Introdução Dentre as vacinas contra a COVID-19 aprovadas pela ANVISA e incorporadas ao Programa Nacional de Imunização (PNI) destacam-se a vacina de vírus inativado CoronaVac (Sinovac/Butantan), e a ChAdOx1 (AstraZeneca/Fiocruz) em plataforma com vetor viral. Diferentes mecanismos de ação resultam em diferentes reações adversas ao imunizante. O objetivo deste trabalho é avaliar a frequência e gravidade dos eventos adversos relacionados às vacinas CoronaVac (VAC) e AstraZeneca (AZV) em uma coorte de trabalhadores da saúde. Métodos Estudo longitudinal observacional de 476 trabalhadores da saúde do Hospital Universitário da Universidade Federal do Espírito Santo (HUCAM-UFES/EBSERH), acompanhados desde o dia da vacina até 28 dias após a aplicação da segunda dose. Um diário padronizado de sinais e sintomas locais e sistêmicos contendo 17 perguntas foi aplicado após a aplicação da primeira e da segunda doses de AZV (215) e VAC (261) para avaliação de segurança dos esquemas vacinais através do mapeamento de eventos adversos até 28 dias após a aplicação de cada dose. Teste T e qui-quadrado foram utilizados nas análises estatística, sendo considerado p < 0,05. Resultados Dos 476 participantes recrutados (215 AZV + 261 VAC), 449 responderam o diário de sinais e sintomas da 1ª dose, e 397 da 2ª dose. Não houve reações graves e a duração dos sintomas foi semelhante entre os grupos. AZV provocou mais sintomas do que VAC em ambas as doses (1ª AZV 87% e VAC 61%, p < 0,001; 2ª dose AZV 57% e VAC 43%, p < 0,001). Os sintomas mais frequentes pela AZV nas duas aplicações foram dor local (1ª AZV 92% e VAC 73%, p < 0,001; 2ª AZV 77% e VAC 61%, p = 0,008), cansaço (1ª AZV 51% e VAC 26%, p < 0,001; 2ª AZV 38% e VAC 22%, p = 0,01) e febre (1ª AZV 23% e VAC 6%, p < 0,001; 2ª AZV 18% e VAC 7%, p = 0,02). Na primeira dose foram cefaleia (1ª AZV 62% e VAC 38%, p < 0,001; 2ª AZV 46% e VAC 41%, p = 0,4) e dores musculares (1ª AZV 46% e VAC 28%, p < 0,001; 2ª AZV 35% e VAC 24%, p = 0,09). Dores articulares (1ª AZV 27% e VAC 20%, p = 0,1; 2ª AZV 19% e VAC 21%, p = 0,7) e demais sintomas foram menos citados pelos participantes. Conclusão As primeiras doses de ambos os esquemas vacinais produzem mais efeitos adversos que as segundas doses. A vacina AstraZeneca provoca mais eventos adversos locais e sistêmicos em comparação com a CoronaVac em ambas as doses aplicadas. Consoante à segurança vacinal e risco de eventos adversos, as duas vacinas são seguras e nenhum evento adverso grave foi observado.
