Introduction: Primary immunodeficiencies (PID) are a group of rare genetic disorders with a multitude of clinical symptoms. Characterization of epidemiological and clinical data via national registries has proven to be a valuable tool of studying these diseases. Materials and Methods: The Russian PID registry was set up in 2017, by the National Association of Experts in PID (NAEPID). It is a secure, internet-based database that includes detailed clinical, laboratory, and therapeutic data on PID patients of all ages. Results: The registry contained information on 2,728 patients (60% males, 40% females), from all Federal Districts of the Russian Federation. 1,851/2,728 (68%) were alive, 1,426/1,851 (77%) were children and 425/1,851 (23%) were adults. PID was diagnosed before the age of 18 in 2,192 patients (88%). Antibody defects (699; 26%) and syndromic PID (591; 22%) were the most common groups of PID. The minimum overall PID prevalence in the Russian population was 1.3:100,000 people; the estimated PID birth rate is 5.7 per 100,000 live births. The number of newly diagnosed patients per year increased dramatically, reaching the maximum of 331 patients in 2018. The overall mortality rate was 9.8%. Genetic testing has been performed in 1,740 patients and genetic defects were identified in 1,344 of them (77.2%). The median diagnostic delay was 2 years; this varied from 4 months to 11 years, depending on the PID category. The shortest time to diagnosis was noted in the combined PIDs-in WAS, DGS, and CGD. The longest delay was observed in AT, NBS, and in the most prevalent adult PID: HAE and CVID. Of the patients, 1,622 had symptomatic treatment information: 843 (52%) received IG treatment, mainly IVIG (96%), and 414 (25%) patients were treated with biological drugs. HSCT has been performed in 342/2,728 (16%) patients, of whom 67% are currently alive, 17% deceased, and 16% lost to follow-up. Three patients underwent gene therapy for WAS; all are currently alive. Conclusions: Here, we describe our first analysis of the epidemiological features of PID in Russia, allowing us to highlight the main challenges around PID diagnosis and treatment.
Changes in sputum bacterial landscape after vaccination against Streptococcus pneumoniae, Haemophilus influenzae type b and influenza in patients with chronic obstructive pul monary disease SummaryThe influence of combined vaccination against Streptococcus pneumoniae, Haemophilus influenzae type b and influenza on sputum bacterial land scape in patients with chronic obstructive pulmonary disease (COPD) has been analyzed in this article. The 1 st group of 31 COPD patients (mean age, 59.25 ± 1.12 years) were vaccinated simultaneously with "Pneumo 23", "Hiberix", "Grippol plus". The 2 nd group consisted of 49 unvaccinated COPD patients (mean age, 57.16 ± 0.98 years). We used a classical microbiological method. Three months after the complete vaccination, there was a significant decrease in number of patients with S. pneumoniae yielded in sputum in comparison with unvaccinated COPD patients. РезюмеЦелью работы было изучение влияния комплексной вакцинации против пневмококка, гемофильной инфекции типа b и гриппа на мик робный пейзаж мокроты через 3 мес. после вакцинации у больных с хронической обструктивной болезнью легких (ХОБЛ) по сравне нию с невакцинированными пациентами. Первую группу составили пациенты с ХОБЛ (n = 31), одномоментно вакцинированные "Пневмо 23", "Хиберикс", "Гриппол плюс" (средний возраст -59,25 ± 1,12 года), вторую группу -невакцинированные пациенты с ХОБЛ (n = 49) (средний возраст -57,16 ± 0,98 года). Использовался классический микробиологический метод. Через 3 мес. после комплексной вакцинации отмечено достоверное снижение числа пациентов, выделяющих с мокротой Streptococcus pneumoniae, по срав нению с невакцинированными больными ХОБЛ. Chronic obstructive pulmonary disease (COPD) is a serious problem for global health. Infectious agents play a main role in the development of COPD exacerbations. Bacterial colonization of the lower respiratory tract is common in patients with stable COPD. The role of microbiota and host immune response to potential pathogens is not well studied. Microbiota composition disorders in respiratory tract are found in patients with COPD and associated with maladaptive changes in the immune system of the lungs and increased level of inflammation. This review investigates role of microbiota in the pathogenesis of COPD and its impact on the course of the disease. Some important issues such as pneumococcal vaccination and antimicrobial resistance of respiratory pathogens are also discussed.

Это связано с высокой распространенностью данного заболевания в мире (≈ 64 млн больных), рас тущей смертностью (по прогнозам, в течение бли жайших 10 лет общая смертность, обусловленная ХОБЛ, увеличится более чем на 30 %) и социально экономическими затратами [1].Основными возбудителями инфекционного обост рения ХОБЛ являются Streptococcus pneumoniae, Haemo philus influenzae, Moraxella catarrhalis [2,3], в связи с чем необходимость вакцинации пациентов с ХОБЛ против пневмококковой инфекции не вызывает сомнений. РезюмеПриводятся сравнительные данные динамики показателей качества жизни (КЖ) у пациентов с хронической обструктивной болезнью легких (ХОБЛ) на фоне вакцинации против пневмококковой инфекции с использованием 13 валентной конъюгированной вакцины "Превенар 13" (ПКВ 13) или 23 валентной полисахаридной вакцины "Пневмо 23" (ППВ 23). Цель: сравнительная оценка влияния вакцинации против пневмококковой инфекции на показатели КЖ у пациентов с ХОБЛ за 12 мес. при использовании ПКВ 13 и ППВ 23. Материалы и методы. В исследовании приняли участие больные ХОБЛ (n = 58). Пациенты, соответствующие критериям включения / исключения, в амбулаторных условиях получили внутримышечно в левое плечо 1 дозу вакцины ПКВ 13 (1 я группа; n = 33) либо ППВ 23 (2 я группа; n = 25). Оценивалась динамика КЖ вакцинированных с использованием COPD Assessment Test (САТ) за 1 год до вакцинации и в течение 1 года после. Результаты. При вакцинации пациентов с ХОБЛ ПКВ 13 отмечено снижение показателей САТ на 10,3 балла; в группе ППВ 23 данный показатель снизился на 8,8 балла (р < 0,05). Заключение. Вакцинация больных ХОБЛ против пневмококковой инфекции с использованием ПКВ 13 или ППВ 23 способствует достоверному улучшению показателей КЖ. При наличии выбора введения ПКВ 13 или ППВ 23 у пациентов с ХОБЛ следует отдавать предпочтение в пользу конъ югированного вакцинного препарата. Требуются дополнительные исследования микробиологического, иммунологического и других эффектов при использовании ПКВ 13 и ППВ 23 у пациентов с ХОБЛ, а не только влияние на показатели КЖ. SummaryThe aim of this study was to evaluate an impact of 13 valent conjugate pneumococcal vaccine (PCV 13) and 23 valent polysaccharide pneumococcal vaccine (PPV 23) on quality of life of patients with chronic obstructive pulmonary disease. Methods. We estimated quality of life from baseline to 12 months after the vaccination using the COPD Assessment Test (CAT). Results. The study involved 58 patients with COPD vaccinated with PCV 13 (n = 33) or PPV 23 (n = 25). The CAT score reduced by 10.3 in patients vaccinated with PCV 13 and by 8.8 in patients vaccinated with PPV 23 (p < 0.05). Conclusion. Vaccination of COPD patients with PCV 13 or PPV 23 could significantly improve quality of life. The conjugate vaccine PCV 13 is more preferable.
