соблюдение диеты и гиполипидемической терапии. В каждом центре выполняли определение концентрации: общего холестерина (ОХС), триглицеридов (ТГ), холестерина липопротеидов высокой плотности (ХС ЛВП) в сыворотке крови. Содержание ХС ЛНП рассчитывали по формуле Фридвальда: ХС ЛНП=ОХС-ХС ЛВП-ТГ/2,2 (ммоль/л). Результаты. Всего включено 1208 больных СГХС и 497 больных очень высокого риска (средний возраст 54±13 и 61±8 лет, соответственно, 38% и 36% мужчин). При включении средние уровни липидов крови: ОХС -9,4±2,3 и 6,9±1,5 ммоль/л, ХС ЛНП -6,6±2,1 и 4,5±1,3 ммоль/л, соответственно. Частота применения гиполипидемической лекарственной терапии достигает в обеих группах 70%, однако достижение целевого уровня ХС ЛНП крайне редко. Заключение. Полученные результаты свидетельствуют о недостаточной приверженности и невысокой эффективности стандартной гиполипидемической терапии как при с СГХС, так и у больных с очень высоким сердечно-сосудистым риском. Для лечения резистентной гиперхолестеринемии показаны ингибиторы PCSK9. In recent years, the leverage on available genetic information and a better understanding of the genetic bases of dyslipidemias has led to the identification of potential new avenues for therapies. Additionally, the development of new technologies has represented the key for developing novel therapeutic strategies targeting not only proteins (e.g. the monoclonal antibodies and vaccines) but also the transcripts (from Antisense Oligonucleotides (ASOs) to small interfering RNAs) or the genomic sequence (gene therapies).These pharmacological advances have led to successful therapeutic improvements, particularly in the cardiovascular arena since we are now able to treat rare, genetically driven, and previously untreatable conditions (e.g: familial hypertriglyceridemia or hyperchylomicronemia).In this review, the pre-clinical pharmacological development of the major biotechnological cholesterol lowering advances will be discussed, describing facts, gaps, potential future step forwards and therapeutic opportunities.
Международное общество по артериальной гипертензии и Мировая антигипертензивная лига организовали скрининговое исследование по измерению артериального давления (АД) под названием MMM19 -MAY MEASUREMENT MONTH 2019 для улучшения выявления повышенного АД и оценки контроля АД у пациентов с артериальной гипертензией (АГ). Цель. Определить приверженность к лечению российского населения по результатам скрининга АГ в рамках международной акции МММ19. Материал и методы. В течение мая 2019г в скрининге приняли участие 5394 человека из 21 региона России. Участие в акции было добровольным без ограничения по полу, возраст старше 18 лет. Во время скрининга трехкратно измерялось АД с помощью автоматических и механических тонометров, заполнялся опросник о наличии сахарного диабета, сердечно-сосудистых осложнений в анамнезе, курении и употреблении алкоголя. Также получена информация о дате последнего контроля АД и приеме статинов, ацетилсалициловой кислоты и антигипертензивных препаратов. Информация о росте и массе тела была получена со слов респондентов. Результаты. В анализ были включены данные 5274 пациентов в возрасте от 18 до 96 лет, из них 1834 мужчин (34,8%). Медиана возраста -25 лет, квартили -20 и 49 лет. Пропорция пациентов с АГ по результатам МММ19 в российской выборке составила 31,9%, а при исключении участников младше 25 лет -51,5%. Пациенты с АГ старше 25 лет принимают антигипертензивную терапию (АГТ) в 73,6%, при этом 38,6% пропускают прием препаратов, а достигают целевого уровня АД <130/80 мм рт.ст. только 11,8% мужчин и 17,2% женщин. Даже при использовании 4-5 антигипертензивных препаратов, только 20-30% участников акции достигали целевого уровня АД. Заключение. По результатам скрининга МММ19 в России лишь треть пациентов высокого риска измеряли АД в течение последних 12 мес., наблюдается низкая приверженность к приему АГТ и недостаточный контроль АГ. Более склонны к регулярному приему гипотензивных средств пациенты с АГ, имеющие в анамнезе сердечно-сосудистые осложнения.Ключевые слова: скрининг, артериальная гипертензия, артериальное давление, приверженность, осведомленность. Aim. To study the prevalence of familial hypercholesterolemia (FH), the characteristics of the clinical features and treatment of the disease in selected regions of the Russian Federation, this article describes the design and initial characteristics of patients included in the study.Material and methods. The study participants were selected among those included in the study “Epidemiology of cardiovascular risk factors and diseases in the regions of the Russian Federation” (ESSE-RF) in different regions of the Russian Federation. The study included individuals with lowdensity lipoprotein cholesterol (LDL-C) levels >4.9 mmol/l or LDL-C levels >1.8 mmol/l, but ≤4.9 mmol/l during statin therapy, according to the data obtained in the ESSE-RF study. These persons are invited for examination and questioning by experts in the field of FH diagnostics. On the basis of the survey data and provided medical documentation, the following information is collected: age, sex, smoking status, presence of hypertension, history of coronary artery disease, stroke, atherosclerosis of cerebral and peripheral arteries, LDL-C level, type, volume and duration of lipid-lowering therapy throughout life, presence and dates of secondary causes of hyperlipidemia, information about the family history of development of early cardiovascular diseases and atherosclerotic diseases, increased levels of LDL-C in relatives of the 1st and 2nd degree of kinship. All patients are examined for the presence of tendon xanthomas (Achilles, metacarpal, elbow, knee tendons) and Corneal arcus. During the visit, blood is taken for subsequent biobanking, measurement of current blood lipid levels, elimination of secondary forms of hypercholesterolemia (for subsequent determination of liver enzymes, thyroid stimulating hormone) and genetic testing. The diagnosis of FH is based on Dutch Lipid Clinical Network Criteria (DLCN). Besides, all participants in the study are tested for compliance with the diagnosis of FH according to Simon Broome criteria. All patients with a definite or probable diagnosis of FH according to DLCN or Simon Broome criteria are subjected to ultrasound examination of carotid, femoral arteries and heart and molecular genetic testing for LDLR, APOB and PCSK9 gene variants.Results. Out of 16 360 participants of the ESSE-RF study in 10 regions, 1787 people (10,9%) met the criteria for inclusion in this study. Among them, men accounted for 35.4%, of which 1150 (7%) patients had a LDL-C level >4.9 mmol/l and 637 (3,9%) had a LDL-C level from 1,81 mmol/l to 4.9 mmol/l during lipid-lowering therapy. When compared to the original cohorts of participants from the 10 regions as compared to 3 previously surveyed regions and selected sub-groups within these cohorts we observed significant differences in several parameters such as age, total cholesterol level, triglycerides, LDL-C, the frequency of cardiovascular diseases, that may indicate regional differences in FH prevalence.Conclusion. The analysis of clinical data of the participants of the ESSE-RF study shows that more than 10% of individuals require an additional examination to verify the FH diagnosis, and regional differences in the FH prevalence are possible.
Genetics and environmental and lifestyle factors deeply affect cardiovascular diseases, with atherosclerosis as the etiopathological factor (ACVD) and their early recognition can significantly contribute to an efficient prevention and treatment of the disease. Due to the vast number of these factors, only the novel “omic” approaches are surmised. In addition to genomics, which extended the effective therapeutic potential for complex and rarer diseases, the use of “omics” presents a step-forward that can be harnessed for more accurate ACVD prediction and risk assessment in larger populations. The analysis of these data by artificial intelligence (AI)/machine learning (ML) strategies makes is possible to decipher the large amount of data that derives from such techniques, in order to provide an unbiased assessment of pathophysiological correlations and to develop a better understanding of the molecular background of ACVD. The predictive models implementing data from these “omics”, are based on consolidated AI best practices for classical ML and deep learning paradigms that employ methods (e.g., Integrative Network Fusion method, using an AI/ML supervised strategy and cross-validation) to validate the reproducibility of the results. Here, we highlight the proposed integrated approach for the prediction and diagnosis of ACVD with the presentation of the key elements of a joint scientific project of the University of Milan and the Almazov National Medical Research Centre.
