Question
What are the differences in care and outcomes of patients with ST-segment elevation myocardial infarction among 3 vertical levels of hospitals in China?
Findings
In this cross-sectional study using data from the China Acute Myocardial Infarction Registry, which included 108 hospitals at the province, prefecture, and county levels, compared with patients in province-level hospitals, the rates of reperfusion therapy were lower among those in prefecture-level and county-level hospitals (69.4% vs 54.3% vs 45.8%). In-hospital mortality rates progressively increased among the 3 levels of hospitals, from 3.1% at the province level to 5.3% at the prefecture level to 10.2% at the county level.
Meaning
These findings suggest that more efforts should be made to address the gaps in care and outcomes of ST-segment elevation myocardial infarction for national quality improvement in China.
Self-assembled nanostructures based on amphiphilic protein-polymer conjugates have shown great advantages in the field of nanomedicine such as inherent biocompatibility with biosystems because of their excellent performance. Herein, a novel biodegradable protein-polymer conjugate was prepared by covalently linking the tailor-made hydrophobic maleimide-functionalized poly(ε-caprolactone) (PCL) to hydrophilic bovine serum albumin (BSA) via the maleimide-sulfhydryl coupling reaction. This protein-based conjugate with a biodegradable polyester was reported for the first time, and the obtained biohybrid displayed well-defined structure, excellent biocompatibility and low cytotoxicity, and self-assembly behaviors similar to those of the traditional amphiphilic small molecules and block copolymers. The amphiphilic BSA-PCL conjugate can self-assemble into a nanosized vesicle with a negative surface charge. Furthermore, the self-assembled vesicle based on the BSA-PCL conjugate was functionalized via linking targeting ligand cetuximab to its surface to enhance cell uptake, and the doxorubicin (DOX)-encapsulated cetuximab-functionalized vesicle exhibited enhanced antitumor activity compared with that of free DOX in vitro. These results indicate that the biodegradable protein-polymer conjugate based on BSA and PCL had great potential as a drug delivery vehicle for cancer therapy.
ACE inhibition after MI inhibits cardiac hypertrophy, preserves cardiac function, and attenuates changes in myocardial gene expression. Gene expression profiling reveals, however, that some elements of the pathophysiology may be unaffected by the treatment and be targets for new therapies.

Pulmonary regurgitation and RVOT aneurysm/akinesia were independently associated with RV dilation and the latter with RV hypertrophy late after rTOF. The RVOT aneurysm/akinesia was common but related only in part to RVOT or transannular patching. Both RV hypertrophy and RVOT aneurysm/akinesia were associated with lower RVEF. Left ventricular systolic dysfunction correlated with RV dysfunction, suggesting an unfavorable ventricular-ventricular interaction. Measures to maintain or restore pulmonary valve function and avoid RVOT aneurysm/akinesia are mandatory for preserving biventricular function late after rTOF.
The incidence of nonsmoking female
patients with non-small cell
lung cancer (NSCLC) has increased in recent decades; however, the
pathogenesis of patients is unclear, and early diagnosis biomarkers
are in urgent need. In this study, 136 nonsmoking female subjects
(65 patients with NSCLC, 6 patients with benign lung tumors, and 65
healthy controls) were enrolled, and their metabolic profiling was
investigated by using pseudotargeted gas chromatography–mass
spectrometry. A total of 56 annotated metabolites were found and verified
to be significantly different in nonsmoking females with NSCLC compared
with the control. The metabolic profiling was featured by disturbed
energy metabolism, amino acid metabolism, oxidative stress, lipid
metabolism, and so on. Cysteine, serine, and 1-monooleoylglycerol
were defined as the biomarker panel for the diagnosis of NSCLC patients.
98.5 and 91.4% of subjects were correctly distinguished in the discovery
and validation sets, respectively. The biomarker panel was also useful
for the diagnosis of in situ malignancy patients, with an accuracy
of 97.7 and 97.8% in the discovery and validation sets, respectively.
The study provides a biomarker panel for the auxiliary diagnosis of
nonsmoking females with NSCLC.
