Human milk (HM) is extremely important to the baby, containing a varied and balanced range of nutrients fundamental to the ideal development of the newborn. Among these nutrients, HM lipids provide 40-60% of the energy. In HM banks, the creamatocrit method is employed as a quick and solvent-free tool, however, since Folch methodology is considered reference for lipid extraction, this study compared the fatty acid (FA) composition and the lipid profile by gas chromatography with flame ionization detector (GC-FID) and direct infusion by electrospray ionization mass spectrometry (DI-ESI-MS) in pools of different HM phases, obtained from Folch and creamatocrit methods, in order to elucidate which method provides better information about HM lipids. Lipid compositon results by Folch were superior to the results obtained by creamatocrit in HM pools investigated. It means, the lipid profiles of all HM pools were more intense, and the number of FA identified by Folch was also higher in comparison to the creamatocrit method (21 and 6 FAs, respectively). GC-FID analysis in combination with principal component analysis (PCA) divided the methods, confirming a greater contribution of the Folch method. Therefore, Folch methodology revealed an effective lipid extraction in comparison to creamatocrit method.
Objetivo: Discutir as recomendações de doação de leite materno aos Bancos de Leite Humano (BLH) frente à pandemia COVID-19. Metodologia: A pesquisa foi realizada envolvendo publicações sobre BLH, leite humano doado (LHD) e COVID-19 indexados nas bases de dados eletrônicas PubMed (US National Library of Medicine), Scientific Electronic Library Online Brasil (SciELO), Web of Science e Portal Capes. Os descritores que delimitaram a pesquisa foram: amamentação, aleitamento materno, BLH, LHD, leite humano, COVID-19, pandemia e Sars-CoV-2 em português e inglês. Resultados e discussão: Como o vírus da COVID-19 é transmitido por gotículas respiratórias e o vírus sobrevive por períodos variados em superfícies inanimadas, neste trabalho observamos as principais medidas de controle de transmissão do vírus pelo LHD durante todo o processo de coleta, processamento e distribuição do leite humano até os recém-nascidos das Unidades de Terapia Intensiva Neonatal (UTIN). É necessário controle de qualidade com alto rigor higiênico-sanitário do LHD como esterilização dos materiais que entrarem em contato com o leite materno, higienização do ambiente onde será realizada a ordenha, utilização de gorro e máscara, lavagem das mãos com água e sabão e eliminação dos primeiros jatos de leite ordenhado. Conclusão: As doadoras de leite humano ordenhado devem realizar uma triagem nos BLH para avaliação e orientações antes de iniciar a doação de leite humano, a fim de evitar a transmissão do vírus da COVID-19 durante todo o processo de coleta, processamento e distribuição do leite humano até os recém-nascidos das UTIN.
Este estudo foi realizado para analisar o perfil lipídico, concentração de calorias, de sódio e água em amostras de colostro, leite de transição e leite maduro, para avaliar se as diferentes fases do leite humano são adequadas para serem oferecidas aos neonatos desidratados a termo. Foi escolhido delineamento transversal e quantitativo, utilizando 44 amostras de leite humano doados para o Banco de Leite Humano. Como critério de inclusão, foram selecionadas amostras de doadoras que amamentavam exclusivamente. Para avaliação dos perfis lipídicos foram preparados pools das três fases, que foram analisados com auxílio de espectrometria de massas com fonte de ionização por eletropulverização. As análises do teor calórico, sódio e umidade foram realizadas em amostras individuais utilizando-se metodologias oficiais da AOAC. Os perfis lipídicos indicaram que os pools podem apresentar importantes ácidos graxos que compõe os triglicerídeos presentes nos lipídios do leite humano. Os teores de sódio quando avaliados individualmente variaram significativamente e na maioria das amostras foram elevados, quando comparados aos valores de referência utilizados em UTIs neonatais. O teor médio de umidade do leite maduro foi maior quando comparado as demais fases, que apresentaram maiores teores de sódio e menores de umidade (p<0,05). A análise estatística por PCA separou as amostras se destacaram em relação ao teor deste mineral. Os perfis lipídicos indicam a presença de ácidos graxos essenciais das famílias ômega-6 e 3, importantes para o desenvolvimento neurológico dos neonatos. O teor médio de sódio encontrado no leite maduro está dentro dos limites adequados para neonatos desidratados.
