Renal arteriovenous fistulas are pathologic communications between the renal arteries and veins without interconnecting capillaries. Reports on the combination of fibromuscular dysplasia, aneurysms, and renal arteriovenous fistula are extremely rare in the literature. In the case of renal arteriovenous aneurysm rupture, urgent nephrectomy was the only life-saving procedure reported. The ex vivo approach seems to be an appropriate alternative to nephrectomy. This article presents a case of successful treatment of bilateral fibromuscular dysplasia with aneurysm and arteriovenous fistulas complicated by renal artery aneurysm rupture.

This article provides indications for and description of surgical technique of carotid endarterectomy with the formation of a new bifurcation. It also presents the results of observations in the immediate postoperative period based on the findings of this surgery among patients of Vishnevsky National Medical Research Center of Surgery.

Leiomyosarcoma of the inferior vena cava (IVC) is a rare malignancy. Approximately 350 cases of this disease are known worldwide. Surgical treatment of this condition remains an extremely difficult task. In this case report, we present an experience of successful radical treatment of a leiomyosarcoma of the suprarenal IVC without vascular reconstruction. The patient was treated with resection of the suprarenal portion of the IVC with the tumor; however, in view of adequate blood outflow from the right lobe of the liver, prosthetic replacement of the IVC with right hepatic vein reimplantation was not performed.

Adipose issue is a source of mesenchymal stem cells (MSC) that can be used to stimulate blood vessel growth in ischemic tissues. Various metabolicdisorders including hypeglycemia may have negative effect on therapeutic properties of these cells. Aim. To study the influence of high glucose concentration on functional activity in human adipose tissue. Materials and methods. Flow cytometry and real time PCR were used to study functional activity of cultured MSC from human adipose issue at highglucose concentration. Results. Prolonged (10-12 days) incubation at a high glucose concentration (25 mM) suppressed the ability of MSC to stimulate angiogenesis. Also,glucose modified expression of genes activating and inhibiting angiogenesis but had no effect on MSC proliferation and apoptosis. Conclusion. High glucose concentration suppresses angiogenic activity of MSC in adipose tissue; it may account for incomplete restoration of bloodflow in diabetic patients.

Extracorporeal partial nephrectomy under pharmaco-cold ischemia without ureter intersection with orthotopic replantation of renal vessels for RCC is a relatively safe organ-sparing treatment. It provides resection of large tumors with any location under prolonged cold ischemia without ureter intersection.

Экстракорпоральные операции на почках и их сосудах являются малознакомой областью хирургии для медицинской общественности. Данный хирургический подход подразумевает выполнение операции на органе вне тела или, иными словами, -ex vivo. Тем не менее подобные вмешательства имеют ряд неоспоримых преимуществ в особых клинических ситуациях. К ним относятся: ишемии почки, лучшая визуализация, работа в бескровном поле и практически неограниченное время, безопасная для функции почки. Однако, несмотря на все достоинства данного хирургического подхода, он не получил широкого распространения в повседневной клинической практике ввиду технической сложности операции. В результате анализа имеющейся литературы становится очевидно, что данные операции могут быть эффективны при целом ряде заболеваний. Особо остро проблема радикального хирургического лечения встает при невозможности стандартного подхода, когда операция выполняется in situ. В подобных случаях выполнение экстракорпоральной операции с последующей аутотрансплантацией почки позволяет выполнить радикальное лечение при особо выраженном поражении почки и/или ее сосудов. Приводится обзор литературы, в котором представлена история становления и развития экстракорпоральных операций на почках и сосудах почки. В статье хронологически описаны экспериментальные экстракорпоральные вмешательства с аутотрансплантацией почки и клинические наблюдения. Обобщены литературные данные выполнения экстракорпоральных операций при широком спектре различных заболеваний. Максимально широко отражен отечественный и зарубежный опыт подобных вмешательств. Описаны этапы развития экстракорпоральной хирургии почек с первых клинических наблюдений до настоящего времени. Приведены отдаленные результаты наиболее крупных исследований, указывающие на уместность, целесообразность и обоснованность выполнения подобных операций. Особую актуальность данная хирургическая техника приобретает при невозможности стандартного, интракорпорального вмешательства. Таким образом, применение экстракорпоральной техники операции позволяет расширить возможности оказания хирургической помощи больным с различной патологией почек и/или их сосудов.

Vascular leiomyosarcoma is a rare tumor originating from the vascular smooth muscle cells. Leiomyosarcomas of the external iliac vein are extremely rare. Only single observations of this tumor are reported. We describe a 48-year-old woman who underwent a two-step combined treatment: robot-assisted removal of the extraperitoneal tumor and removal of the lower part of the tumor from the open femoral approach. There have been no signs of tumor recurrence during the 34-month follow-up period.

Renal arteriovenous fistula is a comparatively rare malformation that accounts for < 1% of all arteriovenous fistulas malformations. Renal ANM are devided into congenital or acquired, traumatic and nontraumatic, that can cause massive hematuria, pain, retroperitoneal hemorrhage, and heart failure. Non-invasive imaging modalities, including US, MDCT and MRI, are widely used to diagnose this payhology, but the gold standard for the diagnosis of renal AVM remains angiography. The advantage of angiography in comparison with other methods is the possibility of simultaneous treatment of such patients. The treatment of renal AVM can include endovascular dissociation of the arteriovenous fistula or an open operation, including the using of ex vivo techniques. The choice of method of the treatment is based on the general patient status and the characteristics of renal arteriovenous malformation.
