Human milk is related to the physiological and nutritional welfare of newborns, providing the necessary dietary energy, physiologically active compounds and essential nutrients for breastfed babies. Human milk fat has an important position as energy source, structural and regulatory functions, being one of the most important components of breast milk. It provides approximately 50-60% of the energy of the human milk, and its composition in fatty acids defines its nutritional and physico-chemical properties. Furthermore, human milk contains the longchain polyunsaturated essential fatty acids (LCPUFA) eicosapentaenoic acid (EPA), arachidonic acid (AA) and docosahexaenoic acid (DHA), which is important for appropriate development of baby's organs, tissues and nervous system. This chapter will address the benefits associated with the consumption of human milk (health, nutritional, immunological and developmental benefits) as well as the analysis applied to determine the lipid quality of this powerful food.
Oral/enteral nutrition supplements enriched with fish oil are widely used for the recovery of the nutritional status of hospitalized patients, due to the omega‐3 fatty acids content. However, they are targets of adulteration. This study evaluates the lipid quality of five brands of oral/enteral nutrition supplements incorporated with fish oil, available on the Brazilian market (Brazil, Holland, and Germany). The fatty acids are quantified by gas chromatography–flame ionization detection, and the lipid profile is acquired by direct electrospray ionization–mass spectrometry, to assess how fatty acids are present in those samples. The eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) contents found ranged from 1.54 to 19.36% and 4.34 to 27.29%, respectively, in the form of triacylglycerols. However, brand 1 presents the lowest levels of EPA + DHA in its lipid composition, and in three brands, the addition of medium chain triglycerides is identified. The lipid quality assessment of these supplements allows an informed choice of appropriate supplements for the nutritional support of patients. Practical Applications: The study highlights and provides information of the lipid composition and fatty acids of oral/enteral nutrition supplements enriched with fish oil, indicated for malnourished patients. The findings allow health professionals to make a more informed choice of oral/enteral nutrition for the adequate treatment of patients. Objetivo: Discutir as recomendações de doação de leite materno aos Bancos de Leite Humano (BLH) frente à pandemia COVID-19. Metodologia: A pesquisa foi realizada envolvendo publicações sobre BLH, leite humano doado (LHD) e COVID-19 indexados nas bases de dados eletrônicas PubMed (US National Library of Medicine), Scientific Electronic Library Online Brasil (SciELO), Web of Science e Portal Capes. Os descritores que delimitaram a pesquisa foram: amamentação, aleitamento materno, BLH, LHD, leite humano, COVID-19, pandemia e Sars-CoV-2 em português e inglês. Resultados e discussão: Como o vírus da COVID-19 é transmitido por gotículas respiratórias e o vírus sobrevive por períodos variados em superfícies inanimadas, neste trabalho observamos as principais medidas de controle de transmissão do vírus pelo LHD durante todo o processo de coleta, processamento e distribuição do leite humano até os recém-nascidos das Unidades de Terapia Intensiva Neonatal (UTIN). É necessário controle de qualidade com alto rigor higiênico-sanitário do LHD como esterilização dos materiais que entrarem em contato com o leite materno, higienização do ambiente onde será realizada a ordenha, utilização de gorro e máscara, lavagem das mãos com água e sabão e eliminação dos primeiros jatos de leite ordenhado. Conclusão: As doadoras de leite humano ordenhado devem realizar uma triagem nos BLH para avaliação e orientações antes de iniciar a doação de leite humano, a fim de evitar a transmissão do vírus da COVID-19 durante todo o processo de coleta, processamento e distribuição do leite humano até os recém-nascidos das UTIN.
