Приведенные в статье клинические наблюдения представляют собой различные варианты болезни Хираяма, протекающей с двусторонним или односторонним поражением верхних конечностей. Болезнь Хираяма (БХ) является редким непрогрессирующим неврологическим заболеванием, ассоциированным с поражением мотонейронов передних рогов спинного мозга при сгибании шейного отдела позвоночника. В настоящее время отмечается рост интереса и увеличение числа клинических описаний редкой формы мономелической амиотрофии (болезни Хираяма) в медицинских журналах, что может быть связано с распространенностью нейровизуализационных исследований и более широкого выполнения МРТ пациентам с дистальной амиотрофией. Диагностика БХ основывается на особенностях клинической картины, течения заболевания и дополняется специфичными для данной патологии МРТ-данными. «Золотым стандартом» диагностики БХ является МРТ шейного отдела позвоночника в положении сгибания. На данный момент этиотропного и патогенетического консервативного лечения не разработано. Возможны хирургическая коррекция или симптоматическая консервативная терапия. The clinical observations presented in the article represent various variants of Hirayama disease, which occurs with a bilateral or unilateral lesion of the upper extremities. Hirayama disease (HD) is a rare non-progressive neurological disease associated with damage to the motor neurons of the anterior horns of the spinal cord during flexion of the cervical spine. Currently, there is the increase of interest and the number of clinical descriptions of a rare form of monomyelic amyotrophy (Hirayama disease) in medical journals, which may be due to the prevalence of neuroimaging studies and more widespread MRI in patients with distal amyotrophy. The diagnosis of HD is based on the features of the clinical picture, the course of the disease, and it is supplemented by MRI data specific to this pathology. The “gold standard” for diagnostics of this pathology is MRI of the cervical spine in the flexion position. Currently, no etiotropic and pathogenetic conservative treatment has been developed. Surgical correction or symptomatic conservative therapy is possible.
Мутации в гене супероксиддисмутазы 1 (SOD1) связаны с боковым амиотрофическим склерозом (БАС) – нейродегенеративным заболеванием, преимущественно поражающим верхние и нижние двигательные нейроны, характеризующимся неуклонным прогрессированием и развитием мышечных атрофий, спастичности, пирамидных знаков. На сегодняшний день описано более 180 различных мутаций гена SOD1, включая точечные мутации, делеции и инсерции по пяти экзонам гена SOD1. Большинство мутаций наследуется по аутосомно-доминантному типу. Семейный БАС выявляется у каждого 10-го пациента, то есть в 10% случаев. Мутация в гене SOD1 ассоциирована с более ранним (до 40 лет) дебютом БАС, а также с его более быстрой и агрессивной прогрессией, приводя в конечном итоге к гибели пациентов, чаще от дыхательной недостаточности. Медико-генетическое исследование БАС имеет большое значение для понимания сложных патогенетических механизмов, ведущих к дегенерации двигательных нейронов. В настоящем исследовании мы представляем описание собственных клинических генетически подтвержденных случаев бокового амиотрофического склероза с гетерозиготной мутацией в гене SOD1 для демонстрации особенностей клинической картины и наследственного анамнеза наших пациентов. Mutations in the superoxide dismutase 1 (SOD1) gene are associated with amyotrophic lateral sclerosis, a neurodegenerative disease predominantly affecting upper and lower motor neurons, characterized by steady progression and development of muscle atrophy, spasticity and pyramidal signs. To date, more than 180 different mutations of the SOD1 gene have been proposed, including point mutations, deletions, and insertions in five exons of the SOD1 gene. Most mutations are inherited in an autosomal dominant manner. Familial ALS is detected in every 10 patients, that is, in 10% of cases. A mutation in the SOD1 gene is associated with an earlier (up to 40 years) onset of ALS, as well as with its more rapid and aggressive progression, leading to the development most often in observable patients, most often from respiratory failure. Medical genetic study of ALS is important for studying the complex pathogenetic mechanisms associated with motor neuron degeneration. In this study we present our own clinical descriptions of genetically confirmed cases of amyotrophic lateral sclerosis with a heterozygous mutation in the SOD1 gene to demonstrate the features of the clinical picture and hereditary history of our patients.
