This is the first prospective study to elucidate autonomic dysfunction in bvFTD patients. There is a considerable higher frequency of cardiovascular dysfunction and OH in bvFTD. History taking may be not conclusive thus cannot exclude cardiovascular dysautonomia.
Ziel und Methodik: Seit 1998 werden substituierte Opiatabhängige vor dem Hintergrund geänderter betäubungsmittelrechtlicher Bestimmungen von Codein auf Methadon umgestellt. Die Fragestellung dieser Untersuchung lautet: Gelingt dieser Wechsel des Substitutionsmittels? Dazu wurden bei 165 Patienten die Umstellungsverläufe retrospektiv untersucht. Es wurden die Patienten selber als auch die behandelnden Ärzte befragt. </P><P>
Ergebnisse: Bei den meisten Patienten ist die Umstellung erfolgreich verlaufen: 73% erhalten nach abgeschlossener Umstellung Methadon die allermeisten dieser umgestellten Personen halten ihre aktuelle Situation für besser als unter der Codeinsubstitution. Bei 19% der Untersuchungsteilnehmer musste die Umstellung auf Methadon rückgängig gemacht werden, d. h. sie werden wieder mit Codeinpräparaten substituiert. Der Unterschied zwischen den erfolgreich umgestellten Patienten und den »Abbrechern« besteht darin, dass die zweite Gruppe schon wesentlich länger mit Codein substituiert worden ist. In anderen Merkmalen unterscheiden sich dagegen die beiden Patienten-Gruppen kaum oder gar
nicht. </P><P>
Schlussfolgerungen: Die Ergebnisse dieser Untersuchung sprechen für die Weiterentwicklung einer differenzierten Indikation bei der Substitutionstherapie und gegen zu starre Regelungen bei der
Anwendung der verschiedenen Substitutionsmittel.
Negotiations are a valuable and important tool for increasing the voice of workers in the informal economy. This paper provides empirical evidence from Africa, Asia and the Americas on negotiations and collective bargaining by workers in the informal economy. These practices demonstrate that negotiations involving workers in the informal economy, including those that culminate in collective agreements, are key to ensuring the fair distribution of the fruits of economic progress and labour protection to workers in many countries. They are also an important tool for reducing informalization trends. The paper also shows the multiple challenges workers in the informal economy are facing when organizing and entering into negotiations and outlines some possible pathways on how these can be reduced.
One of the current paradoxes for trade unions is that organizing is an essentially local or national affair whilst the most pressing challenge for unions, which is globalization, can only be faced in a global context. This paper analyzes to what extent the Global Union Research Network (GURN) has the potential to be regarded as an incremental innovation for research within the international labour movement. The paper argues that the GURN can become an incremental innovation and there are three stages to this argument. Firstly the GURN in conceptualized within the international trade union movement. Secondly the term 'innovation' is defined and the GURN is presented as a potential, albeit incremental, innovation. The final stage examines GURN sustainability and the barriers to its institutionalization.
