Trong thời gian từ tháng 01/2016 đến tháng 12/2021 có 1641 người bệnh được chỉ định cấy máu, 165 người bệnh có kết quả cấy máu dương tính, đã phân lập được 177 chủng vi khuẩn và vi nấm, trong đó có 12 người bệnh phân lập được 2 loại tác nhân gây bệnh trong quá trình điều trị. Trong tổng số 1641 người bệnh được chỉ định cấy máu, tỷ lệ người bệnh cấy máu dương tính là 10.05%. Các mẫu dương tính đều được định danh và làm kháng sinh đồ. Kết quả nghiên cứu cho thấy tỷ lệ vi khuẩn Gram âm gây nhiễm khuẩn huyết là 64,4%, nấm 23,2%, vi khuẩn Gram dương 12.4%. Trong nhóm vi khuẩn Gram âm gây nhiễm khuẩn huyết đứng đầu là Aci.baumannii (20.34%); tiếp đến là P.aeruginosa (14,68%); C. tropicalis (10.73 %). Trong nhóm vi khuẩn Gram dương gây nhiễm khuẩn huyết, căn nguyên chiếm đa số là Staphylococcus aureus (7,91%).
Thalassemia là bệnh lý di truyền gây thiếu máu tan máu. Truyền máu thường xuyên thường liên quan đến tình trạng quá tải sắt ở các cơ quan. Để điều trị tình trạng thừa sắt đòi hỏi phải đánh giá chính xác tình trạng ứ sắt của cơ quan. Mục tiêu: đánh giá tình trạng quá tải sắt và mối tương quan giữa ferritin huyết thanh, nồng độ sắt trong gan (LIC- liver iron concentration) và T2* tim dựa trên cộng hưởng từ (MRI) của bệnh nhân thalassemia tại bệnh viện Nhi Trung ương năm 2018-2020. Đối tượng, phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang, từ 8/2018- 8/2020, chúng tôi thu thập được 79 bệnh nhân nhi thalassemia truyền máu định kỳ tại bệnh viện Nhi Trung ương. Tiến hành đánh giá đặc điểm lâm sàng, xét nghiệm và chụp MRI đánh giá tình trạng ứ sắt ở gan (LIC) và tim (T2*). Kết quả nghiên cứu: Ferritin huyết thanh trung bình (3298.6 ± 3055,2 ng/ml) và LIC trung bình (12.90 ± 5,6 mg/g) đều ở mức trung bình nặng, T2* tim trung bình là 29 ±11,7 ms. Ferritin huyết thanh có mối tương quan yếu với LIC và T2* tim (tương ứng với r= 0,441 và -0,32), LIC và T2*tim không có mối tương quan. Kết luận: Tình trạng quá tải sắt ở gan ở mức cao, nhưng không tương quan với tình trạng quá tải sắt ở tim. Ferritin có mối tương quan yếu với LIC và T2* tim.
Dị tật bẩm sinh (DTBS) là một trong những bất thường hay gặp ở thai nhi và trẻ sơ sinh. Có nhiều nguyên nhân gây ra dị tật bẩm sinh, trong đó dị tật do rối loạn vật chất di truyền ở cấp độ nhiễm sắc thể (NST) có thể gây hậu quả nặng nề 3–4. Để chẩn đoán các bất thường về NST, trước khi thực hiện các kỹ thuật xâm lấn, các phòng xét nghiệm thường chỉ định các xét nghiệm sàng lọc như double test ở thai kỳ I hoặc triple test ở thai kỳ II. Đối với xét nghiệm double test, các kết quả định lượng β-HCG và PAPP-A máu, kết hợp với kết quả siêu âm thai như khoảng sáng sau gáy (NT), chiều dài đầu mông (CRL),... đo được sẽ chuyển sang bội số trung vị MoM theo tuần thai của quần thể, từ giá trị MoM tính nguy cơ mắc bệnh của từng thai theo tỷ lệ xác suất có thể gặp. Tuy nhiên giá trị này có thể thay đổi theo các yếu tố đặc trưng của từng quần thể 7. Một số nghiên cứu gần đây đã chỉ ra sự sai khác của giá trị trung vị quần thể nghiên cứu với các giá trị có sẵn của phần mềm, từ đây giúp cải thiện độ đúng và độ tin cậy của các giá trị sàng lọc. Do đó chúng tôi làm nghiên cứu này để xác lập giá trị trung vị MoM của một số chỉ số hóa sinh trong sàng lọc trước sinh (β-HCG, PAPP-A và NT). Chúng tôi tiến hành khảo sát trên 1265 thai phụ được làm xét nghiệm double test có tuần thai tương ứng từ 11 tuần đến 13 tuần 6 ngày. Kết quả cho thấy, có sự sai khác giữa giá trị trung vị β-HCG, PAPP-A, NT trong nghiên cứu của chúng tôi và giá trị trung vị đang được dùng trên các phần mềm, giá trị cụ thể được thể hiện trong nội dung bài. Giá trị trung vị MoM sau khi được thiết lập có thể ứng dụng cho việc sàng lọc trước sinh ở những thai phụ người Việt Nam với hiệu quả tốt hơn.
Đặt vấn đề: Bệnh viêm da cơ địa là một đáp ứng viêm đặc biệt của da đối với cả hai yếu tố nội và ngoại sinh. Các yếu tố ngoại sinh bao gồm: Khí hậu, môi trường, hóa chất, ánh sáng tia cực tím mặt trời, mỹ phẩm, thực phẩm... Các yếu tố nội sinh như: di truyền, rối loạn tiêu hóa mạn tính, nhiễm trùng, rối loạn chuyển hóa, rối loạn chức năng nội tiết. Viêm da cơ địa là gánh nặng về tâm lý, xã hội cho bản thân và gia đình người bệnh. Các chỉ số sinh học trên da của người mắc bệnh viêm da cơ địa có khác biệt so với người bình thường. Mục tiêu: Mô tả một số chỉ số sinh học trên da của bệnh nhân viêm da cơ địa và xác định mối tương quan giữa các chỉ số sinh học này với các giai đoạn lâm sàng của bệnh viêm da cơ địa. Phương pháp: Thiết kế nghiên cứu mô tả cắt ngang, phương pháp nghiên cứu mô tả và mô tả tương quan trên 30 người bệnh viêm da cơ địa, có đối chứng (n= 30). Kết quả: Định lượng một số chỉ số sinh học trên da của bệnh nhân viêm da cơ địa bằng phương pháp đo trên máy phân tích lượng tử cộng hưởng từ cho thấy có hiện tượng rối loạn một số chỉ số (tăng hoặc giảm) so với người bình thường. Các chỉ số sinh học trên da ở bệnh nhân viêm da cơ địa tăng gổm: chỉ số miễn dịch da tăng 63,3% (3,91 ± 1,62), gốc tự do 53,3% (3,91 ± 1,83), mất độ ẩm da 60,0% (4,94 ± 1,55), mỡ 80,0% (25,42 ± 4,81), sừng da 83,3% (2,30 ± 0,55), melanin 96,7% (0,60 ± 0,15); Các chỉ số giảm: collagen giảm 90,0% (3,20 ± 0,89), đàn hồi da 50,9% (2,30 ± 0,69); Sự khác biệt có ý nghĩa thống kê (với p<0,05,... và p<0,05). Có mối tương quan thuận giữa chỉ số giãn mao mạch da với các giai đoạn lâm sàng (tương ứng với các mức độ từ nhẹ,...đến nặng) ở bệnh nhân viêm da cơ địa (R = 0,44, với p<0,05). Kết luận: Có hiện tượng tăng một số chỉ số sinh học như miễn dịch da, gốc tự do, mất độ ẩm da, mỡ da, sừng da, melanin và giảm ở một số chỉ số: collagen, đàn hồi da; Có mối tương quan thuận giữa chỉ số giãn mao mạch da với các giai đoạn lâm sàng trên bệnh nhân viêm da cơ địa
