Adult WAG/Rij rats are considered adequate genetic models for human generalized absence epilepsy. Rats of this strain of 8, 12, and 18 weeks old and age-matched control Wistar rats were exposed to sound stimulation. After offset of stimulation, all WAG/Rij rats showed cataleptic or even cataplexic reactions, which could persist for up to 20 min. Age effects could be demonstrated. None of the Wistar rats showed cataleptic reactions. Electroencephalographic studies in WAG/Rij rats of 21 weeks showed that spike-wave discharges were abundantly present in the background electroencephalogram prior to sound stimulation. Age-matched Wistar rats had almost no spike-wave discharges. Spike-wave discharges in WAG/Rij rats disappeared during sound stimulation and were then increased compared to the prestimulation and stimulation periods. The electroencephalogram during the cataleptic state was also characterized by the presence of large amplitude 2 Hz waves, interspersed with spike-wave discharges. The data suggest that the cataleptic state can be elicited in genetically epilepsy-prone rats. The youngest WAG/Rij rats showed no spike-wave discharges during the cataleptic state. In all, the data suggest that epilepsy-prone animals are sensitive for catalepsy at an age at which the EEG signs of generalized absence epilepsy are not yet manifest.
An uptick in recent pandemics (Ebola, Zika, MERS, influenza, etc.) underlines the need for a more 'nimble,' coordinated response that addresses a multitude of issues ranging from transportation, access, facilities, equipment, and communication to provider training. To address this need, we have developed an innovative, scalable, logistics-enhanced, mobile, laboratory facility for emergencies and epidemics in resource-constrained global settings. Utilizing a background in clinical operations as an academic medical center, we designed a rapidly-deployable, modular BSL-2 and BSL-3 facility with user-friendly software for tracking and management of drugs and supplies in remote regions during epidemics and outbreaks. Here, we present our intermodal, mobile, expandable shipping-container laboratory units. The design of the laboratory facilitates off-grid usage by minimizing power consumption and allowing alternate water sources. The unit's information communication technology (ICT) platform provides (i) user-friendly tablet-based documentation, (ii) enhanced tracking of patients and supplies, and (iii) integrated communication onsite with built-in telehealth capabilities. To ensure quality in remote environments, we have developed a checklist for a basic laboratory workflow and a protocol for respiratory viral diagnosis using reverse-transcription polymerase chain reaction (RT-PCR). As described, this innovative and comprehensive approach allows for the provision of laboratory capability in resource-limited global environments.
Vaccine uptake is a multifactor measure of successful immunization outcomes that includes access to healthcare and vaccine hesitancy for both healthcare workers and communities. The present coronavirus disease (COVID-19) pandemic has highlighted the need for novel strategies to expand vaccine coverage in underserved regions. Mobile clinics hold the promise of ameliorating such inequities, although there is a paucity of studies that validate environmental infection in such facilities. Here, we describe community-based management of COVID-19 through a Smart Pod mobile clinic deployed in an underserved community area in the United States (Aldine, Harris County, TX, USA). In particular, we validate infection control and biological decontamination of the Smart Pod by testing surfaces and the air-filtration system for the COVID-19 virus and bacterial pathogens. We show the Smart Pod to be efficacious in providing a safe clinical environment for vaccine delivery. Moreover, in the Smart Pod, up-to-date education of community healthcare workers was provided to reduce vaccine hesitancy and improve COVID-19 vaccine uptake. The proposed solution has the potential to augment existing hospital capacity and combat the COVID-19 pandemic locally and globally.
Белорусская медицинская академия последипломного образования Патологические изменения желудочно-кишечного тракта у пациентов, находящихся в критическом состоянии, являются предметом интенсивного изучения. Наиболее явными клиническими проявлениями подобных изменений выступают кровотечения из острых язв желудка, 12-перстной кишки и верхних отделов тонкого кишечника. По данным различных авторов уже на первые сутки госпитализации в отделение интенсивной терапии эндоскопические признаки патологии слизистой верхних отделов желудочнокишечного тракта выявляются у 75-100 % пациентов [8, 14]. В большинстве случаев эти изменения подвергаются обратному развитию без специального вмешательства [9, 16]. Однако у ряда пациентов они прогрессируют, приводя к клинически значимым кровотечениям, проявляющимся рвотой, меленой, снижением гемоглобина, артериальной гипотензией. При неблагоприятном течении возникает необходимость в заместительном переливании больших объемов крови и ее препаратов. Ситуация, когда эти объемы превышают 20 мл/кг в сутки, расценивается как определение органной дисфункции желудочнокишечного тракта и служит неблагоприятным прогностическим признаком в отношении жизни больного. Клинические наблюдения показывают снижение общей частоты стрессовых кровотечений у пациентов отделений интенсивной терапии с 20-30% в 70-е годы к 1,4-14% в конце 90-х, что в определенной степени связывается с совершенствованием лечения основной патологии и широким внедрением в клиническую практику фармакологической профилактики [9]. Однако, не смотря на все достижения современной интенсивной терапии порядка 50-77% пациентов, состояние которых отягощается желудочнокишечным кровотечением, умирает от основного заболевания или присоединяющейся полиорганной недостаточности [9, 18, 20, 24]. Непосредственные патофизиологические причины стрессового повреждения слизистой желудочно-кишечного тракта до настоящего времени окончательно не установлены. Принято считать, что в его основе лежит целый ряд факторов, среди которых ведущее место принадлежит снижению кровотока, ишемии, функциональной неполноценности защитных механизмов эпителиального покрова и действию агрессивной среды содержимого желудочно-кишечного тракта.
