В связи с инвазивностью, сложностью в подготовке и проведении, высокой стоимостью эндоскопического исследования, существует потребность в применении лабораторных биомаркеров язвенного коли-та, с помощью которых можно выделить группу лиц, подлежащих углубленному инструментальному обследованию. В обзоре освещается современный взгляд на патогенез заболевания, специфические лабораторные биомаркеры в диагностике язвенного колита, для мониторин-га эффективности проводимой терапии, в качестве предикторов тяжелого течения язвенного колита, прогнозирования терапевтического ответа и раннего выявления рецидива. AbstractThe basic theory of the pathogenesis of inflammatory bowel disease (IBD) is currently considered a violation of immune activation and immune response against its own antigens to the intestinal flora in genetically predisposed individuals. "The gold standard" diagnosis of UC is colonoscopy with biopsy morphological study. Due to the invasiveness, complexity in the preparing and conducting, the high cost of endoscopy there is a need in the application of laboratory biomarkers UC, with which you can select a group of persons subject to an in-depth examination of the instrumental. The review highlights the modern view of the pathogenesis of the disease, the ability to use a variety of laboratory biomarkers in the diagnosis of UC, for monitoring the effectiveness of the therapy, as predictors of severe course of UC, therapeutic response prediction and early detection of recurrence.
Infl ammatory bowel disease (IBD) has been in the researchers' focus of attention for at least two decades and is one of the most studied human pathologies. On the one hand, advances in scientifi c knowledge about the pathophysiology of IBD have contributed to the development of new drugs. But on the other hand, the etiology of these diseases remains unknown. And we have the right to call IBD "the mystery of modern gastroenterology". Obviously, the increase in the number of patients with ulcerative colitis (UC) and Crohn's disease (CD) occurs simultaneously with the change in lifestyle and basic approaches to nutrition in modern society, the so-called "Western lifestyle". IBD became more common around the same time that the obesity pandemic began. At fi rst glance, the relationship between IBD and obesity can be questioned. However, with a detailed analysis of predisposing factors and the prevalence of these diseases, many common features can be found. Obesity is becoming more common among patients with IBD and plays an important role both in development and in progress of UC and CD. It has been argued that adipocytes, release of pro-infl ammatory cytokines, imbalance of leptin and adiponectin, decrease in mucin synthesis, induction of intestinal permeability, bacterial translocation are involved in the pathogenesis of IBD. Changes in the composition of the intestinal microbiota are probably an important pathogenetic link between obesity and IBD. We present this review to physicians and gastroenterologists for information purposes, focusing on studies conducted over the past years.
With the advent of modern cellular and genomic technologies, we have become participants in the integration of such areas as personalized, predictive, preventive, and precision medicine (referred to as 4P-medicine), into practical healthcare. In replace of the classic methods of diagnosis and treatment of diseases comes medicine, which makes it possible to predict (anticipate) the disease, and a personalized approach to each patient, taking into account their genetic, biochemical and physiological uniqueness. Precision medicine aims to improve the quality of medical care by opening up an individual approach to the patient and covers a wide range of areas, including drug therapy, genetics, and cause-and-effect relationships in order to make the right decisions based on evidence. 4P-medicine combines knowledge in the field of proteomics, metabolomics, genomics, bioinformatics with classical approaches of anatomy, therapy, laboratory and instrumental diagnostics as well as public health. The purpose of this review is to analyze and summarize the information available to date and to present examples of the application of modern approaches of medicine into clinical practice by diving into the example of inflammatory bowel diseases (IBD). The search for literature containing scientific information about relevant studies was conducted in the PubMed and Google Scholar systems with the use of the following keywords: precision medicine, 4P medicine, inflammatory bowel diseases. Despite significant progress in medicine in general, there is still a long way to go before implementing the principles of precision medicine in the field of IBD, since many clinicians continue to treat patients with IBD symptomatically. However, the use of specific biomarkers and new treatment strategies as described in the review, can significantly accelerate this path and contribute to the improvement of diagnostic and therapeutic approaches.
Избыточный вес и ожирение у больного язвенным колитом… | Overweight and obesity in patients with ulcerative colitis… Резюме Основной теорией патогенеза воспалительных заболеваний кишечника в настоящее время считается нарушение иммунорегуляции и активация иммунного ответа по отношению к антигенам собственной кишечной микрофлоры у генетически предрасположенных лиц под воздействием определенных факторов внешней среды. Заболеваемость и распространенность язвенного колита растет параллельно с пандемией ожирения. В последние годы появляется информация об эпидемиологическом взаимодействии предрасполагающих факторов к ожирению и язвенному колиту.Цель исследования: оценка распространенности избыточной массы тела и ожирения у больных язвенным колитом региона Западная Сибирь до появления первых симптомов заболевания и на фоне лечения. Оценка индекса массы тела (ИМТ) у больных язвенным колитом в зависимости от пола, протяженности поражения, степени тяжести, продолжительности заболевания и терапии биологическими препаратами.Материалы и методы: проведено исследование типа «случай-контроль» 127 язвенным колитом 63 здоровых добровольцев сопоставимых по полу и по возрасту. ИМТ рассчитывали по формуле: масса (кг)/вес (м²).Результаты: у 58% больных язвенным колитом региона Западная Сибирь перед появлением первых симптомов заболевания отмечалась избыточная масса тела и/или ожирение. При этом статистически значимое преобладание избыточной массы тела и ожирения отмечалось у мужчин. При динамическом наблюдении за больными ЯК отмечается более низкий ИМТ у больных ЯК при тотальном поражении, тяжелом течении заболевания, а также на фоне продолжительного заболевания. Терапия биологическими препаратами способствует увеличению ИМТ.
