<p>Treatment of patients with multilevel arterial occlusive disease in lower limb arteries is one of the challenges in vascular surgery. The need in blood flow restoration in several arterial segments usually leads to the problem of choosing a surgical procedure. Open surgery of several arterial segments is linked with a high risk of intraoperative complications, while endovascular procedures are not always possible, especially with long lesions. In such cases, hybrid surgery turns out to be a method of choice. We report the case of 50-year old patient with complaints of the right lower limb pain at rest, trophic ulcers in the right foot. The medical record showed 5-year claudification, with a gradual decrease in painless walking distance. In 2017, he underwent right common iliac artery stenting without a positive effect. Upon admission, occlusion of the right external iliac artery and superficial femoral artery was identified. After additional examination, crossing of lesions, balloon angioplasty and stenting of aorto-iliac and femoral-popliteal segments were successfully performed. The presented clinical case demonstrates high effectiveness in combination of open and endovascular procedures, while treating multilevel arterial occlusive disease. Hybrid surgical procedures allow to reduce traumatism, risk of postoperative complications and length of in-hospital stay. The main advantages and disadvantages of current surgical methods for treatment of multilevel aorto-iliac-femoral lesions are discussed. Performing hybrid surgical procedures is possible only with an experienced vascular team and in specialized hybrid operating room.</p><p>Received 3 August 2018. Revised 26 November 2018. РезюмеТромбоэмболия легочной артерии является жизнеугрожающим состоянием вследствие развития острой правожелудочковой недостаточности и кардиогенного шока. Основным методом реперфузии при массивной легочной эмболии остается системный тромболизис, проведение которого сопряжено с риском фатальных геморрагических осложнений. В течение последних двух десятилетий наблюдается все больший интерес к использованию эндоваскулярных технологий, позволяющих восстановить кровоток по окклюзированным легочным артериям в сроки до трех недель от верифицированного эпизода эмболии и снизить риск больших кровотечений. Проводимые в настоящее время исследования подтверждают высокую эффективность и безопасность интервенционных методов лечения, внедрение которых в клиническую практику позволит улучшить прогноз больных с массивной тромбоэмболией легочной артерии.
Purpose. The purpose of the article is to access possibilities of blood flow mathematical analysis in aortic aneurysm before and after bare metal stent implantation.Materials and methods. Mathematical models of aortic blood flow were based on data received at studying 15 CT-scans of patients with abdominal aorta aneurysms (12) and dissections (3) and their duplex ultra-sound hemodynamic data. At constructing mathematical model the program SolidWorks was used. Working with the program consisted of two stages: establishment of conditions for geometric objects; forming of abdominal aorta model from these objects. In the study hemodinamic aneurysm indexes was evaluated on rectilinear and curvilinear segments. Some of characteristics were variable: diameter, aneurysm wall thickness, its length, elasticity.Results. Correlation of extreme tension into aneurysm wall on rectilinear and curvilinear segments according to aneurysm wall thickness was assessed. Possibilities of pathological blood flow changes correction at bare metal stent implantation into aortic aneurysm were estimated: if presence of bare metal stent were introduced into mathematical parameters blood flow characteristics became almost as standard characteristics. Received data can enhance successful endovascular treatment of aortic diseases with using of bare metal stents.Conclusion. Mathematical models of aortic and vascular aneurysms before and after surgery can be an effective tool in bettering quality of medical help for vascular patients.
Резюме Цель исследования: оценить эффективность комплексного лечения пациентов с синдромом диабетической стопы в условиях одного медицинского центра за одну госпитализацию. Материалы и методы: с начала работы научно-исследовательского отдела сосудистой и интервенционной хирургии ФГБУ «НМИЦ им. В. А. Алмазова» (с января 2017 по октябрь 2017), придерживаясь мультидисциплинарного подхода, пролечено 28 пациентов с ишемической и нейроишемической формами синдрома диабетической стопы. У 24 из них были трофические язвы на стопах. Средний ЛПИ составил 0.44 (от 0,2 до 0,9). Лечение проходило в три этапа. Первый этап проходил в отделении эндокринологии Центра и заключался в обследовании и подготовке пациентов к реваскуляризации. Выполнялось дообследование на предмет значимых сопутствующих заболеваний, коррекция гликемического профиля, компенсация других поздних осложнений СД и купирование системного воспаления. На втором этапе лечение пациентов проходило в отделении сосудистой и гибридной хирургии, где выполнялась непосредственно реконструкция артерий нижних конечностей. После операции пациенты продолжали лечение в отделении эндокринологии Центра, где осуществлялся контроль гликемии, коррекция терапии сахарного диабета, выполнялись санации язв и органосохраняющие пластические операции на стопе. Оценивалось изменение выраженности болевого синдрома, динамика заживления язв, перспективы сохранения конечности. Результаты и обсуждение: для оценки уровня и распространения сосудистого поражения на догоспитальном этапе выполнялась МСКТ-или рентген-контрастная ангиография артерий нижних конечностей. В большинстве случаев наблюдалась дистальная форма атеросклероза с преимущественным поражением подколенно-берцового сегмента. Эндоваскулярное лечение выполнено 23 (82%) больным, 4 пациентам (14%) выполнены открытые вмешательства. В одном случае применён гибридный подход. Все эндоваскулярные и гибридная операции выполнялись в условиях гибридной операционной под местной анестезией. В раннем послеоперационном периоде у 79% пациентов достигнуто клиническое улучшение в виде купирования явлений критической ишемии конечности, болей покоя, положительной динамики заживления трофических язв. Семерым пациентам (25%) потребовалась расширенная некрэктомия в пределах стопы с пластикой местными тканями. В отдалённом периоде (от 3-х до 8-и мес.) наблюдения один летальный исход, вследствие инфаркта миокарда, ампутаций не было. У 85% пациентов полное или частичное заживление язвы. Выводы: комплексное мультидисциплинарное лечение пациентов с синдромом диабетической стопы в условиях одного медицинского учреждения способствует индивидуальному подходу к каждому пациенту и улучшению качества оказанной медицинской помощи. <p>Diseases of peripheral arteries are considered to be one of the main reasons for hospitalization of patients in specialized departments of vascular surgery. Patients with atherosclerotic lesions of the lower limb vessels, especially with prolonged occlusions of the aorto-iliac arterial segments, usually undergo open shunt interventions. The aim of the case is to demonstrate successful endovascular treatment of prolonged occlusion of the aorta and iliac arteries. A 54-year-old patient complained of pain in both legs while walking for more than 100 m. The history of the disease was at least 5 years. The examination revealed occlusion of the abdominal aorta at the level of the lower third, common iliac arteries. Given the anatomical features of atherosclerotic lesion, the presence of a residual aortic lumen that could be used as a guiding cult for recanalization of the occluded aorto-iliac segment and a pronounced comorbid background, it was decided to perform endovascular surgery. Stenting of the infrarenal aorta, right and left common iliac arteries yielded good angiographic and clinical results. The case report demonstrates high efficiency of endovascular treatment of the Lerish syndrome and thus reducing the risk of perioperative complications, the length of the patient's stay in the hospital. Such complex and volumetric operations can be performed only with the appropriate equipment, the presence of a hybrid operating room and coordinated work of the “vascular” team the members of which have complete mastery over open and endovascular surgery.<br /><br />Received 14 May 2018.
