Pacientes adultos do sexo masculino I : Presença do fator de risco C: Ausência do fator de risco O: Hérnia inguinal primária Dentre as hérnias, as inguinais são as mais freqüentes na prática clínica, acometendo preferencialmente o sexo masculino. Podem ser divididas em direta e indireta, sendo a primeira mais comum. Acredita-se que alguns fatores ambientais façam parte do desenvolvimento da hérnia direta, como o tabagismo, a idade avançada, desnutrição e a área de atividade ocupacional. Com objetivo de avaliar os fatores de risco para hérnia inguinal na população geral, e dando continuidade à edição anterior "À Beira do Leito, Medicina Baseada em Evidências", a qual avaliou estudos de caso-controle, realizamos uma revisão sistemática de estudos coorte, na base Medline, através da estratégia: "Hernia, Inguinal"[Mesh] AND ("Occupational Diseases"[Mesh] OR "Work"[Mesh] OR "Risk Factors"[Mesh]). Foram recuperados 275 artigos, analisados individualmente pelo título e resumo. Na análise dos resultados foram incluídos os estudos coorte de língua portuguesa, inglesa ou espanhola, relacionados à dúvida clínica (PICO), e que obtiveram escore na escala Newcastle-Ottawa 1 superior a seis. Foram selecionados dois estudos coorte: O seguimento de 14.407 homens, de 20 a 74 anos, por 11 anos permitiu analisar os fatores idade, raça, peso, IMC, história de hérnia hiatal e tabagismo. Os resultados demonstraram aumento do risco absoluto (ARA) de hérnia inguinal em pacientes com 40 a 59 anos de 3,2% (NNT 31) e em pacientes com história de hérnia hiatal de 7,7% (NNT 13). Mostraram-se fatores de proteção com redução do risco absoluto (RRA): pacientes da raça negra, 4,5% (NNT 22), peso maior ou igual a 82,3kg, 3,0% (NNT 33) e IMC maior ou igual a trinta, 4,1% (NNT 24). Nessa amostra, o tabagismo não se revelou como fator de risco para hérnia inguinal 2. Com relação às áreas de atividade ocupacional consideradas de risco para hérnia, foram investigados 51.246.000 pacientes. Em uma prevalência de seis casos de hérnia inguinal para cada 10.000 trabalhadores dessa população, calculamos o risco absoluto (RA) e a redução (RRA), ou aumento do risco (ARA), e o número necessário a tratar para um benefício (NNT) ou o número necessário a tratar para um dano (NNH), de cada uma dessas áreas de atividade, expressos para 10.
P: Pacientes adultos de ambos os sexos com pancreatite por litíase biliar I:Colecistectomia eletiva precoce C: Colecistectomia eletiva tardia O: Mortalidade Os pacientes com pancreatite por litíase biliar devem ser submetidos a alguma forma de tratamento definitivo e essa intervenção deve ser realizada assim que possível. Um atraso na intervenção aumenta as chances de cálculos adicionais migrarem, podendo ocasionar outro episódio de pancreatite. A intervenção, o mais breve possível, por outro lado, pode introduzir infecção no interior da área peripancreática inflamada e/ou agravar a pancreatite.Com objetivo de elucidar o tempo para realização de colecistectomia em pacientes portadores de pancreatite aguda por litíase biliar, realizamos uma revisão sistemática agregando os resultados de estudos do tipo coorte. A base Medline foi consultada através da estratégia [Mesh]: Time Factors AND Cholecystectomy AND Pancreatitis, recuperando 85 artigos, que foram analisados individualmente pelo título e resumo.Não foram incluídos os estudos em línguas diferentes do português, inglês ou espanhol. Foram selecionados oito estudos relacionados à dúvida clínica. Após a avaliação crítica do texto completo, um foi excluído por ser um estudo do tipo transversal 1 , outro estudo foi excluído porque não demonstrou os critérios de prognóstico, não apresentando o grau de pancreatite: leve, severa ou moderada 2 . Foram extraídos os resultados de seis estudos 3-8 com escore na escala NewcastleOttawa 9 de avaliação de qualidade maior que seis, cujos pacientes foram seguidos através do critério de Ranson 10 .Foram incluídos homens e mulheres adultos que apresentaram quadros clínicos de pancreatite aguda por litíase biliar. ResultadosA análise dos resultados quanto ao desfecho mortalidade em pacientes com pancreatite grave (RANSON ≥3) demonstrou aumento do risco absoluto (ARA) na cirurgia até 72h (Precoce) de 6,8% (IC95% 2,9% a 10,7%) e cirurgia de 72h a duas semanas (Tardia) de 17,8% (IC95% 9,0% a 26,6%) quando comparadas à cirurgia após duas semanas (Eletiva).Pacientes com pancreatite leve (RANSON <3) que foram submetidos à colecistectomia precoce, tardia ou eletiva não apresentaram mortalidade.Por meio desta revisão concluímos que em pacientes com pancreatite grave por litíase biliar a cirurgia eletiva é a que demonstra menor mortalidade (NNT 6 ou 15), mostrando-se o tempo mais seguro para a colecistectomia.Apesar disso, devemos levar em consideração que esses estudos não são ensaios randomizados, nos quais a distribuição de pacientes por grupo pode ter sido influenciada pela gravidade individual de cada paciente. Paciente de 60 anos, hipertenso, procurou o cardiologista devido a angina progressiva até classe funcional III, apesar de tratamento clínico otimizado com AAS, betabloqueadores, estatina e nitrato.
