Bất thường Ebstein là một bệnh lý hiếm gặp, chiếm 1% các trường hợp tim bẩm sinh, đặc trưng bởi sự bám thấp của lá vách và lá sau của va ba lá (VBL) vào thất phải (TP) dẫn đến hình thành một vùng nhĩ hóa của TP [1]. Bất thường này thường đi kèm với thông liên nhĩ và hở VBL, hiếm gặp hơn là hẹp VBL. Biểu hiện lâm sàng có thể ngay sau sinh với triệu chứng tím nặng và suy TP. Những trường hợp này cần can thiệp phẫu thuật rất sớm [2]. Đa số các trường hợp biểu hiện ở tuổi thiếu niên với biểu hiện tím vừa và suy TP ở thời điểm lúc can thiệp phẫu thuật [3]. Hiếm gặp hơn là những bệnh nhân Ebstein lớn tuổi. Ở tuổi này thường có các biểu hiện gồm tím, loạn nhịp, suy TP nặng và kèm theo suy thất trái thứ phát do tình trạng tím và suy TP kéo dài. Với các trường hợp này sửa chữa 2 thất thường không có kết quả và tỷ lệ tử vong cao theo nghiên cứu của Carpentier [1]. Chúng tôi báo cáo một trường hợp Ebstein lớn tuổi, thiểu sản TP nặng được sửa chữa theo phương pháp 1-1/2 thất nhằm đưa ra một lựa chọn điều trị trong các phương pháp phẫu thuật cho bệnh nhân Ebstein.
Nghiên cứu cắt ngang mô tả trên 424 học sinh nhằm đánh giá thực trạng bệnh răng miệng của học sinh tại 2 trường Trung học cơ sở Kỳ Bá và Phú Xuân, thành phố Thái Bình. Kết quả nghiên cứu chỉ ra rằng: Tỷ lệ mắc bệnh răng miệng (BRM) chung của hai trường là 53,8% (Phú Xuân :70,9%; Kỳ Bá 41,8%, p<0,05): Sâu răng (33,5%), sâu mất trám – răng (SMT-R) (38,0%), viêm lợi (11,1%); cao răng (11,6%); mảng bám răng (22,6%); mắc BRM ở nữ cao hơn nam (nữ: 56,1%; nam: 43,9%; p<0,05). Bệnh sâu răng: Tỷ lệ SR trường Phú Xuân cao hơn trường Kỳ Bá (44,6%; 25,7% và p<0,05). Học sinh nữ có nguy cơ bị SR cao hơn so với học sinh nam (OR = 2,35; 95%CI: 1,55-3,56). Chỉ số OHI-S tốt đạt 72,6%. Chỉ số bị bệnh nha chu: GI, CI-S và DI-S của trường Phú Xuân đều có nguy cơ cao hơn so với các chỉ số nha chu trên của trường Kỳ Bá lần lượt là (OR = 10,24; 95%CI: 4,47-23,5), (OR = 4,19; 95%CI: 2,18-8,07) và (OR = 4,42; 95%CI: 2,71-7,21).
Mục tiêu: Đánh giá khả năng áp dụng, độ chính xác và an toàn của phẫu thuật cắt chóp với máng hướng dẫn và mũi Trephine ở các vùng răng khác nhau tại Viện đào tạo Răng Hàm Mặt, Đại học Y Hà Nội. Đối tượng, phương pháp: Nghiên cứu can thiệp lâm sàng không đối chứng, chọn mẫu thuận tiện nhóm 30 bệnh nhân với 44 chóp răng có tổn thương quanh chóp mạn tính, đủ tiêu chẩn lựa chọn. Các bệnh nhân được phẫu thuật cắt chóp với máng hướng dẫn phẫu thuật và mũi Trephine. Đánh giá khả năng áp dụng trên các vùng răng khác nhau, các tai biến xảy ra, và sai số so với kế hoạch sau phẫu thuật. Kết quả: 44 chóp răng đã được cắt. Phương pháp có thể thực hiện ở tất cả các nhóm răng. 100% các ca không ghi nhận tai biến gây tổn thương các cấu trúc giải phẫu lân cận. Sai lệch độ dài phần chóp cắt là 0.44mm (95% khoảng tin cậy là: 0,3 -0,54). Sai lệch góc diện cắt qua chóp là 4,860 (95% khoảng tin cậy: 4 - 5,62). Sự khác biệt về độ dài đoạn chóp cắt và góc diện cắt qua chóp của các răng ở các nhóm răng khác nhau không có ý nghĩa thống kê. Kết luận: Kết quả nghiên cứu cho thấy máng hướng dẫn cắt chóp có độ chính xác cao và an toàn, áp dụng được với cả các răng hàm lớn. Phương pháp cần được tiến hành nghiên cứu nhiều hơn về kết quả lành thương sau phẫu thuật.
Nghiên cứu cắt ngang mô tả trên 424 học sinh nhằm đánh giá kiến thức, thực hành về chăm sóc răng miệng và một số yếu tố liên quan đến bệnh sâu răng của học sinh tại 2 trường Trung học cơ sở Kỳ Bá và Phú Xuân, thành phố Thái Bình. Kết quả nghiên cứu chỉ ra rằng: Học sinh nữ (64,8%) có nguy cơ sâu răng cao hơn học sinh nam (35,2%) với p <0,05 (OR=2,35; 95%CI: 1,55-3,56). Có sự khác biệt về tình trạng sâu răng ở vùng thành thị và nông thôn: học sinh tại trường Phú Xuân có tỷ lệ sâu răng cao hơn với học sinh trường Kỳ Bá (46,6%; 25,7%; p<0,05). Sử dụng tăm xỉa răng có nguy cơ sâu răng cao hơn (OR=1,71, 95%CI: 1,12-3,24). Học sinh không chải răng hàng ngày nguy cơ sâu răng cao hơn so với học sinh chải răng hàng ngày (OR=9,23, 95%CI: 2,19-39,2). Súc miệng sau khi ăn sẽ làm giảm nguy cơ mắc bệnh SR (OR=4,18, 95%CI: 2,14-8,15). Học sinh thường xuyên ăn đồ ngọt thì nguy cơ mắc SR tăng hơn với nhóm học sinh không thường xuyên ăn đồ ngọt (OR=2,37, 95%CI: 1,55-3,62). Nguy cơ sâu răng sẽ tăng nếu học sinh không thường xuyên đi khám răng định kỳ (OR=2,02, 95%CI: 1,27-3,23).
Objectives: To evaluate the outcome of selective reconstructive anterior cruciate ligament (ACL) surgery using the All-inside technique with a four-strand semitendinosus or gracilis autograft tendon. Subject and methods: 34 patients were diagnosed with a partial tear of the ACL and underwent selective reconstructive surgery at the Orthopedic Center, Military Hospital 103 from November 2018 to September 2020. Lysholm knee scale and IKDC class were used to access post-operatively clinical outcomes. Stability was evaluated using the anterior drawer test, the Lachman test, the Pivot shift test, and the Rolimeter measure at 20o of knee flexion. Results: The pre-operative and post-operative Lysholm scores were 49.5 ± 18.4 and 94.47 ± 6.81, respectively (p < 0.001). Pre-operatively, most patients were classified as C and on the IKDC score. At the final follow-up, IKDC scores included 22 class A, 10 class B, 2 class C (p < 0.001). The pre- and post-operative laxity measured by Rolimeter were 5.15 ± 1.5 mm and 1.74 ± 1.34 mm, respectively (p < 0.001). Pre-operatively and post-operatively, the Pivot-shift was absent in all cases. Conclusion: Selective reconstruction of the injured ACL bundle restores knee stability and function. All-inside technique can be used to reconstruct the rupture ACL bundle with many advantages. Longer follow-up is needed to fully evaluate clinical outcomes after selective reconstruction in partial ACL tears. * Keywords: All-inside; Partial tear; Selective reconstruction.
