Сепсис остается одной из самых актуальных про блем современной медицины в силу неуклонной тен денции к росту заболеваемости, стабильно высокой ле тальности и постоянным обновлением литературы по данной проблеме. В настоящее время сепсис рассматри вают как одну из клинических форм синдрома систем ной воспалительной реакции (ССВР), где в качестве фактора, инициирующего повреждение, выступают ми кроорганизмы. Ключевой составляющей, определяю щей сущность ССВР, являются микроциркуляторные расстройства [1][2][3][4]. Цель исследования -оценить состояние системы микроциркуляции при критических состояниях, обусловленных аб доминальным сепсисом для дальнейшего дифференцированного подхода к интенсивной терапии. Материал и мето ды. Обследовано 24 пациента с абдоминальным сепсисом, средний возраст -42,9±0,9 года, контрольную группу со ставили 35 практически здоровых человек, средний возраст 40,1±2,1 лет, Состояние микроциркуляторного русла оценивалось на протяжении 11 суток методом накожной лазерной допплеровской флоуметрии, которую осуществля ли с помощью лазерного анализатора капиллярного кровотока (ЛАКК 02) отечественного производства (НПО «ЛАЗМА», РФ) с использованием базового световодного зонда для чрескожных исследований микроциркуляции. Результаты. На протяжении всего периода исследования у пациентов с сепсисом сохраняется перфузия тканей кро вью за счет повышения влияния на микрососудистое русло преимущественно активных компонентов регуляции сосу дистого тонуса. При неблагоприятном исходе происходит снижение как активных, так и пассивных регулирующих влияний на перфузию тканей преимущественно за счет локальных (миогенных) факторов. Заключение. Полученные в ходе исследования данные свидетельствуют об изменении регуляции микрокровотока у пациентов с сепсисом, на правленных на поддержание тканевой перфузии. Динамическое наблюдение за состоянием микроциркуляции может помочь в тактике интенсивной терапии и прогнозировании исхода заболевания. Subjects and methods. Twenty four patients with abdominal sepsis (mean age 42.9±0.9 years) were examined; a control group consisted of 35 apparently healthy individuals (mean age 40.1±2.1 years). Over 11 days, the microcirculatory bed was evaluated by cutaneous laser Doppler flowmetry by means of a ЛАКК 02 laser capillary blood flow analyzer made in the Russian Federation (LAZMA Research and Production Association), by using a basic light guide for percutaneous microcirculation studies. Results. Throughout the study, tis sue blood perfusion remained in the patients with sepsis due to the higher effect of mainly active components of vascu lar tone regulation on the microvascular bed.
