Угольная промышленность по прежнему остается отраслью экономики с наиболее опасными для жизни и здоровья условиями труда (ежегодно на нее приходится более 70% травмированных на производстве). Кроме того, воздействие вредных условий труда в шахте ока зывает отрицательное влияние на организм. Экологиче ская среда угольных шахт обладает потенциально опас ными свойствами, которые проявляются при заболеваниях, производственных и непроизводствен Цель исследования -улучшение результатов лечения шахтеров на основе изучения особенностей течения у них критических состояний, обусловленных тяжелыми травматическими повреждениями и разработки дифференци рованной интенсивной терапии выявленных нарушений. Материал и методы. В рамках работы обследовано 1107 человек. Функциональное состояния центральной гемодинамики исследовано у 371 здоровых шахтеров с различ ным стажем подземных работ, контрольную группу составили 30 здоровых добровольцев, не работающих во вред ных условиях производства. Обследовано 386 пострадавших с ТЧМТ: 218 чел. -пострадавшие без подземного стажа работы, 115 чел. -пострадавшие шахтеры с подземным стажем работы в угольной шахте более 5 лет. В кон трольную группу исследования вошли 53 практически здоровых донора. Обследовано 122 пострадавших шахтера с тяжелой термотравмой. Контрольную группу составили 39 практически здоровых добровольцев. Исследования проведены у 106 ти пострадавших с ОРДС на фоне тяжелой сочетанной травмы в условиях искусственной венти ляции легких. Основную группу составили 63 травмированных шахтера, в группу сравнения вошли 43 человека, не имеющие подземного стажа работы. Заключение. С увеличение продолжительности работы во вредных производ ственных условиях в организме шахтеров возникают донозологические изменения со стороны органов и систем организма, которые носят закономерный фазный характер, укладываются в картину общего адаптационного син дрома и обусловливают особенности течения критических состояний. Особенностями течения критических состо яний, обусловленных тяжелыми травматическими повреждениями у шахтеров, являются раннее развитие и боль шая частота осложнений, более тяжелое течение и поздние сроки восстановления утраченных функций организма в отличие от пострадавших, не работающих во вредных условиях производства. Дифференцированная интенсив ная терапия критических состояний у шахтеров при тяжелых травматических повреждениях позволяет улучшить результаты лечения и уменьшить летальность. В п о м о щ ь п р а к т и к у ю щ е м у в р а ч у Острые нарушения мозгового кровообращения (ОНМК) являются одной из основных причин смерт ности населения во всех странах мира. «Омоложение» больных с инсультом, высокие частота, летальность и инвалидизация выживших обуславливают особую со циально экономическую значимость проблемы сосуди стых повреждений головного мозга [1-3]. Суть мозгового повреждения заключается в том, что вследствие различных анатомо морфологических Цель исследования-изучить состояние общей микроциркуляции и функции эндотелия при комах, обусловленных ост рым нарушением мозгового кровообращения. Материал и методы. Исследование проведено в первые 7 суток остро го периода ОНМК у 38 человек, находящихся в коме, которые были разделены на группы согласно этиологии инсуль та: 20 больных геморрагическим инсультом составили I группу, средний возраст у них составил 56,6±2,3 лет, 18 человек с ишемическим инсультом-II группу, средний возраст-58,5±2,2 лет. Контрольную группу составили 34 практически здоровых человека, средний возраст 55,1±2,1 лет. Состояние микроциркуляторного русла оценивалось на протяжении 7 суток методом накожной лазерной допплеровской флоуметрии, которую осуществляли с помощью лазерного анали затора капиллярного кровотока (ЛАКК 02) отечественного производства (НПО «ЛАЗМА», РФ). Одновременно с ре гистрацией состояния микроциркуляторного кровотока проводили забор образцов крови для исследования концентра ции в сыворотке крови эндотелина 1, стабильных метаболитов оксида азота, фактора Виллебранда и тромбомодулина. Результаты. На протяжении всего периода исследования у пациентов, находящихся в коме, обусловленной острым нарушением мозгового кровообращения, регистрируются нарушения микроциркуляции, заключающиеся в увеличе нии микроперфузии на фоне выраженного повышения влияний активных модуляторов сосудистого тонуса и определя ются функционально структурные изменения эндотелия. Заключение. Полученные в ходе исследования данные сви детельствуют об однотипности изменений состояния и регуляции общего микрокровотока у больных острыми нарушениями мозгового кровообращения вне зависимости от типа инсульта (геморрагический или ишемический).
