A coronary heart disease and diabetes mellitus type 2 are the most complicated and urgent problem of medical science and public health. The standard therapeutic measures in some cases do not provide significant clinical improvement of patients with a stable course of the disease. This requires further study of the pathogenesis of the heart and vessels lesions in patients with combined pathology and the search for new medicines. In this work, the authors studied the state of microcirculation with the morphological assessment of the microvascular bed, considering the effectiveness of therapeutic effects of angiotensin-converting enzyme inhibitor lisinopril a daily dose of 5 mg in patients with ischemic heart disease (IHD), stable angina ΠΙ FC in combination with type 2 diabetes. Baseline, after 1 and 6 months of treatment, the authors conducted a conjunctival biomicroscopy of the eye vessels with a caliber measurement and mor-phometric analysis. The results of the study were the following: the combination of IHD and type 2 diabetes forms the gross disorders of microcirculation. This is reflected in the development of widespread perivascular edema, multiple hemorrhages, sudden depletion of the vascular bed with nonvascular lesions, intravascular aggregation of erythrocytes -"sludge-phenomenon". The prolonged complex use of lisinopril caused the positive dynamics of microcirculatory disorders, in particular, intravascular link with increasing flow velocity and decreasing the aggregation of erythrocytes. This is the basis for recommendations to the widespread use of this drug in this combined pathology.

Little is known about the mechanisms of the action of high humidity in the coastal cities on the course of coronary heart disease (CHD). Obviously, with high humidity, sweating and heat loss are difficult, and overheating increases the heart rate and exacerbates CHD. To assess the dynamics of age-related death rates from CHD, a comparative analysis of data on 11,134 deaths from CHD for 2002-2014 was conducted in the seaside town Makhachkala. The results of the study showed that mortality from CHD in Makhachkala decreased in the period 2002-2014 in men by 32.4% and in women by 40.8%, especially in young men and women aged 40-49. In the seaside town there is an increase in mortality from CHD with a low (40-50%) and high (> 70%) relative humidity, while the town Makhachkala is characterized mainly by high humidity. Thus, in the conditions of the town Makhachkala, a downward trend in the mortality rate of the population from CHD is observed, and a U-shaped relationship is observed between this index and the relative humidity of air.
Цель. Разработка блок-схемы краткосрочного персонифицированного про-гноза клинического исхода острого коронарного синдрома (Окс) в q-инфаркт миокарда на госпитальном этапе. Материал и методы. В работу были включены 68 пациентов с диагнозом Окс. В качестве прогностических факторов выступали наиболее информативные интервалы концентраций показателей субклинического неспецифического вос-паления, эндотелиальной дисфункции, кардиоспецифических изменений в иммунной системе и сывороточные кардиомаркёры. для тестирования выше-обозначенных показателей использовался метод твёрдофазного иммунофер-ментного анализа. Рассчитывалась величина относительного риска (ОР) с дове-рительными интервалами (ди), величина абсолютного риска (АР), диагностиче-ская эффективность (дэ) использованных методов в отношении клинического исхода Окс в q-иМ в связи с изучаемым фактором риска (интервалы концент-раций показателей). статистическую обработку данных проводили с помощью пакета программ statistica (версия 6,0), а также "Biostat 4.03". Результаты. На основании комплексного анализа полученных результатов была сформирована итоговая блок-схема краткосрочного персонифициро-ванного прогноза клинического исхода Окс в q-иМ. В неё вошли наиболее информативные интервалы концентраций изученных показателей. Персони-фицированный краткосрочный прогноз клинического исхода Окс в q-иМ на госпитальном этапе связан с интервалами концентраций кардиомаркеров: ТП-I от 2,2 до 2,7 нг/мл, BnP-32 от 1079 до 1270 пг/мл; маркеров воспаления: сРб от 20 до 25 мг/л, иЛ-1β от 0,54 до 1,54 пг/мл и ТНф-α от 1,1 до 1,6 пг/мл; марке-ров эндотелиальной дисфункции: nO от 7 до 11 мкмоль/л, эТ от 5 до 6 фмоль/мл, ММР-9 от 270 до 370 нг/мл и tIMP-1 от 140 до 150 нг/мл; иммуномаркеров: НП от 20 до 28 нмоль/л, АТ к кЛ от 15 до 20 Ед/мл и в 75% случаев встречаются АТ к миокардиальным клеткам. Пациенты, имеющие указанные показатели при поступлении в стационар, являются группой высокого риска в отношении кли-нического исхода Окс в q-иМ. Заключение. В случаях, когда при поступлении в стационар значения показа-телей воспаления, эндотелиальной дисфункции, сывороточных кардиомар-кёров и иммуномаркёров попадают в интервалы концентраций, указанные в блок-схеме, эти пациенты относятся к группе высокого риска клинического исхода Окс в q-иМ. это позволяет уже на раннем этапе госпитализации про-вести лечебные мероприятия в соответствии со стандартами ведения больных с окклюзирующими поражениями коронарных сосудов, обуславливающих крупноочаговый иМ.
