We report a progressive disruption of 24-h rhythms in fasting blood glucose (FBG), body temperature (BT) and heart rate (HR) associated with metabolic dysfunction and the development of prediabetes (PD) and type 2 diabetes mellitus (T2DM) in overweight middle-aged (40-69 years old) humans. Increasing BT and HR mean values and declining 24-h BT and HR amplitudes accompany adverse changes in metabolic state. Increased nocturnal BT and a phase delay of the 24-h BT rhythm, deviant 24-h HR profile and a phase advance of the 24-h HR and FBG rhythms are early signs of the PD metabolic state. In T2DM, the 24-h FBG rhythm is no longer detectable, and the 24-h amplitudes of BT and HR are greatly diminished. In addition, lepton and creatinine values were lowered in T2DM. Moreover, positive correlations between FBG and body mass index, BMI, and negative correlations between the 24-h amplitude of FBG and BMI indicate that overweight is an additional factor causing disruption of the circadian rhythms. Further studies on circadian disruption as a consequence of metabolic dysfunction are necessary. The quantitative analysis of changing circadian BT and HR rhythms may provide prognostic markers of T2DM and therapeutic targets for its prevention and correction.
В течение последних десятилетий исследователи активно обсуждают тему о биоритмах эндокринной системы, а именно их изменения при развитии нарушений углеводного обмена (НУО). Особый интерес с позиций хронобиологии представляет гормон шишковидного тела мелатонин. Циркулирующий в крови мелатонин может действовать как типичный гормон, достигая далеко расположенных клеток-мишеней, и играть ключевую роль в качестве паракринной сигнальной молекулы для регионарной координации клеточных функций [1]. Данный гормон синхронизирует гормональные стимулы и метаболические процессы с учетом меняющегося времени суток [2]. Мелатонин участвует в регуляции секреции инсулина и вносит вклад в патофизиологию нарушений углеводного обмена. Поэтому среди ученых идет активное обсуждение о перспективах применения мелатонина для лечения СД. Такие состояния, как нарушение гликемии натощак (НГН), нарушение толерантности к глюкозе (НТГ), ожирение, дислипидемия, на сегодняшний день уже являются самостоятельными патологическими процессами, требующими внимания врачей. Целью нашей работы было изучение циркадианных ритмов физиологических функций (температура тела, гликемия натощак, частота сердечных сокращений) и роли лептина в формировании нарушений углеводного обмена (НГН, НТГ, СД2). В ходе исследования было подтверждено, что в формировании НУО играет важную роль преобладание жировой ткани над мышечной, которое формирует абдоминальное ожирение на фоне внешних факторов среды и поведенческих особенностей человека, а также развитие лептино-и инсулинорезистентности, которое по мере прогрессирования нарушений метаболизма глюкозы приводит к инсулино-и лептинодефициту. В данной работе отражены изменения биологических ритмов, происходящие до манифестации СД2. Так, на фоне ожирения происходит изменение суточных колебаний БТ, гликемии натощак, ЧСС, а при развитии РНУО и СД2 усугубление метаболических нарушений приводит к изменению структуры циркадианных ритмов в организме, что, в свою очередь, может быть как следствием, так и причиной НУО. Нарушение показателей терморегуляции тесно связано с уровнем лептина в организме и указывает на вовлеченность в данный процесс гормона мелатонина, который после дополнительных исследований, возможно, может быть использован в качестве препарата терапии и профилактики нарушений углеводного обмена.

История вопроса В 1957 г. была сформулирована гипотеза, согласно которой проводником биологического действия соматотропного гормона (СТГ) в организме является циркулирующий в крови «сульфирующий фактор», содержание которого в крови увеличивается под действием СТГ. Несколько лет спустя в крови людей была обнаружена инсулиноподобная активность, которая не подавлялась антителами к инсулину. Позднее проводники биологического действия СТГ стали называться соматомединами [12, 13]. В периферических тканях именно ИФР-I обеспечивает практически все физиологические эффекты соматотропного гормона. Очистка и исследование веществ, определяющих неподавляемую антителами инсулиноподобную активность крови, закончились выделением двух пептидов и установлением их аминокислотных последовательностей [21, 32, 33]. Они проявили высокое сходство с проинсулином и были названы инсулиноподобными ростовыми факторами в связи с их способностью стимулировать поглощение глюкозы мышечной и жировой тканью аналогично инсулину. Рецепторы и сигнальный поток Выделены два вида инсулиноподобных факторов роста: ИФР-I и ИФР-II. Они представляют собой одноцепочечные полипептиды, состоящие из 70 и 67 аминокислотных остатков соответственно. Степень сходства между двумя этими гормонами достигает 62%, причем аминокислотные остатки в каждом из них на 50% идентичны таковым в инсулине.
