A comunicação buco-sinusal é uma ocorrência comum, caracterizada pela união do seio maxilar com a cavidade bucal após extrações dentárias superiores posteriores cujas raízes possuem íntima relação com o seio maxilar. Ao epitelizar-se, este trajeto denomina-se fístula buco-sinusal (FBS). Este trabalho relata o fechamento de uma fístula buco-sinusal com enxerto pediculado do corpo adiposo da bochecha (EPCAB). Paciente saudável, feminino, 40 anos, relatou dor no terço médio da face no lado esquerdo após exodontia traumática a um ano. Clinicamente havia alteração mucosa na região do elemento 26 extraído. A TCFC evidenciou uma fístula buco-sinusal de aproximadamente 8 milímetros. Foi proposto o fechamento com EPCAB. Após bloqueio dos nervos alveolares superior, posterior e médio e palatino maior, foi feito um retalho com incisão crestal e duas relaxantes na região entre os elementos 25 e 27. Desepitelizou-se o trajeto da fístula. Após localização do corpo adiposo da bochecha e feita a divulsão, a massa gordurosa foi suturada à margem gengival palatina com fio reabsorvível de seda 4.0. A paciente foi medicada e orientada sobre os cuidados pós-operatórios e, após seis meses, evoluiu sem queixas clínicas. Portanto, o uso do EPCAB para o fechamento da FBS é uma técnica benéfica.
