Цель работы заключалась в сопоставительном анализе содержания α-дефензинов (HNP 1-3), про-и противовоспалительных цитокинов в плазме крови и цереброспинальной жидкости (ЦСЖ) и их роли в развитии системного воспалительного ответа в посттравматическом периоде у пациентов с тяжелой черепно-мозговой травмой (ТЧМТ).Материалы и методы. Под наблюдением было 20 пациентов с тяжелой черепно-мозговой травмой в возрасте от 18 до 78 лет (средний возраст 38,3±11,5 лет). В первые 24 часа и через 7 суток после травмы пациентам выполнялась люмбальная пункция с забором цереброспинальной жидкости и периферической крови из центральной вены. Количественное содержание α-дефензинов в плазме крови и цереброспинальной жидкости, цитокинов в сыворотке крови и ЦСЖ определяли методом ИФА с набором для определения человеческих α-дефензинов и цитокинов.Результаты проведенных исследований показали, что тяжелая ЧМТ сопровождается повышенной продукцией противомикробных пептидов (α-дефензинов -HNP 1-3), про-и противовоспалительных цитокинов, продукция которых в динамике развития воспалительного процесса в головном мозге имеет ряд особенностей. В первые сутки после ЧМТ в плазме крови и ЦСЖ наблюдается значительное увеличение концентрации HNP 1-3. При этом уровень в плазме крови достоверно превышает их содержание в ЦСЖ в 9,7 раза. В динамике на седьмые сутки после травмы отмечалось снижение содержания HNP 1-3 в плазме крови и ЦСЖ, более выраженное в ликворе. Между уровнем HNP 1-3 и ИЛ-1β в плазме крови, HNP 1-3 и содержанием белка, цитозом в ликворе в 1-е сутки, а также между HNP 1-3 и противовоспалительным цитокином ИЛ-10 как в плазме крови, так и в ликворе на 7-е сутки выявлена прямая корреляционная зависимость, отражающая динамику течения воспалительного процесса в головном мозге.Заключение. В статье представлены результаты исследования цитокинового профиля, противомикробных пептидов (αдефензинов), а также показателей клеточного иммунитета у пациентов с сезонным аллергическим ринитом в стадии обострения. Показано, что иммунный статус больных характеризуется увеличением относительного количества CD3-CD19+ лимфоцитов и снижением CD3+CD8+ клеток, активацией гуморального иммунитета и увеличением концентрации цитокинов-регуляторов Тh2 иммунного ответа (ИЛ-4 и ИЛ-10) в назальном секрете и сыворотке крови. Это сочеталось со снижением функциональной активности Тh1-лимфоцитов-регуляторов клеточного иммунного ответа, повышенной продукцией провоспалительных цитокинов (ИЛ-1β, ИЛ-8) и противомикробных пептидовα-дефензинов (HNP1-3), способствующих дегрануляции тучных клеток и секреции медиаторов, поддерживающих аллергическое воспаление. Маркса, д. 3 Цель: изучить морфологические изменения, происходящие в тканях легкого при буллезной эмфиземе и при терапии ее стимулятором неоколлагеногенеза.Материалы и методы. Исследование проводилось 60 особях крыс-самцов линии «Вистар». Крысы были подразделены на три равные группы, 1 группа -группа контроля (введение лекарственных препаратов не производилось), 2 -экспериментальная с моделированием буллезной эмфиземы легких (животным в трахею вводили в течение 4 недель раствор карипазима), 3 -экспериментальная с моделированием буллезной эмфиземы легких с терапией препаратом «Солкосерил» (в течение 14 дней ежедневно вводили внутрибрюшинно солкосерил в необходимой дозировке в течение 14 дней). Для выполнения цитоморфометрического исследования при световом микроскопировании использовалось окрашивание препаратов: гематоксилин + эозин (Г+Э). Выполнялось гистологическое исследование микропрепаратов легких лабораторных животных, динамика изменений клеточного состава бронхиальной стенки определялась с помощью количественного подсчета эозинофилов, тучных клеток, лимфоцитов, нейтрофилов, макрофагов, фибробластов.Результаты. При буллезной эмфиземе наблюдаются изменения всех структурных элементов легкого: сочетание выраженного спазма и расширения бронхиол, деструкция стенок, расширение альвеолярных ходов, формирование множественных воздушных полостей, слущивание эпителия, перибронхиальная инфильтрация и фиброз; нарушения в системе кровообращения: признаки гиперемии и малокровия, сужение просвета артериол, гиалиноз их стенок. При терапии солкосерилом у животных выявлена выраженная гиперемия и расширение сосудов легких, стенки бронхов сохранны, участки деструкции не визуализируются, воздушные полости редко встречающиеся, в основном мелкие, плевра над которыми не повреждена.Заключение. Развитие буллезной эмфиземы приводит к формированию панацинарной эмфиземы, фиброза, периваскулярной и перибронхиальной инфильтрации (в инфильтрате -увеличение клеток воспалительного ряда), истончению плевры. Были получены данные, свидетельствующие, что препарат «Солкосерил» снижает выраженность деструкции легочной ткани, способствует уменьшение воспалительного процесса.Ключевые слова: буллезная эмфизема легких, спонтанный пневмоторакс, коллагенонез, солкосерил.
