Plasma rico em Plaquetas em lesÕes de joelhoAs lesões que acometem o joelho ocorrem com uma incidência relativamente alta, principalmente em atletas, e causa instabilidade e diminuição da função, além de serem muito dolorosas, principalmente na fase aguda. O tratamento cirúrgico é amplamente aceito como a escolha da terapia e é recomendada especialmente para jovens atletas que precisam de retorno imediato às suas atividades.Nos últimos anos o uso de fatores de crescimento tem sido utilizado para acelerar a cicatrização, com a ideia de um retorno mais rápido à atividade sem restrições¹. O plasma rico em plaquetas (PRP) é definido como um volume da fração do plasma de sangue autólogo com uma concentração de plaquetas acima do valor inicial. As plaquetas contêm um grande número de fatores de crescimento e citocinas que teriam um papel fundamental na regeneração óssea e maturação dos tecidos moles². A justificativa para utilização do PRP reside na inversão de relação de glóbulos vermelhos no sangue que são menos úteis no processo de cicatrização e aumento de plaquetas para a recuperação³. Visto que a utilização do PRP está aumentando para melhorar o tratamento de diferentes patologias ortopédicas, realizou-se revisão a fim de elucidar a eficácia do PRP em pacientes com lesões no joelho.A dúvida clínica foi: "Em pacientes com lesão no joelho o uso do PRP acelera a resposta clínica?Para responder a esta questão, realizou-se uma busca criteriosa da literatura na base de dados MEDLINE através da seguinte estratégia: (prp OR platelet rich plasma) AND (ligament OR Knee). Apenas estudos publicados em inglês, português e espanhol foram incluídos. Incluíram-se apenas ensaios clínicos randomizados e estudos coortes (força de evidência 1B ou 2B). As medidas consideradas na análise foram a redução ou aumento do risco absoluto com um intervalo de confiança de 95%.Após avaliação crítica foram incluídos apenas dois estudos relacionados à questão clínica. O primeiro estudo 4 analisou 40 pacientes com lesão do ligamento cruzado anterior diagnosticada pelo exame clínico e com imagem de ressonância magnética, sem cirurgia prévia no joelho ou antecedentes de lesões ligamentares. Os pacientes foram divididos em dois grupos: 10 pacientes foram submetidos ao tratamento convencional, e 30 foram submetidos ao mesmo tratamento e adicionado o uso do concentrado rico em plaquetas de três diferentes formas: 1ª) PRP no final da cirurgia, 2ª) PRP no final da cirurgia e intra-articular e 3ª) Combinado de PRP com trombina. Os resultados mediram o nível de celularidade e vascularização no túnel femoral por meio da intensidade do sinal da ressonância magnética. Não houve diferenças significativas entre os grupos com relação a ambos os desfechos.O segundo estudo incluído 5 utilizou testes clínicos e resultados de ressonância magnética em pacientes sem antecedentes de lesões.
