In children with respiratory symptoms and/or fever, RV but not IFV is associated with wheeze and an asthma-like presentation. In those with an asthma history, IFV causes more generalised and severe disease that may be better described as "asthma-augmented influenza" rather than an "asthma attack." Differences in the acute conditions caused by these viruses should be considered in the design of epidemiological studies.
PURPOSE:Influenza infection is associated with high hospitalization rates among young children.Rapid diagnosis of influenza infection is particularly useful in order to prevent nosocomial infection and allows for timely initiation of antiviral treatment.We evaluated the performance of a rapid influenza test in hospitalized children during the influenza season.
Μια προοπτική επιδημιολογική μελέτη - επιτήρηση των σχετιζόμενων με γρίπη νοσηλειών πραγματοποιήθηκε προκειμένου να υπολογιστεί η συχνότητα νοσηλείας βασισμένη σε πληθυσμιακά κριτήρια. Παιδιά, ηλικίας 6 μηνών έως 14 ετών που πληρούσαν συγκεκριμένα κριτήρια επιλογής και εισάγονταν για νοσηλεία σε ένα από τα δυο μεγαλύτερα παιδιατρικά νοσοκομεία της Αθήνας, περιελήφθηκαν στη μελέτη κατά τη διάρκεια δυο επιδημικών περιόδων. Ρινοφαρυγγικό έκπλυμα από κάθε ασθενή ελεγχόταν με PCR για εργαστηριακή επιβεβαίωση πιθανής λοίμωξης από τον ιό της γρίπης. Βρέθηκε ότι η γρίπη ευθύνεται για το 9.9–11.8% όλων των εισαγωγών κατά την διάρκεια των επιδημικών περιόδων της γρίπης, ενώ το ετήσιο ποσοστό νοσηλείας ανέρχεται σε 13.6–16.8 περιστατικά ανά 10,000 παιδιά και ήταν υψηλότερο σε παιδιά ηλικίας μικρότερης των 5 ετών (26–31.2/10,000 παιδιά).Οι πυρετικοί σπασμοί και η οξεία μέση πυώδης ωτίτιδα είναι οι δυο συχνότερες επιπλοκές σχετιζόμενες με τη γρίπη ενώ στο 61% των θετικών για γρίπη ασθενών χορηγήθηκαν αντιβιοτικά. Συνεπώς, η γρίπη σχετίζεται με υψηλά επίπεδα νοσηλείας στους παιδιατρικούς πληθυσμούς, γεγονός που μπορεί να περιοριστεί σημαντικά με το συστηματικό, καθολικό εμβολιασμό των μικρών παιδιών κατά της λοίμωξης αυτής.
