SummaryThis article contains clinical guidelines and current approaches to diagnosis and treatment of idiopathic pulmonary fibrosis (IPF). The aims of devel opment this guidelines were to improve early detection and efficacy of pharmacological and non pharmacological therapy of IPF. Achieving these goals indicates improvement in medical care quality for these patients. These guidelines are intended to pulmonologists, therapeutists and other medical specialists, healthcare managers and other healthcare providers.
Treatment of patients with chronic obstructive pulmonary disease (COPD) is one of the most important tasks of clinical medicine due to high prevalence of and high mortality from COPD. Treatment of COPD as a heterogeneous disease requires different approaches depending on the severity and the clinical course of COPD. Clinical guidelines have become the principal document that regulates different aspects of management of COPD patients. Algorithms of management and clinical decision making are the crucial part of guidelines because they help achieving a balance between making the decision in different clinical situations and simple graphics. The simplified approach is important for implementation the algorithm into clinical practice. Different evidence-based therapeutic schemes proposed by experts of Russian Respiratory Society and other countries have been enclosed into this article with discussion about their strengths and limitations and possibility of practical use. The Russian algorithm reflects a current view on differentiated management of COPD patients and fits the requirements of Healthcare Ministry of Russian Federation and needs of achieving the optimal balance between high informative significance and a simple graphics of the scheme.
РезюмеНаиболее важными симптомами при хронической обструктивной болезни легких (ХОБЛ) являются одышка, кашель и продукция мок роты. По данным различных исследований, у пациентов с ХОБЛ на текущей терапии клинически значимая одышка сохраняется в 50-70 % случаев. Одышка имеет важное прогностическое значение, коррелируя с частотой обострений и выживаемостью больных. Важные кри терии эффективности проводимой терапии -снижение выраженности симптомов (в первую очередь -одышки). Инструменты измере ния одышки несовершенны, понятие -субъективно, однако существующие количественные инструменты ее оценки позволяют опре делить степень выраженности симптомов у пациентов с ХОБЛ. Применяемые инструменты (модифицированная шкала Британского Медицинского cовета -Modified Medical Research Council; COPD Assessment Test -САТ) должны быть адаптированы к уровню оказания медицинской помощи (первичное звено, специалист и т. д.). Длительно действующие бронходилататоры являются обязательным ком понентом терапии больного ХОБЛ независимо от его фенотипа. Пациенты, получающие комбинированные лекарственные средства, включающие ингаляционные глюкокортикостероиды (иГКС), должны быть фенотипированы для определения показаний для назначе ния ГКС. Длительная терапия иГКС показана пациентам следующих групп: с сочетанием бронхиальной астмы и ХОБЛ; с высоким рис ком обострений (объем форсированного выдоха за 1 ю секунду (ОФВ1) < 50 %долж. при ≥ 2 неинфекционных обострениях в год или с ≥ 1 госпитализацией по поводу неинфекционного обострения); с эозинофилией мокроты > 3 % и / или > 300 клеток в 1 мл крови с учетом тяжести течения заболевания. При назначении иГКС необходимо учитывать риск побочных эффектов. Не следует добавлять иГКС при ОФВ1 ≥ 50 %, у пациентов с < 2 обострениями или отсутствием указаний на госпитализацию по поводу обострения ХОБЛ в течение 1 года; иГКС могут быть отменены, если они были назначены не по показаниям. У пациентов с сохраняющейся симптоматикой на фо не монотерапии длительно действующими бронходилататорами, иГКС / длительно действующими β2 агонистами или без четких пока заний к назначению иГКС оптимальным выбором может послужить комбинация индакатерол / гликопирроний. One of the most common comorbid condition in people over 40 years old is: arterial hypertension (AH) and chronic obstructive pulmonary disease (COPD). The frequency of AH in patients with COPD varies from 6.8 to 76.3%, in average 34.3%. COPD is detected in every fourth patient with hypertension in the age group of 2564 years. The current trend towards an increase in life expectancy and therefore growing pool of elder cohort, will lead to a higher number of patients with comorbid disorders. Diagnosis and treatment of AH and COPD are determined by current clinical recommendations for both nosologies, however, a number of mutual pathophysiological mechanisms lead to a more severe course of these diseases with frequent exacerbations. The choice of antihypertensive therapy in patients with AH in combination with COPD should be given to drugs that can provide an adequate decrease in blood pressure in hypoxic conditions, especially at night and early morning hours, that have prolonged effect or could be prescribed in the evening, and to those that dont worsen bronchial obstruction or exacerbate hypoxia. Patients with AH and COPD should be given recommendations on lifestyle changes, especially smoking cessation. As initial therapy calcium channel blockers, angiotensin receptor blockers or angiotensin-converting enzyme inhibitors should be considered. Calcium channel blockers/renin-angiotensin system blockers should be considered as the first line for combination therapy. Thiazide, thiazide-like diuretics orb1-selective adrenergic blockers could be prescribed in case of insufficient antihypertensive response or depending on different clinical scenarios. Bronchodilators are the baseline therapy in COPD with concomitant AH. According to modern concepts, prescription of combination therapy with different mechanisms of action is the most proven and justified approach, which leads to a decrease in the frequency of exacerbations of COPD and amelioration of the symptoms. The choice of bronchodilator in the case of a COPD and AH combination, should take into account the proven long-term safety regarding the risk of cardiovascular complications. Tiotropium bromide as the monotherapy, including as a liquid inhaler and aclidinium/formoterol, tiotropium/oladeterol as combination therapy showed cardiovascular safety in the long-term studies. The escalation of COPD therapy with the need of inhaled steroids requires careful monitoring of blood pressure and, possibly, a revision of antihypertensive treatment leading to its escalation as well. COPD has many phenotypes requiring different medications, eg.: roflumilast, theophylline, macrolides and mucoactive drugs, that could also require tighter blood pressure control on patients with COPD and AH.
