Некротизирующий саркоидный гранулематоз (НСГ) -продуктивный васкулит мелких артерий и вен с образова-нием массивных скоплений саркоидоподобных гранулем, сопровождающийся ишемическим некрозом разной сте-пени выраженности и давности. Заболевание поражает исключительно легкие. Этиология и патогенез НСГ оста-ются малоизученными из-за редкого выявления этого за-болевания, отсутствия патогномоничных лабораторных, лучевых и функциональных критериев. Согласно мирово-му опыту, основным методом диагностики НСГ остается морфологическое исследование [1][2][3][4].Термин «некротизирующий саркоидный гранулема-тоз» был впервые предложен в 1973 г. А. Liebov [5]. Автор описал 11 наблюдений пациентов, в биоптатах легких ко-торых гистологически был выявлен продуктивный васку-лит. Васкулит сопровождался наличием саркоидоподоб-ных гранулем с образованием конгломератов и некроти-ческими изменениями. При этом отсутствовала внутри-грудная лимфаденопатия, характерная для саркоидоза. У этих пациентов наступала регрессия процесса спонтан-но или после непродолжительной терапии. necrotizing sarcoid granulomatosis (nsG) belongs to productive small-vessel vasculitis with the formation of sarcoid-like granulomas, which is accompanied by ischemic necrosis of varying degrees and duration. The disease involves the lung only. The clinical symptoms of the disease are nonspecific so the latter is detected rather rarely. The main diagnostic technique is morphological examination. Immune complex inflammation develops in the vessel walls with the formation of macrophage-histiocytic granulomas that do not contain epithelioid cells. The etiology and pathogenesis of nsG remain little studied. Its differential diagnosis is mainly presented with tuberculosis, sarcoidosis, and granulomatosis with polyangiitis.
За период с 6 марта по 30 апреля 2020 г. в разных стационарах Москвы умерли 543 (219 женщин и 324 мужчины), средний возраст которых составил 67,47±14,67 года. Изучены легкие от 23 умерших с прижизненно подтвержденным диагнозом COVID-19 (ПЦР-исследование на материале из носоглотки, у части умерших положительный анализ ПЦР в материале, полученном из трахеи и ткани легких при аутопсии). Средний возраст этих умерших составил 72,27±10,74 года, длительность заболевания колебалась от 5 до 32 сут. При тяжелом течении COVID-19 у больных возникает вирусная интерстициальная пневмония с развитием диффузного альвеолярного повреждения, которое обусловливает тяжелое течение заболевания, гипоксию и дыхательную недостаточность. Обнаружено неполное соответствие между морфологическими изменениями в легких (фазы диффузного альвеолярного повреждения) и длительностью заболевания, что, вероятно, связано с неустановленной продолжительностью бессимптомного течения у многих пациентов. Течение новой коронавирусной инфекции характеризуется преимущественным повреждением легких, повышением концентрации гемоглобина в крови, снижением SpO 2 , лимфопенией с одновременным нарастанием лейкоцитоза, признаков гиперкоагуляции у крайне тяжелых больных. Возникновение вирусно-бактериальной пневмонии возможно прежде всего у лиц при интубации трахеи с трахеостомой и длительной ИВЛ на любом этапе течения заболевания.
Introduction. The pathogenesis of the novel coronavirus infection COVID-19 is being actively studied. Nevertheless, our up-to-date knowledge of lymphoid tissue response in the secondary immune organs dur-ing severe COVID-19 remains extremely limited. The aim of the study was to evaluate patterns of immu-nomorphological alterations in lymph nodes in patients with severe COVID-19 and to assess lymphocytes functional activity in them. Materials and methods. Lymph node tissue (autopsy material) from 17 deceased patients with severe COVID-19 was examined by histological and immunohistochemical methods using antibodies to CD4, CD8, CD20, CD30, CD123, CD138, PD-1. Results. Examined lymph nodes demonstrated lymphoid follicles reduction and paracortex expansion with reactive plasmacytosis and extrafollicular B-cell activation as well as sinus histiocytosis, variable hemophago-cytic cells, and blood vessel congestion. Red thrombi were observed in some lymph nodes. Hemorrhages in the stroma were frequent, and massive hemorrhages were found in individual nodes. Immunohistochemical study revealed CD4+ T-helpers predominance in the paracortex andcytotoxic CD8+ lymphocytes depletion together with an increase in the expression of both the PD-1 suppressor protein and the CD30 activation anti-gen on the lymphocyte surface. CD123-positive plasmacytoid dendritic cells resided in sinuses in abundance. Conclusion. Demonstrated B-associated zone reduction and cytotoxic T-lymphocytes depletion with an up-regulation of PD-1 expression in the lymph nodes in patients with severe COVID-19 indicate immune response exhaustion. At the same time, observed significant reactive plasmacytosis with the presence of numerous T-helper cells constitutes a morphological substrate of the humoral immunity. These findings might indicate the ineffec-tiveness of the humoral response at late stages of COVID-19 infection in context of cytotoxic immunity failure. We studied parameters of clinical and biochemical blood tests in 54 patients with severe course of the disease caused by the novel coronavirus infection (COVID-19), who died in the intensive care unit. We established that COVID-19 patients had a hypercoagulation shift with intravascular coagulation, as evidenced by shortened activated partial thromboplastin time (APTT), elevated fibrinogen and D-dimer levels. We discovered that all patients with severe COVID-19 course had increased values of platelet anisocytosis, which indicated the presence of a hypercoagulation shift. At the same time, most patients had signs of consumption coagulopathy, as evidenced by decreased platelet and plateletcrit indices.
