Severe pancytopenia associated with a minor paroxysmal nocturnal haemoglobinuria clone and response to ciclosporin A in a heavily pretreated multiple myeloma patientParoxysmal nocturnal haemoglobinuria (PNH) is a rare acquired clonal haematopoietic stem cell disorder characterized by a somatic mutation in the gene, PIGA, resulting in a lack of glycosylphosphatidylinositol (GPI)-anchored molecules. The well-known association between PNH and aplastic anaemia suggests a PNH clone selection as a result of their relative resistance to immune attack. There are few data about the possible role of chemotherapy and radiotherapy in PNH clone expansion. Haemolytic PNH was described in two patients with breast cancer after doseintensive chemotherapy or radiotherapy (Hakim et al, 1996;Granel et al, 2002). We previously observed PNH-like phenotype without clinical manifestation in two of 20 heavily pretreated lymphoma patients (Kulagin et al, 1999). Here, we report a heavily pretreated patient with multiple myeloma (MM) who developed severe pancytopenia with a minor PNH clone and a good response to ciclosporin A (CsA) therapy.A 49-year-old male patient was initially admitted in November 1996 with severe bone pain, symptoms of hypercalcaemia and persistent elevation of the erythrocyte sedimentation rate. Numerous bone lesions in the skull, ribs, pelvis and vertebrae with an L5 vertebral fracture and three extramedullary components were revealed by X-ray examination and computed tomography (CT) scan. Serum protein electrophoresis and quantification of serum immunoglobulins had identified IgG paraproteinemia (37 g/l). Full blood count (FBC) and serum creatinine were normal. Repeated random bone marrow (BM) aspirations did not show elevated numbers of plasma cells. The diagnosis of MM had been confirmed by excision biopsy of the rib, which showed complete infiltration by atypical plasma cells in the lytic lesion. The patient was

Цель исследования: оценить эффективность и безопасность длительной терапии адалимума-бом в сочетании с иммунодепрессантами у пациентов с ЮИА без системных проявлений. Методы: проведено моно-центровое наблюдательное сравнительное исследование. Изучали результаты лечения пациентов с ЮИА, получавших адалимумаб в сочетании с иммунодепрессантами (n = 215) и метотрексат (n = 200). Оценку эффективности терапии проводили с использованием педиатрических критериев Американской коллегии ревматологов (АКР педи) ОБОСНОВАНИЕЮвенильный идиопатический артрит (ЮИА) -системное хроническое заболевание, развивающее-ся у детей в возрасте до 16 лет, характеризующее-ся преимущественным поражением суставов, а также патологией других органов и тканей с формировани-ем полиорганной недостаточности различной степени выраженности [1].Лечение ювенильного артрита остается одной из наиболее сложных и актуальных проблем ревма-тологии [2]. В лечении этого заболевания используют широкий спектр противоревматических лекарственных средств: нестероидные противовоспалительные пре-параты (НПВП), глюкокортикоиды (для перорального, внутривенного и внутрисуставного введения), хиноли-новые производные, Д-пеницилламин, препараты золо-та, сульфасалазин, лефлуномид; иммунодепрессанты из группы цитостатиков (циклофосфамид, азатиоприн, хлорамбуцил), антиметаболитов (метотрексат); селек-тивные иммунодепрессанты (циклоспорин) [3,4]. Однако у многих пациентов по-прежнему не удается достичь стойкой ремиссии и контроля над заболеванием при помощи базисных противоревматических препаратов [5,6]. Угроза инвалидизации таких больных определяет необходимость разработки и внедрения новых подходов к патогенетической терапии, основанных на современ-465 ных медицинских технологиях и расшифровке фундамен-тальных механизмов развития болезни [7].Значительный прогресс в терапии ревматических болезней связан с внедрением в клиническую практику принципиального нового класса препаратов, объеди-ненных общим термином «генно-инженерные биологи-ческие препараты» (ГИБП) [8][9][10]. ГИБП -это проти-вовоспалительные лекарственные средства, которые производят с применением биотехнологий. Мишенями для ГИБП служат цитокины, их рецепторы, CD и кости-мулирующие молекулы и др. [11]. Основным преимуще-ством биологической терапии является максимальная избирательность воздействия на иммунную систему, позволяющая устранить необходимое звено в патогене-тической цепи, не воздействуя на клетки других органов и систем [12].Центральное звено в патогенезе ЮИА без систем-ных проявлений -фактор некроза опухоли (ФНО) ␣. ВВЕДЕНИЕ Юношеский артрит -одна из наиболее часто встре-чающихся инвалидизирующих ревматических болезней у детей. Основным ее клиническим признаком является артрит. Патологические изменения в суставе характери-зуются болью, припухлостью, деформациями [1].У детей наиболее часто поражаются крупные и сред-ние суставы, в частности коленные, голеностопные, луче-запястные, локтевые, тазобедренные, реже -мелкие суставы кистей и стоп. Пролиферативно-экссудативные изменения в суставах быстро приводят к развитию стой-ких деформаций и контрактур, амиотрофии, гипотрофии [2]. Юношеский артрит, как правило, имеет прогресси-рующее течение и приводит к ранней инвалидизации и снижению качества жизни пациентов. Кроме того, многими авторами сообщается, что ювенильный ревма-тоидный артрит уменьшает продолжительность жизни больных в среднем на 10 лет, хотя при контролируемом многолетнем лечении она может быть сопоставима с популяционными значениями [3].Ранняя диагностика и своевременное начало адек-ватной иммуносупрессивной терапии (еще до появления деструктивных изменений в суставах и инвалидизации пациента) способны изменить прогноз, уменьшить степень инвалидизации и вернуть ребенка к полноценной жизни.Основой патогенетического подхода к лечению юно-шеского артрита является назначение иммуносупрес-сивной терапии [4][5][6][7][8][9].
ВВЕДЕНИЕ Ювенильный идиопатический артрит (ЮИА) -тяже-лое, хроническое, неуклонно прогрессирующее заболе-вание, в основе развития которого лежит активация кле-точного и гуморального звена иммунитета. Иммунное воспаление приводит к развитию хронического сино-вита и экстраартикулярных проявлений, результатом которых нередко становится полиорганная недостаточ-ность, деструкция хрящевой и костной ткани суставов. Подобные изменения являются причиной инвалидиза-ции, а в некоторых случаях -смерти пациентов [1].Контроль течения ЮИА достигается только примене-нием патогенетической терапии и подразумевает пода-вление активности воспалительного синовита, макси-мальное восстановление функции суставов, замедление прогрессирования деструктивных изменений в суста-вах, контроль системных проявлений болезни, а так-же купирование боли и уменьшение психологических ограничений, связанных с заболеванием. В настоящее время к противоревматической терапии предъявляются требования достижения ремиссии или хотя бы очень низкой активности болезни [2][3][4].
