This paper describes the collaborative partnership between two geographically diverse NHS trusts in the UK serving patient populations with very different demographic profiles. A programme of structured exchange has been developed allowing diabetes specialist nurses (DSNs) to visit the partner trust to share and compare clinical practice. The model allows the individuals involved to critically reflect on their own and others' practice and has facilitated networking between the DSNs and service developments within the trusts.Eur Diabetes Nursing 2006; 3(2): 98-101.
Hypoglycaemia is an acute complication associated with the use of insulin or sulphonlyureas to treat diabetes. This article will focus on the risks of hypoglycaemia in older people including the signs and symptoms. It will give an overview of diabetes and discuss the specific problems associated with aging in managing diabetes. Poorly-controlled diabetes is associated with long-term complications. Monitoring glycaemic control will be discussed including the research promoting optimal control. A case history will be included to demonstrate the risks of hypoglycaemia in older people. Discussion will include developing an individualised management plan with the patient to promote safe practice for the individual and balancing risks
