The article presents the data on the state of endothelial function in the normally and in various diseases and conditions. The basic functions of endothelium in modulation of vascular tone, atrombogenicity and thrombogenicity of the vascular wall, regulation of vascular wall adhesion, regulation of vascular growth are described. The main causes leading to the formation of endothelial dysfunction and the mechanisms underlying it are highlighted. Numerous studies on the evaluation of endothelial function in various diseases are presented. The basic methods of drug and non-drug correction of endothelial dysfunction are presented.
Cardiovascular diseases in Russia are leading in the structure of total mortality. Atherosclerosis is considered a progressive inflammatory systemic disease. The role of endothelium in the development of the atherosclerotic process is described in detail. The main functions of endotheliocytes are normal and in various pathological conditions. The main markers of endothelial dysfunction are presented. Data on the development of the atherosclerotic process in time, risk factors are presented. Local and systemic risk factors for atherosclerosis are highlighted. Own data on the frequency of occurrence of atherosclerotic changes in the main vessels of the head in young people (up to 45 years) based on the results of ultrasonic duplex scanning during professional examinations are presented. The main directions of correction of endothelial dysfunction are described.
Epilepsy is a common chronic neurological disease. The problems of people with epilepsy go well beyond epileptic seizures. Comorbidities in epilepsy are very common and often cause more problems to the patients than the seizures themselves. Although seizures are the most prominent clinical presentation of epilepsy, people with epilepsy are exposed to risk of not only seizures, but also of many health problems. Both children and adults with epilepsy often complain of memory impairment. It is commonly believed that cognitive dysfunction in epilepsy is multifactorial. The components that deteriorate cognitive functions include active seizures and especially generalized tonic-clonic seizures, traumatic brain injuries, structural epilepsy, and drug therapy. Cognitive dysfunction is very often present as far back as during the onset of epilepsy. The cognitive dysfunction detected in patients with epilepsy depend on the localization of the epileptic focus, the frequency and type of epileptic seizures, and changes in the interictal electroencephalogram. Seizures can be controlled with antiepileptic drugs in most patients with epilepsy. Therapy of cognitive dysfunction in patients with epilepsy presents significant difficulties, as there is no evidence of the efficacy of various drugs in cognitive disorders. The article presents a new Russian antiepileptic drug based on phenosanic acid as part of combination therapy in patients with partial epileptic seizures with or without secondary generalization, which can improve cognitive functions in patients with epilepsy.
A new 2019 coronavirus disease has been spreading worldwide for more than a year, with a high risk of infection and death. Various sequelae and complications can develop in COVID-19 survivors, lasting from several weeks to several months after initial recovery, affecting different organs and systems. Various sequelae and complications can occur in COVID-19 survivors not only in adults and the elderly, but also in young people. A wide range of neurological manifestations of COVID-19 are now described in the available literature. The incidence of selected neurological symptoms, syndromes and nosological forms in individuals both in the acute period of COVID-19 disease and in the short- and long-term follow-up of these patients is presented. In this article, cognitive impairments occurring in individuals who have had coronavirus disease are discussed in depth. Data on the prevalence of cognitive impairment in different regions and at different periods of the disease are presented. The main possible pathophysiological processes and risk factors for the development of cognitive impairment in COVID-19 are described. Possible ways of drug and non-drug rehabilitation of patients with cognitive impairment in coronavirus infection that is a new problem of modern medicine are considered. Attention is also paid to neuroprotection as one of the therapy areas.
