In its current form the 2002 UICC TNM staging system is not applicable to papillary renal cell carcinoma. Clinical and radiological followup should be offered at frequent intervals to patients with venous thrombus and/or locally advanced disease. The role of age remains unclear but should not be underestimated in risk stratification after surgery.
EinleitungHauptursache für Gelenkerguss, Schwellung und Schmerzen im Rahmen chronisch entzündlicher Gelenkerkrankungen ist eine Synovialitis, die entweder durch Autoimmunprozesse z. B. bei der rheumatoiden Arthritis (RA), den reaktiven Arthritiden oder der Gruppe der Spondarthritiden oder durch mechanischen Stress mit Knorpel-und Knochenabrieb bei der so genannten aktivierten Arthrose verursacht wird. Das Spektrum der Behandlung chronisch entzündlicher Gelenkerkrankungen reicht in Abhängigkeit von der zugrunde liegenden Erkrankung von der systemischen medikamentösen Therapie über lokal konservative Maßnahmen bis hin zur chirurgischen Sanierung. Bei der RA beispielsweise lassen sich Gelenkbeschwerden bei vielen Patienten ausreichend gut im Rahmen der Basistherapie mit antientzünd-lichen oder immunmodulierenden Medikamenten behandeln. Häufig sprechen aber nicht alle Gelenke auf die medikamentöse Therapie gleich gut an und einzelne Gelenke bleiben schmerz-
ZusammenfassungIn der Behandlung chronisch entzündlicher Gelenkerkrankungen hat neben medikamentösen und arthroskopisch/chirurgischen Maßnahmen auch die nukleramedizinische Gelenktherapie, die Radiosynoviorthese (RSO), ihren festen Stellenwert. In Deutschland zur Therapie zugelassen sind die b-Strahler Yttrium-90, Rhenium-186 und Erbium-169 in Form kolloidaler Suspensionen zur intraartikulären Applikation. Die vorliegende Übersichtsarbeit soll sowohl die theoretischen Grundlagen der RSO, Indikationen, Kontraindikationen sowie potenzielle Nebenwirkungen und Komplikationen darstellen als auch praktische Anregungen zur Patientenvorbereitung, RSO-Durchführung und Nachsorge liefern. Überlegungen zum Strahlenschutz für Patient und Personal sollen das Bild abrunden.

Approved radiopharmaceuticals in Germany are the b-emitters yttrium-90, rhenium-186, and erbium-169 in colloidal suspensions for intra-articular administration. This review will give an overview of the basic physiology, indications, contraindications, potential side effects, and complications of radiation synovectomy as well as practical information on patient preparation, treatment procedure, and patient follow-up. Radiation safety considerations for both the patient and the personnel will complete this review.
