Background: COVID–19 is still a challenge, both with regard to its treatment and to the actual efficacy of the vaccines available to date, especially with the emergence of new variants. We evaluated the efficacy of the measles–mumps–rubella (MMR) vaccine in preventing SARS–CoV–2 infection and severity of COVID–19 in health workers. Methods: This analysis includes data from one ongoing blinded, randomized, placebo–controlled trial with participants aged 18–60 years were randomly assigned to receive the MMR vaccine or a placebo. The primary efficacy analysis included all participants with a positive nasopharyngeal RT–PCR test since their inclusion. Results: The MMR vaccine did not prevent the SARS–CoV–2 infection. Participants in the MMR group, compared with those in the placebo group, had a 48% risk reduction in symptomatic COVID–19 (RR = 0.52; 95% CI: 0.33-0.83; p=0.004) and a 76% risk reduction in COVID–19 treatment (RR = 0.24; 95% CI: 0.06-0.88; p = 0.020) with one dose and a 51% risk reduction in COVID–19 symptoms (RR = 0.49; 95% CI: 0.31 – 0.78; p = 0.001) and a 78% risk reduction in COVID–19 treatment (RR = 0.22; 95% CI: 0.06-0.82; p = 0.015) with two doses. Conclusions: This interim analysis of an ongoing clinical trial suggests that compared with a placebo, the vaccine reduces the risk of COVID–19 symptoms and reduces the need for COVID–19 treatment.
Objective The present study aimed to evaluate the antenatal care adequacy for women who gave birth at the University Hospital of Santa Catarina in Florianopolis (Brazil) during the COVID-19 pandemic, and to evaluate the association of adequacy with sociodemographic, clinical, and access characteristics. Methods Data were collected between October and December 2020, including 254 patients who delivered in the University Hospital from Federal University of Santa Catarina and answered our questionnaires. Additional data were obtained from patients' antenatal booklets. Antenatal care was classified as adequate, intermediate, or inadequate according to the number of appointments, gestational age at the beginning of follow-up, and tests results. We carried out a descriptive statistical analysis and a bivariate/with odds ratio analysis on maternal sociodemographic, clinical and health access variables that were compared with antenatal adequacy. Results Antenatal care was considered adequate in 35.8% of cases, intermediate in 46.8%, and inadequate in 17.4%. The following maternal variables were associated with inadequate prenatal care (intermediate or inadequate prenatal care): having black or brown skin colour, having two or more children, being of foreign nationality, not being fluent in Portuguese, and using illicit drugs during pregnancy; the clinical variables were more than 6 weeks between appointments, and not attending high-risk antenatal care; as for access, the variables were difficulties in attending or scheduling appointments, and attending virtual appointments only. Conclusion In a sample of pregnant women from a teaching hospital in Florianópolis during the COVID-19 pandemic, antenatal care was considered adequate in 35.8%, intermediate in 46.8%, and inadequate in 17.4% of cases.
Introdução: Durante a pandemia de COVID-19, as gestantes e puérperas são consideradas grupo de risco para uma evolução mais grave. No entanto, os dados de prevalência da doença são mais difíceis de obter. Como a testagem sistemática de toda a população ou apenas da população obstétrica não foi realizada durante o ano de 2020, é difícil estabelecer a incidência da doença nessa população. É importante conhecer a incidência da doença na população obstétrica. Objetivo: O objetivo deste estudo é verificar o número de mulheres em uma amostra que relatam ter tido diagnóstico confirmado ou suspeita de diagnóstico de COVID-19 durante a gestação e puerpério imediato em uma cidade no Sul do Brasil. Métodos: Este estudo apresenta resultados parciais de uma pesquisa denominada de “Complicações obstétricas e puerperais durante a epidemia de COVID-19”, realizada em um hospital universitário do Sul do Brasil. O hospital em que foi feita a coleta de dados não é referência para casos de COVID-19 na gestação. Foram selecionadas para a coleta de dados mulheres que tiveram seus partos ou cesarianas após o início da epidemia de COVID-19. Os sujeitos do estudo foram as puérperas cujos bebês nasceram no hospital estudado. Os dados foram coletados em dois momentos: logo antes da alta hospitalar após o nascimento e entre um a dois meses de puerpério. Em ambos os momentos, os dados foram avaliados por questionários autopreenchidos pela mulher. Na alta hospitalar o questionário foi preenchido manualmente, e o questionário do puerpério mais tardio foi preenchido em plataforma eletrônica. Todas as mulheres participantes assinaram um Termo de Consentimento Livre e Esclarecido antes do início da pesquisa. Resultados e conclusão: Participaram da pesquisa 257 mulheres. Todas preencheram o questionário na alta após o nascimento do bebê. A amostra considerada nesta pesquisa foi de 206 mulheres, que correspondem às que responderam à variável sobre ter sido diagnosticada ou considerada caso suspeito para COVID-19 na gestação. Foram consideradas casos suspeitos 31 mulheres, ou 15,1% da amostra. Delas, 12 foram tomadas como casos suspeitos por contato, 19 tiveram sintomas gripais, e só nove tiveram diagnóstico laboratorial. As mesmas mulheres foram convidadas a responder a uma entrevista após um a dois meses da alta, e 103 responderam ao questionário eletrônico encaminhado. Apenas duas não responderam à pergunta sobre ter COVID-19 entre a alta e o questionário. Relataram o diagnóstico de COVID-19 11 mulheres (sete com diagnóstico confirmado e quatro consideradas casos suspeitos). Uma relatou que o recém-nascido também teve COVID-19. No puerpério, a porcentagem de mulheres com diagnóstico ou suspeita foi de 10,9%. A incidência de diagnóstico ou suspeita de COVID-19 nesta amostra foi de 0,2% ao se considerarem apenas as que responderam à pergunta de interesse e de 0,16% ao se considerarem as respostas em branco como não tendo tido o diagnóstico.
Introdução: O úraco é um remanescente embrionário obliterado, que se estende do ápice vesical à cicatriz umbilical. Cerca de 30% dos adultos possuem úraco com lúmen e, quando alguma extremidade dessa estrutura está obliterada, há acúmulo de fluido capaz de formar cistos, cujo quadro clínico é inespecífico. Neoplasias podem simular esses cistos, motivo pelo qual eles devem ser ressecados. Relato de caso: Puérpera, 21 anos, G4P4, hígida, evoluiu no 3º dia de puerpério com disúria. Procurou a unidade de saúde no 11º dia, onde recebeu cefalexina para tratamento de infecção do trato urinário (ITU), sem melhora. Evoluiu com dor abdominal em hipogástrio, febre e persistência de disúria. Na admissão da emergência obstétrica, apresentou útero involuído, lóquios fisiológicos, ausência de peritonite, sinal de Giordano ausente. Ultrassonografia pélvica mostrou parede vesical posterior espessa e irregular, massa heterogênea acometendo parede anterior e superior da bexiga, com conteúdo líquido complexo, medindo 8 x 7 cm, que sugeria hematoma ou abscesso. Hemograma evidenciou leucocitose sem desvio e anemia. Foram iniciados clindamicina e gentamicina e indicou-se laparotomia exploradora. Na cirurgia, foi identificado abscesso em recesso vesicouterino. Durante ressecção da cápsula, concluiu-se que se tratava de cisto infectado de úraco. Paciente evoluiu com melhora e recebeu alta hospitalar. Laudo anatomopatológico apresentou cisto com parede de tecido conjuntivo, intenso infiltrado inflamatório e ausência de neoplasia. Conclusão: O cisto de úraco geralmente é descoberto ao acaso em exames de imagem de abdômen e pelve por infecções ou tumores, assim como no caso da paciente relatada. A ultrassonografia é citada como importante método diagnóstico em razão do fácil acesso, da ausência de radiação e de grande parte do úraco ser próximo à parede abdominal, facilitando sua visualização. Tomografia computadorizada e ressonância nuclear magnética podem ser úteis. A sintomatologia é inespecífica, com dor periumbilical e/ou hipogástrica, febre, náuseas, ITU e leucocitose. A paciente do caso em questão cursou com dor em hipogástrio e disúria com exame físico abdominal inespecífico, sendo inicialmente tratada para ITU. Oitenta por cento dos cistos são infectados por Staphylococcus aureus e podem cursar com eritema periumbilical e fístulas. O adenocarcinoma de úraco, em 85% das vezes, é do tipo mucinoso e pode facilmente ser confundido com um cisto. Atualmente, recomenda-se a ressecção cirúrgica eletiva de cistos de úraco pelo risco de neoplasia e de infecção.
