Tổng quan: Nội soi tăng cường hình ảnh (image inhanced endoscopy - IEE) đóng vai trò quan trọng trong phát hiện và đánh giá mức độ tổn thương tuy nhiên chỉ được tích hợp trong các dòng máy nội soi thế hệ mới, có giá thành cao. Tại Việt Nam, đã có những kết quả bước đầu trong ứng dụng trí tuệ nhân tạo trong nội soi. Mục tiêu: Nghiên cứu nhằm đánh giá tính khả thi của việc ứng dụng trí tuệ nhân tạo trong việc sinh ảnh giả lập các chế độ tăng cường đối với ảnh nội soi thực quản. Phương pháp: Tập ảnh đánh giá được xây dựng gồm 240 ảnh nội soi ở vị trí đường Z: 60 ảnh ở chế độ FICE, 60 ảnh giả lập FICE, 60 ảnh ở chế độ LCI, và 60 ảnh giả lập LCI. Ảnh giả lập FICE và ảnh giả lập LCI được sinh ra từ ảnh ánh sáng trằng bằng cách sử dụng mạng CycleGAN. Mỗi chế độ ánh sáng/giả lập sẽ có 30 ảnh đường Z bình thường và 30 ảnh có tổn thương viêm trào ngược. Tập ảnh đánh giá sẽ được trộn ngẫu nhiên sau đó được gửi cho 5 bác sỹ nội soi mới (< 5 năm kinh nghiệm) và 5 bác sĩ nội soi có kinh nghiệm (≥5 năm kinh nghiệm). Các bác sĩ được cho biết là trong bộ ảnh có ảnh giả lập được sinh từ thuật toán AI và được yêu cầu xác định ảnh nào là ảnh ánh sáng tăng cường thật sự, ảnh nào là ảnh giả lập AI. Kết quả: Nhóm bác sĩ mới đánh giá việc nhận định ảnh khó hơn so với nhóm bác sĩ có kinh nghiệm (52,1% ảnh được nhận định là khó so với 25,8% ở nhóm bác sĩ có kinh nghiệm). Tỉ lệ nhận định ảnh đúng của nhóm bác sĩ mới và nhóm bác sĩ có kinh nghiệm lần lượt là 54,67% và 52,67%. Mức độ nhận định đúng giữa nhóm bác sĩ mới và nhóm bác sĩ có kinh nghiệm là tương đương nhau. Kết luận: Kết quả bước đầu gợi ý khả năng ứng dụng phần mềm này trong thực tế nhằm nâng cao khả năng phát hiện tổn thương ở các đơn vị y tế còn hạn chế về nguồn lực.
Năm 2021, lần đầu tiên Bộ Y tế đưa gói chăm sóc bệnh nhân HIV tiến triển của WHO vào Hướng dẫn quốc gia về phòng, chống HIV/AIDS nhằm giảm hơn nửa gánh nặng tử vong do HIV. Chúng tôi tiến hành nghiên cứu định lượng kết hợp định tính để đánh giá khả năng áp dụng gói chăm sóc bệnh HIV tiến triển của WHO tại 41 cơ sở điều trị bệnh nhân HIV tại Việt Nam. Kết quả cho thấy chỉ có 3/41 (7,3%) cơ sở có thể thực hiện chẩn đoán HIV thông qua xét nghiệm CD4 hoặc xét nghiệm tải lượng virus. Khả năng chẩn đoán các nhiễm trùng cơ hội cũng còn nhiều hạn chế khi 34/41 (82,9%) và 33/41 (80,5%) cơ sở không thể chẩn đoán Cryptococcus và Talaromyces marneffei. Chỉ 1 cơ sở (2,4%) có thể xét nghiệm kháng nguyên CrAg huyết thanh và không có cơ sở nào thực hiện xét nghiệm CrAg dịch não tủy. Kết quả nghiên cứu định tính phản ánh một thực trạng thiếu nhân lực, thiếu trang thiết bị và sinh phẩm, thường xuyên gián đoạn cung ứng thuốc tại các đơn vị điều trị.
Thiếu máu là tình trạng phổ biến ở bệnh nhân HIV tiến triển, thường liên quan đến tình trạng nhiễm trùng cơ hội hoặc tác dụng phụ của thuốc ARV. Chúng tôi tiến hành nghiên cứu hồi cứu ở 1304 bệnh nhân HIV tiến triển tại 43 cơ sở điều trị ARV thuộc 15 tỉnh, thành phố tại Việt Nam. Tiêu chuẩn lựa chọn gồm những bệnh nhân HIV > 18 tuổi có bệnh HIV tiến triển. Tỉ lệ bệnh nhân có thiếu máu là 53,3% (695/1034). Tỉ lệ thiếu máu mức độ nhẹ, trung bình và nặng lần lượt là 26,2%, 22,4% và 4,7%. Tỉ lệ thiếu máu cao hơn ở bệnh nhân có CD4 thấp (53,4% ở bệnh nhân có CD4 < 100 tế bào/mm3 so với 42,5% ở bệnh nhân có CD4 100 - 200 tế bào/mm3). Bệnh lý nhiễm trùng cơ hội có tỉ lệ thiếu máu nhiều nhất là lao ngoài phổi (66,9%). Kết quả cho thấy thiếu máu là tình trạng phổ biến ở bệnh nhân HIV tiến triển. Việc đánh giá toàn diện và thực hiện gói chăm sóc bệnh HIV tiến triển là cần thiết để đánh giá nguyên nhân thiếu máu và giảm nguy cơ tử vong cho bệnh nhân.
Nghiên cứu nhằm đánh giá thuận lợi, khó khăn và tính khả thi về tiếp cận chẩn đoán và điều trị viêm gan C cho bệnh nhân đồng nhiễm viêm gan C/HIV tại tuyến huyện từ quan điểm của người sử dụng dịch vụ. Nghiên cứu định tính với 19 phỏng vấn sâu người sử dụng dịch vụ tại 5 tỉnh từ tháng 6 - 12/2022. Kết quả cho thấy các dịch vụ điều trị VGC và ARV lồng ghép tại cơ sở điều trị tuyến huyện tạo điều kiện cho người bệnh được tiếp cận cùng lúc nhiều dịch vụ, giảm chi phí đi lại, tiết kiệm thời gian. Cơ sở vật chất tại các cơ sở y tế điều trị viêm gan C được người bệnh đánh giá tốt, quy trình khám điều trị rõ ràng, thủ tục khám, xét nghiệm nhanh, thời gian chờ đợi không quá lâu. 100% bệnh nhân có cải thiện rõ rệt về sức khỏe sau khi hoàn thành phác đồ điều trị viêm gan C. Tuy nhiên, xét nghiệm tải lượng virut lại thực hiện tại các bệnh viện tuyến trên gây khó khăn nhiều cho người bệnh trong việc tiếp cận điều trị và hoàn thành phác đồ. Có hơn 50% người bệnh sẵn sàng cố gắng đồng chi trả tiền thuốc điều trị cùng với bảo hiểm y tế.
Tổ chức Y tế Thế giới khuyến cáo nếu xét nghiệm creatinin thường quy, cần ước tính mức lọc cầu thận trước khi điều trị phác đồ có tenofovir. Chúng tôi tiến hành nghiên cứu hồi cứu những bệnh nhân bệnh nhân > 18 tuổi được chẩn đoán HIV tiến triển tại các cơ sở điều trị ARV tại 15 tỉnh, thành phố tại Việt Nam từ tháng 1/2021 đến 5/2022. Nghiên cứu này được thực hiện tại 15 tỉnh, thành phố tại Việt Nam. Chức năng thận được đánh giá theo ước tính mức lọc cầu thận (eGFR) sử dụng công thức MDRD (Modification of Diet in Renal Disease). Tỉ lệ bệnh nhân có eGFR < 60 ml/min/1,73m2 là 6,0% (108/1787). Các yếu tố liên quan đến mức lọc cầu thận thấp (eGFR < 60 ml/min/1,73m2) là giới tính nữ (OR = 4,07, 95%CI: 2,71 - 6,08), hemoglobin thấp (OR = 1,86, 95%CI: 1,01 - 3,22) và tuổi (OR 1,07, 95%CI: 1,05 - 1,09). Trên những bệnh nhân xác định được tình trạng sống còn, tỉ lệ tử vong ở bệnh nhân có và không giảm mức lọc cầu thận là 11,5% (7/61) và 7,5% (99/1326). Tỉ lệ giảm chức năng thận thường gặp ở bệnh nhân HIV nên cần thiết phải đánh giá chức năng thận trước khi bắt đầu điều trị ARV và theo dõi định kỳ trong quá trình điều trị.
