Цель работы заключалась в сопоставительном анализе содержания α-дефензинов (HNP 1-3), про-и противовоспалительных цитокинов в плазме крови и цереброспинальной жидкости (ЦСЖ) и их роли в развитии системного воспалительного ответа в посттравматическом периоде у пациентов с тяжелой черепно-мозговой травмой (ТЧМТ).Материалы и методы. Под наблюдением было 20 пациентов с тяжелой черепно-мозговой травмой в возрасте от 18 до 78 лет (средний возраст 38,3±11,5 лет). В первые 24 часа и через 7 суток после травмы пациентам выполнялась люмбальная пункция с забором цереброспинальной жидкости и периферической крови из центральной вены. Количественное содержание α-дефензинов в плазме крови и цереброспинальной жидкости, цитокинов в сыворотке крови и ЦСЖ определяли методом ИФА с набором для определения человеческих α-дефензинов и цитокинов.Результаты проведенных исследований показали, что тяжелая ЧМТ сопровождается повышенной продукцией противомикробных пептидов (α-дефензинов -HNP 1-3), про-и противовоспалительных цитокинов, продукция которых в динамике развития воспалительного процесса в головном мозге имеет ряд особенностей. В первые сутки после ЧМТ в плазме крови и ЦСЖ наблюдается значительное увеличение концентрации HNP 1-3. При этом уровень в плазме крови достоверно превышает их содержание в ЦСЖ в 9,7 раза. В динамике на седьмые сутки после травмы отмечалось снижение содержания HNP 1-3 в плазме крови и ЦСЖ, более выраженное в ликворе. Между уровнем HNP 1-3 и ИЛ-1β в плазме крови, HNP 1-3 и содержанием белка, цитозом в ликворе в 1-е сутки, а также между HNP 1-3 и противовоспалительным цитокином ИЛ-10 как в плазме крови, так и в ликворе на 7-е сутки выявлена прямая корреляционная зависимость, отражающая динамику течения воспалительного процесса в головном мозге.Заключение.
