РезюмеВ обзоре изложены современные представления о фар-макодинамике основных групп антигипоксантов. На основании анализа данных литературы и собствен-ных экспериментальных исследований обсуждаются механизмы антигипоксической активности препара-тов прямого и непрямого энергизирующего действия. Рассматриваются вопросы клинической эффективно-сти и применения в медицинской практике лекарствен-ных препаратов с антигипоксической активностью. ВВЕДЕНИЕПриоритет в создании специфических фармако-логических средств для защиты от гипоксии принад-лежит отечественным ученым. Впервые это направ-ление стало разрабатываться в 60-е годы XX века на кафедре фармакологии Военно-медицинской академии им. С. М. Кирова в Ленинграде под руко-водством профессора В. М. Виноградова. В те же годы был введен в употребление термин «антиги-поксанты». Первыми соединениями, которые стали называть антигипоксантами, были аминотиоловые производные, такие как гутимин [13].В последующие годы соединения иной химической структуры (цитохром С, убихинон, гипоксен, мекси-дол и др.) существенно расширили класс антигипок-сантов, и на сегодняшний день это уже достаточно внушительный самостоятельный класс фармаколо-гических веществ. В настоящее время обсуждение вопросов фармакологии антигипоксантов перестало носить сугубо экспериментально-теоретический ха-рактер. Накопленная база экспериментальных дан-ных, успешные клинические исследования позволили многим соединениям найти своё клиническое приме-нение [33, 34, 42, 45, 46, 47, 48, 49].Взгляд на то, какие именно лекарственные сред-ства можно отнести к собственно антигипоксантам, с течением времени претерпел ряд изменений. По-скольку главными показателями гипоксии являются нарушения энергетического обмена, то и защита организма от гипоксии должна заключаться, прежде всего, в восстановлении энергетического баланса клетки. Поэтому в настоящее время антигипоксан-тами принято называть вещества метаболического типа действия, способные корригировать наруше-ния энергетического обмена и их последствия и по-вышать тем самым устойчивость клеток, органов и организма в целом к недостатку кислорода и дру-гим воздействиям, нарушающим энергопродукцию [7, 18, 36, 43].Сегодня антигипоксанты все чаще назначают в составе комбинированной фармакотерапии при различных заболеваниях, сопровождаемых со-стояниями гипоксии и ишемии. Если очертить круг основных показаний к применению антигипоксан-тов, то можно выделить такие значимые направле-ния их использования, как нейропротекция, кар-диопротекция, гепатопротекция, гастропротекция, ангиопротекция.По механизму действия и месту приложения дей-ствия в системе внутриклеточного метаболизма антигипоксанты можно подразделять на вещества прямого и непрямого энергизирующего действия.
ВВеденИеСогласно современным представлениям, сво-бодные радикалы и другие активные формы кисло-рода (АФК) играют значительную роль в регуляции основных функций клетки, как в обычных условиях, так и при воздействии на клетку различных патоген-ных факторов. При этом следует отметить, что АФК в зависимости от силы воздействующего на клетку патогенного фактора могут выступать либо индукто-рами процессов адаптации, либо индукторами апоп-тоза. Кроме того, АФК способны оказывать прямое деструктивное действие на клеточные структуры, а также инициировать свободнорадикальное окис-ление липидов, белков, нуклеиновых кислот, что лежит в основе патогенеза многих заболеваний [24, 29, 30]. Свои физиологические и патологиче-ские эффекты АФК реализуют в тесном взаимодей-ствии с другими регуляторными факторами клетки, модулируя их активность [15, 39].Фармакологическая регуляция активности про-цессов свободнорадикального окисления (СРО) занимает важное место в современных медико-биологических исследованиях, поскольку позволяет проводить эффективную патогенетическую фарма-котерапию заболеваний, в генезе которых деструк-тивную роль выполняют АФК, например, при забо-леваниях, связанных с гипоксией и реоксигенацией, ишемией и реперфузией, дегенеративных пораже-ниях нервной системы и других [7, 14, 20]. Знание ключевых вопросов генерации АФК, их регулятор-ной и деструктивной роли в процессах клеточного метаболизма, функционирования антиокислитель-ной системы клетки необходимы для практической реализации возможности фармакологической регу-ляции СРО путем таргетного воздействия на отдель-ные компоненты и этапы этого процесса, что пред-полагает разработку новых лекарственных средств направленного действия и оптимизацию антиради-кальной и антиоксидантной фармакотерапии.
Резюме:В эксперименте на модели карагенин-индуцированного воспаления лапы крыс наблюдали динамику развития острой воспалительной реакции. Регистрировали из-менения объема пораженной конечности (отек лапы) и показатели свободнорадикального окисления. Пока-зано, что ацетилсалициловая кислота в дозе 100 мг/кг уменьшает развитие воспалительной реакции. Гипок-сен и метапрот в дозе 50 мг/кг обладают слабой про-тивовоспалительной активностью, но потенцируют действие ацетилсалициловой кислоты. При совмест-ном энтеральном введении гипоксена и метапрота с ацетилсалициловой кислотой на фоне применения карагенина отмечен мощный противовоспалительный эффект, что проявлялось существенным снижением регистрируемых показателей воспаления и быстрым восстановлением объема пораженной конечности. Наиболее эффективна комбинация гипоксена с аце-тилсалициловой кислотой. ВВЕДЕНИЕОдним из наиболее распространенных типовых патологических процессов, который развивается в организме от различных этиологических факторов (физических, химических, инфекционных, эндоген-ных), является воспаление. Обязательные компо-ненты воспаления, как известно, -повреждение тканей и комплекс сосудисто-экссудативных и про-лиферативных реакций, направленных на удаление патогенного фактора и восстановление целостно-сти тканей [2]. Современная концепция воспаления рассматривает этот патофизиологический феномен с позиции решающего участия в нем окислительного стресса [9]. К настоящему времени накоплено мно-жество клинических и экспериментальных данных, доказывающих важную роль активированных мета-болитов кислорода и других радикалов в патогенезе воспаления, в возникновении и развитии всех его основных стадий [10].Средствами выбора при фармакотерапии вос-палительной реакции являются нестероидные про-тивовоспалительные препараты (НПВП). Однако несмотря на несомненную клиническую эффектив-ность, применение любых НПВП имеет свои огра-ничения. Это обусловлено тем, что даже кратковре-менный прием данных препаратов может приводить к развитию побочных эффектов, которые в целом встречаются примерно в 25 % случаев, а у 5 % боль-ных могут представлять серьезную угрозу для жизни [8, 11]. Многие исследователи среди перспективных путей повышения эффективности и безопасности НПВП рассматривают, в частности, их комбиниро-ванное использование с антигипоксантами/анти-оксидантами [1, 5]. В связи с этим большой научно-практический интерес представляют исследования влияния антигипоксантов на развитие воспалитель-ной реакции и эффективность НПВП при их комби-нированном применении.Цель исследования состояла в изучении эффек-тивности применения гипоксена и метапрота с кис-лотой ацетилсалициловой при остром карагенин-индуцированном воспалении. МАТЕРИАЛы И МЕТОДы ИССЛЕДОВАНИЯЭксперименты выполнены на 182 крысах ли-нии Wistar массой 190-210 г. Модель карагенин-индуцированного воспаления лапы крыс воспроизво-дили согласно «Руководству по экспериментальному (доклиническому) изучению новых фармакологиче-ских веществ» [6].
