Aim. Pharmacoeconomic comparison of medication management strategies (valsartan+ sacubitril) for patients with heart failure (HF), stabilized after an episode of acute decompensated heart failure (ADHF).Material and methods. “Cost — effectiveness analysis” and “Budget impact analysis” were used. The study is conducted in terms of the interests of Russian Federation health care system and budgets of individual regions.Results. The use of valsartan+sacubitril combination will require an increase in direct medical costs for 1 year by 38,5% compared with enalapril. The cost of one life year gained when using the valsartan+sacubitril combination was 307,294 rubles. When estimating data for the target ADHF population (n=200,769), valsartan+sacubitril will require additional 4,4 billion rubles per year. At the same time, this will save almost 17 thousand lives and prevent 126 thousand ambulance calls and 33,9 thousand rehospitalizations, including more than 6,5 thousand in the intensive care unit.Conclusion. The use of valsartan+sacubitril combination in HF patients hospitalized with ADHF is cost-effective management strategy that significantly improves the prognosis in this category of patients.
At the present time diabetes mellitus (DM) is a main noninfectious disease that not only significantly influences quality of life and life expectancy of every patient but also represents a heavy burden for the healthcare system. Medico social significance of DM is defined by high prevalence of this disease in population and high risk of its specific macro- and microvascular complications. According to latest data the main chain in the development of all specific complications of DM is chronic hyperglycemia, the important role belongs also to arterial hypertension and dyslipidemia. The multicenter randomized controlled study UKPDS assessed the influence of levels of hyperglycemia and arterial pressure as well as common methods of their corrections on development of complications of type 2 DM. The results of the UKPD study showed the significance of the intensive control of blood glucose and arterial pressure for the prophylaxis of type 2 DM complications and lethal outcomes. Эти изменения предрасполагают к развитию сер-дечных аритмий, являющихся важной причиной забо-леваемости и смертности в пожилом возрасте [1][2][3].В свою очередь сердечные аритмии повышают риск развития сердечной недостаточности и инсульта, яв-ляются основной причиной внезапной смерти, а также множественных симптомов (сердцебиения, боль в гру-ди, одышка, синкопе и др.), снижающих качество жиз-ни пожилых больных и приводящих к развитию трево-ги и депрессии [4,5]. Кроме того, аритмии могут клини-чески проявляться спутанностью сознания и необъясни-мыми падениями, которые у лиц пожилого возраста со-пряжены с повышенным риском переломов [6].Распространенность аритмий увеличивается с воз-растом. При целенаправленном скрининге желудочковые аритмии обнаруживаются у 70-80% пациентов старше 60 лет, хотя во многих случаях остаются бессимптомны-ми [7][8], фибрилляция предсердий -менее чем у 1% лиц моложе 60 лет и у 9-17,6 % -старше 80 лет [9][10][11]. Бо-лее 80% внезапных сердечных смертей возникает у лиц старше 65 лет [12].Данные об эффективности и безопасности анти-аритмической терапии у пациентов очень пожилого возраста ограничены, так как в рандомизированных контролируемых исследованиях (РКИ) участвовало край-не небольшое число пациентов старше 80 лет [8]. Оцен-ку эффективности антиаритмической терапии у этой ка-тегории пациентов затрудняет и лимитированная ожи-даемая продолжительность жизни.Помимо возрастных изменений сердечно-сосуди-стой системы при проведении фармакотерапии у паци-ентов старшего возраста следует учитывать их субъективное восприятие аритмии, отличное от такового у более мо-лодых, и обусловленное нарушением множественных фи-зических и когнитивных функций, а также взаимодей- Применение антиаритмических препаратов I класса у пожилых пациентов ограничивают их нежелательные реакции, аритмогенное действие (I A и I C) и высокий риск лекарственных взаимодействий. Применения дизопирамида следует избегать из-за выраженных антихолинергических свойств, сопряженных с риском развития ког-нитивных и соматических расстройств и падений у самых пожилых пациентов. Имеющиеся данные позволяют предположить, что у пожилых лиц нет существенных ограничений к применению β-адреноблокаторов. Однако для определения препаратов данной группы с оптимальным соотношением польза/риск у лиц пожилого воз-раста с коморбидностью необходимы дальнейшие исследования. Острая боль, особенно скелетно-мышечная и послеоперационная, является широко распространенной патологией, сопряженной с ухудшением качества жизни пациентов, временной нетрудоспособностью, склонностью к рецидивированию, а в ряде случаев и к хроническому течению, поэтому ее своевременное и адекватное лечение важно, в том числе с точки зрения профилактики серьезных медицинских, социальных и экономических последствий. При скелетно-мышечной боли в качестве средств первого выбора в современных рекомендациях рассматриваются нестероидные противовоспалительные препараты (НПВП). Базовым компонентом лечения дорсалгий также являются миорелаксанты центрального действия и их комбинации с НПВП. Однако доказательная база имеется лишь для отдельных миорелаксантов, в частности для орфенадрина. Включение НПВП в состав мультимодальной послеоперационной аналгезии значительно уменьшает потребность в опиоидах, снижает риск развития нежелательных реакций последних и повышает удовлетворенность пациентов качеством обезболивания. Целесообразно использование в составе мультимодальной аналгезии и центральных миорелаксантов, обладающих аналгезирующими свойствами, например тизанидина и орфенадрина. В статье обсуждаются фармакологические свойства фиксированной комбинации диклофенака и орфенадрина (Неодолпассе ®), ее преимущества перед монотерапией отдельными ингредиентами и результаты клинических исследований данной комбинации при болевых спинальных синдромах и в послеоперационном периоде.
(1) Background: Ciprofloxacin (CPF) is widely used for the treatment of cystic fibrosis, including pediatric patients, but its pharmacokinetics is poorly studied in this population. Optimal CPF dosing in pediatric patients may be affected by gene polymorphism of the enzymes involved in its biotransformation. (2) Materials and Methods: a two-center prospective non-randomized study of CPF pharmacokinetics with sequential enrollment of patients (n-33, mean age 9.03 years, male-33.36%), over a period from 2016 to 2021. All patients received tablets of the original CPF drug Cyprobay® at a dose of 16.5 mg/kg to 28.80 mg/kg. Blood sampling schedule: 0 (before taking the drug), 1.5 h; 3.0 h; 4.5 h; 6.0 h; 7.5 h after the first dosing. CPF serum concentrations were analyzed by high performance liquid chromatography mass spectrometry. The genotype of biotransformation enzymes was studied using total DNA isolated from whole blood leukocytes by the standard method. (4) Results: a possible relationship between the CA genotype of the CYP2C9 gene (c.1075A > C), the GG genotype of the CYP2D6*4 gene (1846G > A), the AG genotype of the GSTP1 gene (c.313A > G), the GCLC* genotype 7/7 and the CPF concentration in plasma (increased value of the area under the concentration–time curve) was established. Conclusions: Gene polymorphism of biotransformation enzymes may affect ciprofloxacin pharmacokinetics in children.
Functional dyspepsia is one of the most common functional disorders of the gastrointestinal tract, which resulted from impaired motor skills, visceral hypersensitivity, increased mucosal permeability, disorders of the autonomic nervous system, etc. There is no specific therapy for this disease, which often leads to the irrational use of various groups of drugs. Drug therapy is recommended only during periods of symptoms. The main options of pharmacotherapy include the use of proton pump inhibitors, phytotherapeutic drugs, eradication therapy of Helicobacter pylori. Against the background of possible motor disorders, prokinetics are also one of the possible treatment options, but cisapride has long been withdrawn from sale due to cardiotoxicity, the use of domperidone and metoclopramide is limited due to side effects, especially with long-term therapy, so currently the only prokinetic that can be used in everyday clinical practice is itopride. In refractory cases, tricyclic antidepressants and psychotherapeutic approaches are another effective treatment option.
