Our data suggest that PEG should be the procedure of choice when considering gastrostomy insertion and RIG should be reserved as a second-line approach for cases in which PEG is technically difficult or contraindicated.
Hypoglycaemia is an acute complication associated with the use of insulin or sulphonlyureas to treat diabetes. This article will focus on the risks of hypoglycaemia in older people including the signs and symptoms. It will give an overview of diabetes and discuss the specific problems associated with aging in managing diabetes. Poorly-controlled diabetes is associated with long-term complications. Monitoring glycaemic control will be discussed including the research promoting optimal control. A case history will be included to demonstrate the risks of hypoglycaemia in older people. Discussion will include developing an individualised management plan with the patient to promote safe practice for the individual and balancing risks
This article focuses on a case study of an individual who experiences pain that cannot fully be accounted for by physical illness. We explore the different theories within mental health that may explain the physical pain felt. Factitious and dissociative disorders are touched upon yet the case predominantly describes a somatoform pain disorder, which is explored in more detail.
