Đặt vấn đề: Việc xác định các yếu tố nguy cơ mới của bệnh mạch vành (BMV) ngày càng được nghiên cứu nhiều hơn nhằm mục đích phát hiện sớm và phòng ngừa căn bệnh nguy hiểm này. Nồng độ beta-2-microglobulin (B2M) huyết tương được báo cáo là có liên quan đến sự tiến triển của BMV. Mục tiêu: 1) Mô tả nồng độ B2M huyết tương trên bệnh nhân BMV tại Viện Tim mạch Bệnh viện Bạch Mai; 2) Phân tích mối liên quan giữa nồng độ B2M huyết tương với mức độ hẹp mạch vành trên bệnh nhân BMV. Đối tượng và phương pháp: Nghiên cứu mô tả cắt ngang trên 50 bệnh nhân được chẩn đoán xác định mắc BMV dựa trên kết quả chụp ĐMV qua da và 50 người khỏe mạnh được định lượng nồng độ B2M huyết tương bằng phương pháp miễn dịch đo độ đục trên máy Cobas c702 hãng Roche Diagnostics. Kết quả: Nồng độ B2M huyết tương ở nhóm bệnh nhân BMV (2,33 ± 0,47 mg/L) cao hơn so với nhóm chứng (1,84 ± 0,28 mg/L) có ý nghĩa thống kê với p<0,001. Nồng độ B2M huyết tương có mối tương quan thuận ở mức độ trung bình với số nhánh động mạch vành (ĐMV) hẹp và mức độ hẹp ĐMV trên bệnh nhân BMV với hệ số tương quan lần lượt là 0,469 và 0,481. Kết luận: 1) Nồng độ B2M huyết tương nhóm bệnh nhân BMV cao hơn nhóm chứng; 2) Có mối tương quan thuận ở mức độ trung bình giữa nồng độ B2M huyết tương với số nhánh ĐMV hẹp và mức độ hẹp ĐMV trên bệnh nhân BMV.
Bệnh lý thiếu hụt carnitin nguyên phát (CDSP) gây ra bởi một đột biến lặn trên gen SLC22A5 mã hóa cho protein vận chuyển carnitin (OCTN2) trong huyết thanh và được đặc trưng bởi triệu chứng hạ đường huyết giảm lượng ketone máu (hypoketotic hypoglycemia), bệnh lý về xương và nhược cơ tim có thể dẫn tới tử vong nếu không được điều trị kịp thời [1,2]. Bệnh nhân mắc bệnh thiếu hụt carnitin nguyên phát thường đáp ứng tốt với việc bổ sung L-Carnitin đường uống và cải thiện đáng kể các triệu chứng lâm sàng của họ [3]. Do đó, chẩn đoán sớm và can thiệp kịp thời có ý nghĩa rất quan trọng với bệnh nhân thiếu hụt carnitin nguyên phát. Xét nghiệm phân tích gen SLC22A5 mã hóa protein vận chuyển carnitin có giá trị chẩn đoán xác định bệnh thiếu hụt carnitin nguyên phát bởi kỹ thuật này xác định được chính xác các đột biến trên gen SLC22A5 ảnh hưởng trực tiếp tới việc tổng hợp protein vận chuyển carnitin. Ở Việt Nam hiện nay để xác đột biến gen SLC22A5 chủ yếu sử dụng các bộ mồi được tham khảo từ nghiên cứu trước đây trên thế giới tuy nhiên vẫn gặp phải sự không đặc hiệu về mồi dẫn đến khó xác định được đột biến. Nghiên cứu này tập trung vào thiết kế và tối ưu hóa bộ mồi đặc hiệu trong xác định đột biến gen SLC22A5 gây bệnh thiếu hụt carnitin nguyên phát ở người Việt Nam.
Dị tật bẩm sinh (DTBS) là một trong những bất thường hay gặp ở thai nhi và trẻ sơ sinh. Có nhiều nguyên nhân gây ra dị tật bẩm sinh, trong đó dị tật do rối loạn vật chất di truyền ở cấp độ nhiễm sắc thể (NST) có thể gây hậu quả nặng nề 3–4. Để chẩn đoán các bất thường về NST, trước khi thực hiện các kỹ thuật xâm lấn, các phòng xét nghiệm thường chỉ định các xét nghiệm sàng lọc như double test ở thai kỳ I hoặc triple test ở thai kỳ II. Đối với xét nghiệm double test, các kết quả định lượng β-HCG và PAPP-A máu, kết hợp với kết quả siêu âm thai như khoảng sáng sau gáy (NT), chiều dài đầu mông (CRL),... đo được sẽ chuyển sang bội số trung vị MoM theo tuần thai của quần thể, từ giá trị MoM tính nguy cơ mắc bệnh của từng thai theo tỷ lệ xác suất có thể gặp. Tuy nhiên giá trị này có thể thay đổi theo các yếu tố đặc trưng của từng quần thể 7. Một số nghiên cứu gần đây đã chỉ ra sự sai khác của giá trị trung vị quần thể nghiên cứu với các giá trị có sẵn của phần mềm, từ đây giúp cải thiện độ đúng và độ tin cậy của các giá trị sàng lọc. Do đó chúng tôi làm nghiên cứu này để xác lập giá trị trung vị MoM của một số chỉ số hóa sinh trong sàng lọc trước sinh (β-HCG, PAPP-A và NT). Chúng tôi tiến hành khảo sát trên 1265 thai phụ được làm xét nghiệm double test có tuần thai tương ứng từ 11 tuần đến 13 tuần 6 ngày. Kết quả cho thấy, có sự sai khác giữa giá trị trung vị β-HCG, PAPP-A, NT trong nghiên cứu của chúng tôi và giá trị trung vị đang được dùng trên các phần mềm, giá trị cụ thể được thể hiện trong nội dung bài. Giá trị trung vị MoM sau khi được thiết lập có thể ứng dụng cho việc sàng lọc trước sinh ở những thai phụ người Việt Nam với hiệu quả tốt hơn.
