Bất thường Ebstein là một bệnh lý hiếm gặp, chiếm 1% các trường hợp tim bẩm sinh, đặc trưng bởi sự bám thấp của lá vách và lá sau của va ba lá (VBL) vào thất phải (TP) dẫn đến hình thành một vùng nhĩ hóa của TP [1]. Bất thường này thường đi kèm với thông liên nhĩ và hở VBL, hiếm gặp hơn là hẹp VBL. Biểu hiện lâm sàng có thể ngay sau sinh với triệu chứng tím nặng và suy TP. Những trường hợp này cần can thiệp phẫu thuật rất sớm [2]. Đa số các trường hợp biểu hiện ở tuổi thiếu niên với biểu hiện tím vừa và suy TP ở thời điểm lúc can thiệp phẫu thuật [3]. Hiếm gặp hơn là những bệnh nhân Ebstein lớn tuổi. Ở tuổi này thường có các biểu hiện gồm tím, loạn nhịp, suy TP nặng và kèm theo suy thất trái thứ phát do tình trạng tím và suy TP kéo dài. Với các trường hợp này sửa chữa 2 thất thường không có kết quả và tỷ lệ tử vong cao theo nghiên cứu của Carpentier [1]. Chúng tôi báo cáo một trường hợp Ebstein lớn tuổi, thiểu sản TP nặng được sửa chữa theo phương pháp 1-1/2 thất nhằm đưa ra một lựa chọn điều trị trong các phương pháp phẫu thuật cho bệnh nhân Ebstein.
Chẩn đoán phân biệt của khối bất thường trong tim bao gồm: huyết khối, u tim. Kích thước, hình dạng, vị trí, tính di động, mật độ kết hợp với các triệu chứng lâm sàng để chẩn đoán phân biệt các nguyên nhân. Siêu âm tim qua thành ngực và đặc biệt là siêu âm qua thực quản trở thành tiêu chuẩn để chẩn đoán các khối bất thường trong tim. Chụp cắt lớp vi tính, cộng hưởng từ tim cũng là một phương tiện để chẩn đoán với độ chính xác cao [1]. Chẩn đoán chính xác nguyên nhân là rất quan trọng trong lựa chọn giải pháp điều trị thích hợp và tiên lượng bệnh nhân. Chúng tôi báo cáo một trường hợp có khối bất thường nhĩ phải nhằm thảo luận các chẩn đoán có thể xảy ra trên bệnh nhân và nhìn lại y văn các nguyên nhân gây ra khối bất thường trong nhĩ phải.
